Your Job as a Substance Use Counselor:
The Substance Use Counselor is instrumental in our patient's treatment and recovery from opioid use disorder. Substance Use Counselors provide high quality, compassionate care through various mediums which include both individual and group counseling sessions.
Job Responsibilities:
Provide high quality, compassionate guidance in both individual and group counseling sessions.
Plan, oversee, facilitate and document patient's recovery.
Co-facilitate assigned group or family sessions as needed.
Ensure all documentation regarding patient care, treatment, and incidents is completed timely and in a clear, concise manner.
Prepare individual treatment plans for each assigned patient.
Initial assessments as well as follow up assessments.
Evaluate patient needs and determine if referrals to other programs or facilities are needed.
May plan for aftercare for assigned patients.
Provide crisis intervention to patients, as needed.
Provide case management duties for patients, ensuring individualized quality care as needed.

Compensation for roles at Riverside CTC varies depending on a wide array of factors including but not limited to the specific location, role, skill set, and level of experience. Required Education, Skills, and Qualifications:
HS graduate or equivalent required;
Prefer Bachelor's or Master's degree in social or health services field;

* Familiarity with Medication-Assisted Treatment (MAT), including methadone, buprenorphine, and naltrexone.
* Experience conducting individual and group counseling sessions focused on substance use recovery.
* Knowledge of evidence-based practices, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and relapse prevention techniques.

The On-Call SUD Counselor is a registered or certified substance use disorder professional who maintains registration from an approved/accredited California agency. The Counselor works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360's philosophy, where program schedule requires additional staff. The On Call SUD Counselor provides supervision and oversight of the clients and children throughout the facility with shifts ranging to provide 24/7 coverage.
The On-Call SUD Counselor is an integral part of an interdisciplinary team that supports our clients through assessment of their needs and strengths in support of their health and recovery. On-Call SUD Counselor will perform counseling in individual and group setting, deliver curricula in accordance with program specific requirements and case management, and assist clients in navigating systems of care while providing a supportive treatment environment.

KEY RESPONSIBILITIES
Facilitate group counseling sessions, community meetings, and teach classes on various treatment-related topics to assist clients in meeting their treatment goals.

Properly document all group counseling sessions and all individual sessions as needed within 24-72 hours per contract requirements.
Must meet residential service delivery goal of 60%.

Document client updates and incidents daily.

Support intake process such as preparing rooms and maintaining supplies.
Perform property and room searches as needed.
Respond to overdose and other medical emergencies.

Observe clients every 30 minutes and document on observation log, per program requirements.
Collect, clean, or dispose of biohazard waste.
Disinfect and clean rooms and common areas as needed.
Accurately enter data in various electronic systems for all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.

The El Monte FOTEP is a residential program serving women and children. All of the women that are enrolled in the program are on State parole and usually have extensive histories with the criminal justice system. Some of the women have co-occuring disorders that are managed by prescribed medications and individual therapy. The program is very fast paced and is a very structured environment.
Key Responsibilities
The primary duties of the Intake Counselor is to welcome new arrivals and complete the necessary paperwork to enroll them in the program. The Intake Counselor needs to be a person that can connect with people from diverse backgrounds and be able to explain the expectations of the program and their own rights and responsibilities as a client.
Conduct all intakes with all new admissions.
Screen all new intakes for health and safety reasons.
Enter client information into EHR systems.
Ensure that intake binders are distributed to new arrivals within 24-48 hours.
Ensure that all new arrivals understand their personal rights and responsibilities as a client of HR360.
Ensure that all new arrivals are oriented to the program within their first 24 hours of admission.
Ensure that new arrival room is in order for all new admissons.
Ensure that all new arrivals of aware of COVID policy.
Ensure that all new arrivals are scheduled for covid testing whether or not they come fully vaccinated.
Education and Knowledge, Skills and Abilities
The Intake Counselor must be certified or have an up to date registration.
The Intake Counselor must be familiar with residential programs.
The Intake Counselor must have a welcoming demeanor and is able to get along well with others.
The Intake Counselor must be able to navigate computer systems and enter accurate information into EHR systems.
The Intake Counselor must be able to pass a CDCR clearance.
The Intake Counselor must have some knowledge of behaviors associated with the person served.
The Intake Counselor must have good writing skills.

We are looking for full-time Social Services Assistant.
Directs the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations and Company policies and procedures to assist each resident and family adjust to placement, illness, and plan of care so as to attain the highest practicable level of functioning. Duties and Responsibilities
Assists in interviewing and obtaining information from residents and families to assist Social Services Director or Social Services consultant in development of psychosocial history, assessment and care plan upon admission, quarterly and upon change of condition for social service needs.
Assists resident and family with transition and adjustment to placement in facility.
Assists with determination of legal status of residents upon admission, provides information and obtains copies of advance directives, assures that a resident and family wishes regarding treatment (including Do Not Resuscitate (DNR) and withholding treatment) are implemented in accordance with Company policy.
Informs the resident/family of resident's personal and property rights.
Provides input on social, psychological, emotional needs of resident/family to supervisor.
Ensures residents have required evaluations for visual, audio, mental health and dental needs, and follows up as appropriate.
Arranges residents' appointments including transportation.
Knowledge, Skills, Abilities & Qualifications
Previous experience with geriatric population or in long-term care setting strongly preferred.
High School diploma or equivalent required: AA or Bachelor's degree preferred.
Ability to interact tactfully, effectively, and professionally with other employees, residents, family members, visitors, government agencies/personnel and the general public, as needed
Must be able to relate to and work with physically and/or mentally ill, disabled, elderly, emotionally upset, and at times hostile people within facility and treat them with respect and consideration regardless of their cognitive or functional level
Willingness and ability to function as an effective team member through good communication and cooperation to meet departmental goals
Must be able to multi-task and handle pressure
Willing to work beyond normal working hours, weekends, holidays, and/or on other shifts and in other positions, when necessary
Sufficient computer skills including proficiency in Microsoft Office, Outlook, and /or Internet to perform required duties
Ability to exercise discretion and independent judgment, and reach logical, practical decisions
