Purpose of Position- Serves as a personal advocate for residents with intellectual and developmental disabilities (IDD) in the house. Coordinates and documents services received to ensure fulfillment of Person Centered Support Plans. Provides for the health, safety and welfare of the residents in their homes and in the community. Implements residents' Person Centered Support Plans. Monitors and reports on data to ensure compliance with all applicable regulating agencies. Maintains professional relationships with residents as well as members of treatment teams.
