Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.

Requirements:
Active and unrestricted license to practice as a Licensed Professional Counselor in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.

DUTIES AND RESPONSIBILITIES:
1. Provide counseling services to members in Drug & Alcohol program
2. Complete Intake, treatment plans, evaluations and discharges on time as prescribed in the policies
3. Provide Individual and group therapies consistently and family therapy when needs
4. Use ASAM evaluation at the beginning and at every treatment plan sessions
5. Complete documentation on time as prescribed in the policies and procedure manual
6. Ensure all members provide UDS every week without fail
7. Apply for BHSI for clients who doesn't have insurance
8. Co-ordinate with Septa logistics care for bus passes for members to come to facility
9. Co-ordinate with Psychiatrist for evaluations, suboxone prescription for members and case consultations
10. Work closely with supervisor and complete required supervision hours and case consultations
11. Ensure all documentation and data are complete and in compliance with APM, city, state, and federal regulations
12. Makes referrals to appropriate levels of care based on ASAM criteria
13. Maintains strong interagency community, multidisciplinary and government agencies relationships.
14. Maintains ongoing communication with other community treatment facilities, primary care physicians, hospitals, schools, multidisciplinary units, and Probation/Parole officers, the Forensic Intensive Recovery (FIR) unit, etc. as necessary.
15. Maintains a caseload that supervisor deemed appropriate in your full time position
16. Participate in meetings with the agency interdisciplinary team as clinically necessary.
17. Participate and maintain required CE credits and other mandatory trainings
18. Ensure all charts are up to date
19. Actively engage in program expansion and development
20. Works closely with the Clinical Director and set quarterly targets and achieve them
21. Must be able to provide training and supervision if more counselors are hired for DA program
23. Provides updated FBI, PA criminal background, child abuse, CPSL and CPR clearances before the start date
25. Performs other program related duties as assigned by the Clinical Director.

Ability to take initiative and collaborate with other resources, agencies, hospitals and judiciary system in the community

QUALIFICATIONS:
Bachelor's Degree in the human services or related field with a minimum of 1-year work experience with clients in a rehabilitation, social services, mental health or related services.
CAC, CDAC licensed or qualified
Completed ASAM training
Trained in Motivational Interviewing
Bilingual (English/Spanish)
Proficiency in the DAP documentation
Proficiency in the use of Microsoft Word
Experience in facilitating group therapy
Working with medication managed populations
Strong verbal and written communication skills

Addictions Counselor
Counselor internship job in Philadelphia, PA
The Caring Together Addictions Counselor provides direct counseling services and appropriate documentation for women enrolled in the Caring Together Program. Duties include intake evaluations, biopsychosocial evaluations, individual and group counseling, recovery planning, including treatment plans and ensuring that all appropriate documentation and clinical notes are completed in the patient charts within the required timeframe. Knowledge and acceptance of medication assisted treatment (MAT) is required.
Essential Functions
Provides direct client services including individual and group counseling for a caseload.
Maintains timely& accurate documentation in client files, including individual and group progress notes, treatment plans, discharge summaries, aftercare plans as well as related client correspondence.
Ensure all notes, releases of information, intake paperwork, including ASAM and treatment plans are filed in chart and up to date Ensures confidentiality with all cases. Coordinates services with all patients' ancillary services.
Meets productivity targets set in collaboration with the program manager.
Ensures accurate billing.
Provides expert consultation to staff and community on women with addictive and psychiatric disorders.
Attend weekly staff and clinical supervision meetings to present issues and collaborate with the treatment team.
Coordinates services for mothers in the program to ensure appropriate childcare services are available.
Coordinates psychiatry and case management services for all patients on caseload.
Participate in treatment team meetings. Performs other duties as assigned.

Required Qualifications
Minimum of Masters Degree in Human Services. (or equivalent experience).

Additional Information
This position is classified as an exempt grade I. Compensation for this grade ranges from $ 41,120.00 to $61,680.00 per year. Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate's qualifications and experience, department budget, and an internal equity review.
