Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Professional Counselor in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.

•Assists consumers with applying for appropriate financial assistance, developing a budget, establishing checking/savings accounts, and preparing tax forms, rebates, etc.
•Educates and assists consumers with daily living skills such as, household chores, laundry and personal hygiene. Hands on assistance is required for residents who require more than verbal reminders.
•Assesses, educates, and assists consumers with public transportation, which includes accompanying them on the bus when necessary.
•Provides orientation to the local community with location of pharmacies, grocery stores, recreational facilities, hospitals, doctors, and dentists.
•Educates consumers on emergency procedures such as fire safety, natural disasters, loss of utilities, violence and abuse
•Teaches and encourages the development of social skills through role-plays and participation in recreational activities.
•Communicates with treatment team members and keeps members informed of consumer's progress.
•Attends treatment team and all other scheduled meetings as deemed appropriate. Attends all monthly staff meetings.
•Provides internal and external liaison to appropriate service providers in accordance with the consumer's service plan.
•Completes hourly safety/maintenance checks in all bedrooms, bathrooms and common areas and documents results. Immediately notifies Supervisor of concerns/discrepancies.
REQUIRED MINIMUM QUALIFICATIONS:
•Bachelor's degree in Social/Human Science field with six (6) months related experience preferred, will consider combination of education and experience.
