JOB OBJECTIVE
The Case Manger (CM) will work in office, and/or field settings supporting our Clinicas del Camino Real membership. Performs care management duties to assess, plan and coordinate aspects of medical and supporting services across the continuum of care for member identified to be medium rising risk or high utilizers. This position works with the care management and coordination teams to identify care medical and social needs and to establish support services to promote quality member outcomes.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
The case manager plays a primary role in working with medium rising risk members or high utilizers to improve their health outcomes through medical and social care coordination approaches, community engagement, and targeted programs to support the unique needs of each assigned case management member. Case management strategies will be driven by incorporating the member's ethnicity, age, specific chronic health conditions, social designations such as SUDs, prisoners, survivors of abuse, and those with mental health issues.
The fundamental principles of case management include:
Assessment- involves a comprehensive collection of data and ongoing collaborative process of observation, information gathering, reflection and engagement with the client, and analysis of the client's overall situation and needs.
Member Centric- actively participate in and are at the center of decision making and support planning. This fosters trust and ensures that the services provided align with the Members' expectations and needs by Empowering them to take an active role.
Planning to establish specific objectives and care goals, both short- and long-term, based on the client's needs identified earlier in the assessment process.
Effective Communication
Individual CM
extends to writing concise and accurate reports, documentation, and referrals and leveraging technology to facilitate collaboration.
Members
utilize effective listening to understand the Member's needs, concerns, and feedback, ensuring that they feel heard and respected.
Care Team promote seamless communication among all team members supported by regular meetings and shared data methodologies.
Conduct Regular Evaluations- Regularly evaluate the case management processes to identify strengths and pinpoint areas for improvement.Such as encourage regular client check-ins and perform ongoing updates and activities resulting from CM arrangements such as PCP, Specialty and Community encounters.
Advocacy Skills. As the case manager, to advocate on behalf of assigned CM members to ensure they receive the services and support to promote quality CM outcomes.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES (Continued)
The Case Manager works within a team environment to:
Develop, guide and provide case management services by utilizing clinical expertise, and coordination efforts with the medical management team, community supports and contracted upstream health plan.
Analyze medical trends and intervene with members identified to be at rising medium risk and/or high utilizers.
Interact with members' primary care physicians, and specialists to provide seamless case management activities across the care continuum,
Interact with community/system resources, and maintain resource directories of collaborative and supporting agencies,
Establish individualized member care plans that incorporate prioritized case goals, implementation plans, and ongoing measurement of outcomes,
Access and utilize data from various data systems to create a comprehensive member case file that is supported with member assessments, clinical reports, encounters, community appointments, medication management, and identified SDOHs.
Collaborate with healthcare professionals to assess, plan, and coordinate patient care needs.
Participate in quality improvement activities to enhance clinical outcomes and resource utilization.
Collect and analyze patient information for third-party vendors,
Arrange or participate in member case conferences internally at CDCR, externally with GCHP and community agencies.
Submit timely progress reports and updated member care plans and profiles as necessary to support continuum in the member's case management process.
Performs telephonic assessment to assess member needs and collaborate with resources and provides education/support for treatment regimen adherence and medication management to support self-management and independent living.
Identifies potential care gaps and makes referrals as appropriate.
Identifies available community services and health resources and facilitates access to care and services available to patient/family when needed.
Collects, documents, and maintains all member information and care management activities to ensure compliance with current state, federal, and third-party payer regulations
Makes referrals for Complex Case Management, Enhanced Case Management, or Basic Case Management.
Provides feedback to leadership on opportunities to improve and enhance care and quality delivery for members in a cost-effective manner
Attends all required team/staff meetings.
Performs other duties as assigned by people leader to meet business needs
EXPERIENCE AND EDUCATION REQUIREMENTS
Any of the following:
Completion of an accredited Registered Nurse (RN) program or accredited Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) Program.
1-3 years in case management, disease management, managed care or medical or behavioral health settings.
Valid RN or LVN/LPN license is required for the job. License must be active, unrestricted and in good standing.
A valid driver's license with good driving record and be able to drive within applicable state or locality with reliable transportation.
PREFERRED EXPERIENCE:
3-5 years in case management, disease management, managed care or medical or behavioral health settings.

Key Responsibilities:
Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.
Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies.
Involve patients in the assessment, treatment planning, and intervention aspects of treatment, which provides the patient with a sense of entitlement to their own recovery.
Administer the biopsychosocial assessment, which evaluates all areas of patients' lives, including their addictions, medical and mental health history, socioeconomics, legal, etc. at intake and on an ongoing basis.
Recognize and utilize patients' Strengths, Needs, Ability, and Preferences (SNAP), to develop and implement individualized Treatment Plans that effectively help patients to meet their short & long-term goals. • Determine the clinical necessity of counseling services based on the clinical assessment and evaluation of the patients.
Monitor closely, and document any change of circumstances with the patients, especially as it relates to matters that may affect their recovery, identifying and addressing relapse indicators to promote relapse prevention interventions in an early stage.

Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition).
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.
Engaging in Community Relations and Outreach efforts.
Attending all recommended training including Physician Training of Counselors, RCL training, and case conferences with the program physicians and physician assistants.
Collecting research data when requested.
Observing the collection of patients' urine samples when required
Attend team meetings and complete all training courses timely as required.
Other duties as assigned.
Qualifications:
Counselor I: $20/hour to $22/hour
Possess an associate degree in relevant field and has the registration/credentials necessary to provide counseling services to our patient population.
Kern County requires a bachelor's degree or High school diploma/GED with 75% certification education.

Counselor II: $22/hour to $25/hour

Qualifications
EXPERIENCE:

Some examples include working with typically developing children in a group setting (children with demonstrated deficits or challenging behavior preferred), having a family member with ASD, a neighbor with a developmental disability, or even volunteer work in a classroom with a child with ASD, etc.
EDUCATION:
Degree not required, but must have at least completed some college coursework or be currently enrolled at an accredited college or university. We have a preference to programs related to early childhood education, psychology, nursing or related field.

POSITION SUMMARY:
Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. The master's level social worker represents Holt in their community and ensures the successful adjustment of child and family by conducting an assessment of the family and providing appropriate services throughout the adoption process. The master's level social worker works cooperatively with families, the Branch Office and Adoption Teams headquartered in Eugene, Oregon. Note: The Adoption Social Worker is primarily a home-based position. The Adoption Social Worker position requires travel (most often by car) to different locations within the assigned area to complete the essential functions of the position.
ESSENTIAL JOB FUNCTIONS (to be completed within timeframes outlined by the Branch Director):
1. Determines a family's ability and capacity to parent an adopted child by conducting pre-home study meetings as requested, reviewing and assessing agency-requested information, evaluating the family's knowledge and perceptions of adoption, teaching adoption education courses as needed, and conducting psycho-social assessments of family members.
2. Contributes to the delivery of high-quality adoption services by producing adoption home studies and post placement reports that comply with provided guidelines and meet current agency, state, country and USCIS requirements.
3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database.
4. Prepares family for travel and assists with post placement adjustment by providing training and identifying resources available to help meet the child's needs and aid in the child's adjustment to the family. Conduct home visits as required by state law and country program guidelines, assist in early identification of problematic issues, and make a determination regarding the stability and appropriateness of the placement.
5. Assures child's physical and emotional well-being and eases family's adjustment by providing counseling to the child and family and/or helping arrange for counseling with a clinician outside the agency if needed. In case of an adoption disruption or dissolution, maintain relationship with family to ensure transition plan is developed that meets the child's needs. Assist child and family with transition of child to new placement.
6. Contributes to public knowledge about Holt and supports Holt programs by participating in activities arranged by Holt including picnics or parent groups, representing Holt at adoption meetings, coalitions and organizations, and performing other outreach activities as appropriate.
7. Maintains professional knowledge by attending relevant seminars and course work, keeping informed about State regulations, legislation, adoption issues and child-related issues overseas; maintaining license, if required.
8. Contributes to a positive, productive work environment by meeting attendance and punctuality guidelines and pre-arranging time off with appropriate notice; treating all people with dignity and respect; and exhibiting good listening and comprehension skills including giving and welcoming feedback.

Responsible for supervision of clients, evaluating immediate client need, responding to client needs, and directing them to services into treatment with them, so they do not have to separate from their children to get services. Our goal is to help stabilize women in their recovery and transition them to outpatient services, stable housing, a new support group and employment/school.

Education and Knowledge, Skills and Abilities
High School Diploma/GED.
Minimum of one year experience working with substance abusing and/or mentally ill population.
Ability to provide proof of current registration from one of the addiction counselor certification boards in California.
Enrollment or completion of a State approved addiction education program working toward State certification as an addiction counselor.
Willingness to work and learn in a community-based setting emphasizing integration of alcohol, drug, and mental health treatment concepts.
Dependable automobile and insurance registration with valid CA Driver's License.

POSITION SUMMARY:
Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. The master's level social worker represents Holt in their community and ensures the successful adjustment of child and family by conducting an assessment of the family and providing appropriate services throughout the adoption process. The master's level social worker works cooperatively with families, the Branch Office and Adoption Teams headquartered in Eugene, Oregon. Note: The Adoption Social Worker is primarily a home-based position. The Adoption Social Worker position requires travel (most often by car) to different locations within the assigned area to complete the essential functions of the position.
ESSENTIAL JOB FUNCTIONS (to be completed within timeframes outlined by the Branch Director):
1. Determines a family's ability and capacity to parent an adopted child by conducting pre-home study meetings as requested, reviewing and assessing agency-requested information, evaluating the family's knowledge and perceptions of adoption, teaching adoption education courses as needed, and conducting psycho-social assessments of family members.
2. Contributes to the delivery of high-quality adoption services by producing adoption home studies and post placement reports that comply with provided guidelines and meet current agency, state, country and USCIS requirements.
3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database.
4. Conduct home visits as required by state law and country program guidelines, assist in early identification of problematic issues, and make a determination regarding the stability and appropriateness of the placement.
5. Assures child's physical and emotional well-being and eases family's adjustment by providing counseling to the child and family and/or helping arrange for counseling with a clinician outside the agency if needed. In case of an adoption disruption or dissolution, maintain relationship with family to ensure transition plan is developed that meets the child's needs. Assist child and family with transition of child to new placement.
6. Contributes to public knowledge about Holt and supports Holt programs by participating in activities arranged by Holt including picnics or parent groups, representing Holt at adoption meetings, coalitions and organizations, and performing other outreach activities as appropriate.
7. Maintains professional knowledge by attending relevant seminars and course work, keeping informed about State regulations, legislation, adoption issues and child-related issues overseas; maintaining license, if required.
8. Contributes to a positive, productive work environment by meeting attendance and punctuality guidelines and pre-arranging time off with appropriate notice; treating all people with dignity and respect; and exhibiting good listening and comprehension skills including giving and welcoming feedback.

Job Overview
Our SUD CounselorInternship program is specifically for students enrolled in a Drug and Alcohol Studies Program through an accredited educational program. During their time in our programs, interns are able to observe the daily activities of a residential or outpatient SUD treatment program.
