About the Role
The CSAC will deliver individual and group counseling, facilitate treatment planning, and support relapse prevention strategies. This position works under the supervision of a Licensed Substance Abuse Treatment Practitioner (LSATP) or Licensed Clinical Social Worker (LCSW) to ensure compliance with Virginia Department of Health Professions, Medicaid, and DBHDS standards.
Key Responsibilities
Provide individual and group counseling sessions to clients with substance use and co-occurring disorders
Complete assessments, intake forms, and treatment plans in compliance with regulations
Maintain accurate and confidential documentation in the electronic health record (EHR)
Support client recovery through relapse prevention strategies and education for clients and families
Collaborate with supervisors, case managers, and community providers to coordinate care

Minimum Requirements:

This position has the primary responsibility of providing the full breadth of Child and Youth Behavioral (CYB) counseling services to children of all ages of military service members and their families at military installations and off military installations. These services may include non-medical counseling, training/ health and wellness presentations, consultation with parents, personnel, schools, and consultation to installation command regarding behavioral health issues affecting children and youth. CYB School counselors will be assigned to Department of Defense (DoDEA) schools and community schools and will need to have experience working in a school setting to qualify for a school position. CYB School counselors will also be assigned to youth programs, summer camps, and on demand assignments. The counselors work closely with the installation and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation and the schools.
* Provides non-medical, short-term, solution focused, counseling directly to eligible children and youth of service members at assigned primary and secondary locations. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
* CYB counselors will also have a focus on supporting the staff and personnel of DoDEA , community schools, summer camps (if available/assigned) as well as providing parent training and guidance.
* Provides training and health and wellness presentations, participates in health fairs and other base/installation activities focused on children and youth.
* Enters counselor activity data daily through smart phone, or web application, to assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service/family member.
* Establishes and maintains working relationships with community resources focused on children and youth and provides appropriate linkages. Consistently visible within the school which allows MFLC services to be accessible when working with children, the counselor must abide by line of sight protocol (another adult with responsibility for the child must be able to see the counselor and child they are working with at all times).
* Partners with POC to provide CYB services in a manner that addresses the needs of the installation/facility. Develops an effective and professional working relationship with the installation/facility POC.
* Manages duty to warn and restricted reporting situations according to DoD protocol and staffs the cases with Regional Supervisor/Regional Director.
* Communicates with Regional Supervisor and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations and in facilities to which they are assigned. Responds to critical incidents and special requests as directed by the POC and approved by the Office of Secretary of Defense (OSD) program manager. The CYB will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director.
* Participates in regular in-services/training, Quality Improvement committees, initiatives, studies, pilot programs other contract activities as assigned/appropriate as directed by the customer and/or Magellan. This includes participation in pilot program certification and training processes, completion of activity documentation, integration of pilot activities in non-medical counseling work, and the utilization of applicable technology to complete required activities.

Master's Degree from an accredited Graduate Program in a Mental Health related field.
Valid unrestricted independent counseling license from a State, District of Columbia, US Territory or Commonwealth that grants authority to provide counseling services as an independent practitioner. A minimum two years of post-licensure clinical experience
Demonstrate intermediate-level competencies in technology and software (i.e., Microsoft Suite, Internet navigation, Smartphone/Tablet and application navigation, video-conferencing platform navigation).
Requires ability to engage and communicate with military members or children as assigned, in order to accomplish job functions, and to respond quickly to emergent situations in any physical location on a military installation and/or within a school setting, which includes the need to traverse short and/or long distances within the base to both indoor and outdoor locations, to maneuver through rugged, outdoor or uneven locations (e.g., steep inclines, stairs, grass), and work in outdoor weather and other military base conditions. These cases range from assault and battery, sexual assaults, stalking, abductions, weapon offenses, and family abuse. In addition to time spent in court, the attorney also meets with victims and their supportive family members, to "break the cycle" of domestic violence - this is typically done with our office's Victim Services Unit. The part-time grant funded domestic violence attorney also participates in training with local law enforcement regarding the intricacies of effectively investigating domestic violence cases for successful case prosecution.
The project period for this grant is 7/1/2025 through 6/30/2026.
This is a part-time position compensated at $36.25 per hour, for up to 29 hours per week.

This role is responsible for performing field investigations and surveillance and requires possession of a valid state driver's license.
Duties will include alerting people to their health risks, behavior changes needed to reduce risks to the individual or to others, resources and methods available to treat and control the disease as well as to contain further transmission of disease. Conduct Follow up on contacts to determine if they have received treatment, changed their behavior to reduce health risk, and procured other available services as necessary to ensure improved health care.

2. Identify community changes and opportunities such as legislation, regulations and programs that affect nursing home residents.
3. Provide information to residents/families/responsible party as to Medicare and Medicaid, and other financial assistance programs available to the resident.
4. Ensure that all residents upon admission have an accurate Preadmission Screening and Resident Review (PASRR) and that the record is updated and maintained as needed with change in resident condition.
5. Communicate and coordinate assistance with Medicaid applications as needed.
6. Ensure that all new residents/family/responsible party on admission have been given resident rights.
7. Issue and ensure proper notice is given and procedure is followed including time frames, for Notices of Medicare Non Coverage, Skilled Nursing Facility Advanced Beneficiary Notice and Transfer and Discharge Notices.
8. Review complaints and grievances made by the resident and make a written or oral report to the Director and Administrator indicating what action(s) were taken to resolve the complaint or grievance. Follow facility's established procedures.
9. Review with the resident/responsible party the resident's advanced directives/code status on admission, quarterly, and with changes in condition.
10. Educate the resident and responsible party regarding rights related to advanced directives. Assist the resident/responsible party and coordinate with nursing and the Attending Physician the resident/responsible party regarding the resident's wishes and expectation for advanced directives/code status.
11. Assist the resident in completing Health Care Proxies, Durable Power of Attorney, and Living Wills.
12. Utilize the state designated orders for life sustaining treatment, i.e. DMOST, POLST, MOLST, and facility forms as directed by the Administrator and Medical Director.
13. Educate the resident/responsible party and the interdisciplinary care team on resident's bed hold rights in accordance with the regulations specific to Federal and State guidelines.
14. Coordinate resident room changes including documentation in the resident's medical record that the resident and responsible party if applicable were notified of the room change and accepting of the room change, the reason for the room change and documentation that the resident's roommate was also notified of receiving a new roommate.
15. Assess and provide support for adjustment to a new room and/or new roommate.
Clinical Assessment and Care Planning
1. Complete a social history, complete relevant sections of the Resident Assessment Instrument, MDS, and CAA Summary, and Social Services evaluations for all residents.
2. Communicate any significant changes in a resident's condition based on MDS assessments, observation, documentation, and critical thinking.
3. Complete psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
4. Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident, the goals to be accomplished for those needs/issues, and the appropriate interventions.

Therapeutic and Behavioral Interventions
1. Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long- term care facility, including their social, emotional, psychological needs and are Care Planned appropriately.
2. Ensure or provide support and education to residents/family members/significant others/responsible party to assist in their understanding of placement and facility issues in addition to referring them to the appropriate Social Service Agencies when the facility does not provide the needed services.
3. Provide support groups for residents/family members/significant others/responsible party as appropriate to their needs,

4. Provide clinical interventions to address catastrophic events that occur during the resident's stay in the facility.

Clinical
Discharge Planning
1. Maintain written and verbal communication on resident discharges to the community with the Interdisciplinary Team.
2. Coordinate the resident discharge planning process and make referrals for appropriate home care services prior to the resident's return to the community, i.e. Home Health Care Agency, Area on Aging, community Social Services Agencies, etc.
3. Complete and audit the status of Section Q referrals and referrals to the Local Contact Agency.
4. Make referrals for durable medical equipment for the resident's needs in the community, i.e. hospital bed, oxygen, wheel chair, shower chair, rolling walker, etc.
5. Coordinate referrals for outpatient services including outpatient rehabilitation, mental health follow up, primary care physician appointments and other consults that are assessed as needed by the Attending Physician and Interdisciplinary Team, i.e. cardiology, pulmonology, dialysis, wound care, etc.
6. Ensure that Transition of Care Booklets are completed timely, and are provided to the resident/family/responsible party/healthcare coach as well as PCP and community agencies as indicated and as approved by the resident/ responsible party.
7. Provide telephone follow up contact with the discharged resident and/or responsible party within 24-72 hours post discharge.
8. Maintain accurate record of the discharge follow up call.
9. Communicate with the Interdisciplinary Team and the Home Health Care Agency as needed regarding any needs that were discussed by the resident/responsible party during the post discharge follow-up call.
Clinical
Therapeutic Leave of Absence
1. Communicate with the resident, the family/responsible party and interdisciplinary team regarding requests for therapeutic leaves of absence (LOA).
2. Assist in ordering necessary equipment for LOA, i.e. portable oxygen.
3. Coordinate the assessment for caregiver training needs for the LOA.
4. Document in the resident's medical record the purpose of the LOA, the location, the length of time, and method of transportation.
5. Assist in planning for transportation needs for the LOA.
6. Communicate with the Interdisciplinary Team and resident/responsible party regarding allotted days for the therapeutic leave of absence in accordance with state and federal guidelines.
Education
1. Educate the staff regarding the psychosocial needs of the residents and their families/significant others/responsible party regarding the needs of aging and disability.
2. Educate staff regarding cultural diversity and each staff member's importance when caring for residents.
3. Educate staff regarding resident rights and how to recognize and prevent abuse, neglect and maltreatment.
4. Educate residents and families/significant others/responsible party regarding their rights and responsibilities, effective problem solving and the extent of community, health and Social Services that are available to them, including those necessary for effective discharge planning.
5. Supervise students assigned to Social Services in accordance with the respective school guidelines and monitor and document the progress of their work.

Participates in regular in-services/training, Quality Improvement committees, initiatives, studies, pilot programs other contract activities as assigned/appropriate as directed by the customer and/or Magellan. Candidates must be local or willing to relocate to become local and licensed at the independent level as a Social Worker (LICSW, LCSW-R or LICSW-CP), Mental Health Counselor (LPC or LMHC), or Therapist (LMFT) or equivalent independent licensure. This position has the primary responsibility of providing the full breadth of Child and Youth Behavioral (CYB) counseling services to children of all ages of military service members and their families at military installations to include civilian expeditionary work force and their families (when activated). These services may include non-medical counseling, training/health and wellness presentations, consultation with parents, personnel at child development centers, and consultation to installation command regarding behavioral health issues affecting children and youth. CYB counselors will be assigned to child development centers, youth programs, summer camps, and on demand assignments. The counselors work closely with the installation and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation.
* Provides non-medical, short-term, solution focused counseling directly to eligible children and youth of service members at assigned primary and secondary locations. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
* CYB counselors will also have a focus on supporting the staff and personnel of Child Development Centers (CDC), youth programs and summer camps (if available/assigned) as well as providing parent support, training, and guidance.
* Provides training and health and wellness presentations, participate in health fairs and other base/installation activities focused on children and youth.
* Enters counselor activity data daily through smart phone or web application to assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service/family member.
* Establishes and maintains working relationships with community resources focused on children and youth and provides appropriate linkages. Consistently visible within the assigned location(s) which allows MFLC services to be accessible. When working with children, counselor must abide by line-of-sight protocol. * Partners with POC to provide CYB services in a manner that addresses the needs of the installation/facility. Develops an effective and professional working relationship with the installation/facility POC.
* Manages duty to warn and restricted reporting situations according to Department of Defense (DoD) protocol and staffs the cases with Regional Supervisor/Regional Director.
* Communicates with Regional Supervisors and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations and in facilities to which the counselor is assigned. Responds to critical incidents and special requests as directed by the POC and approved by the Office of Secretary of Defense (OSD) program manager. The CYB will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director.
* Participates in regular in-services/training, Quality Improvement committees, initiatives, studies, pilot programs other contract activities as assigned/appropriate as directed by the customer and/or Magellan. This includes participation in pilot program certification and training processes, completion of activity documentation, integration of pilot activities in non-medical counseling work, and the utilization of applicable technology to complete required activities.
