Coordinate and monitor the delivery of behavioral health services, including therapy, medication management, and support groups. Connect clients with community resources, such as housing, food assistance, employment services, and transportation. Develops the Comprehensive Treatment Plan with the input of the client and through participation in treatment planning meetings and process. Advocate for clients' rights and needs with healthcare providers, social service agencies, and other organizations. Provide crisis intervention and support as needed. Maintain accurate and timely documentation of client interactions and progress. Collaborate with interdisciplinary team members, including therapists, psychiatrists, and other healthcare professionals. Contacts residents' family members to set appointments or educate the family on the residents' current status and/or progress. Monitor client progress and adjust care plans as needed. Participate in team meetings and training sessions. Maintain compliance with all applicable regulations and ethical standards. Our interdisciplinary team of professionals provides comprehensive substance use disorders, mental health, and psychiatric treatment that ranges from 60 days to 6 months. The On-Call SUD Counselor is a registered or certified substance use disorder professional who maintains registration from an approved/accredited California agency. The Counselor works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360's philosophy, where program schedule requires additional staff. The On Call SUD Counselor provides supervision and oversight of the clients and children throughout the facility with shifts ranging to provide 24/7 coverage. The On-Call SUD Counselor is an integral part of an interdisciplinary team that supports our clients through assessment of their needs and strengths in support of their health and recovery.
