Job Responsibilities: Provide high quality, compassionate guidance in both individual and group counseling sessions. Plan, oversee, facilitate and document patient's recovery. Co-facilitate assigned group or family sessions as needed. Ensure all documentation regarding patient care, treatment, and incidents is completed timely and in a clear, concise manner. Prepare individual treatment plans for each assigned patient. Initial assessments as well as follow up assessments. Evaluate patient needs and determine if referrals to other programs or facilities are needed. May plan for aftercare for assigned patients. Provide crisis intervention to patients, as needed. Provide case management duties for patients, ensuring individualized quality care as needed. Act as a liaison between referral sources and patients, as needed. Submit necessary referral forms to team supervisor for approval or assignment in electronic health record (EHR). * Ensure all needed documentation and forms are completed and up-to-date for assigned SCOAP patients. Minimum Qualifications: Bachelor's degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. The counselor's job role is to provide goal-directed, client centered help to clients with an addiction by educating them on the skills or the mechanics to deal with their addiction. ESSENTIAL POSITION RESPONSIBILITIES: 1. Provides services in order to maintain required productivity/billing standard of 1200 hours per year (Exempt), 70% of hours worked (Non-Exempt) 2. Conducts comprehensive assessments through the use of client interviews, review of records, and conferring with other professionals. 3. Counsels clients' in group or individual sessions to help them to cope with any substance abuse problems. 4.
