The Youth Care Professional III delivers relationship-based, competence centered, and trauma informed solutions to youth and families to help them reach their full potential. This position mentors and trains Youth Care Professional I staff and performs shift charge responsibilities. The Youth Care Professional III functions with independence and can be depended upon for resolution of more complex issues.
Essential Job functions
Engage youth and families in trust-based, productive relationships to help them prepare for transitions, interact in the community, and promote respect and independence.
Develop and execute treatment and behavior management plans designed to assist youth and their families to achieve their goals and objectives.
Manage youth and family activities, within an individual or group dynamic.
Supervise youth receiving services according to established program standards.
Maintain a clean and welcoming living environment where youth are encouraged to learn and practice daily living skills such as household chores and laundry.
Respond appropriately to resident emergency and safety issues/situations in compliance with agency and regulatory mandated reporting protocols.
Recognize and address dysregulated youth behavior and demonstrate proficiency in behavior management, the use of crisis de-escalation/intervention techniques and physical interventions (holds) when needed consistent with Therapeutic Crisis Intervention (TCI) protocols in a life or limb situation.
Mentor and train Youth Care Professional I staff.
Maintain a thorough awareness of all procedures relating to emergencies.
Complete and maintain documentation in medical record system and daily logs to meet program requirements.
Oversee the paperwork of staff.
Provide life skills and social skills training for youth and families.
Serve as an adult role model for youth and families in all behaviors including communication, hygiene, appearance, management of self, response to stress and challenges, and positive relationships with others.
Manage special team projects.
Perform a wide variety of support tasks including transportation, as required.
Perform shift charge responsibilities.

Education & Experience
High School Diploma or GED required

Special Requirements

We are seeking a talented Mental Health Specialist, Court Liaison to join our team of nearly 500 employees. The Mental Health Specialist, Court Liaison, works with justice involved individuals, who are diagnosed with a mental illness, through their transition into treatment and services in the community. The Court Liaison will have a primary role in Monroe County Mental Health Treatment Court and attend all Court appearances to provide routine progress updates of assigned participants on active caseload. This role requires significant collaboration between the Court systems and local treatment/service systems; attention to deadlines, Court requirements and completion of documentation in a timely manner is essential.
CCSI is partnering with Monroe County Office of Mental Health to hire a Mental Health Specialist, Court Liaison This position is employed by CCSI and working on behalf of Monroe County Office of Mental Health.
In this role, you will
Have a routine presence as liaison in Monroe County Mental Health Treatment Court
Maintain caseload of estimated 35-45 participants
Record progress notes in comprehensive, timely manner and complete all reporting as required by the Monroe County Office of Mental Health and Mental Health Treatment Court
Participate in case review meetings with direct Court staff, as scheduled
Receive assigned cases, review and/or formulate recommendations, submit referrals based on client needs
Recommend adjustments to treatment plans, advocates for equity and system movement to support individual identified needs
Obtain status updates and feedback from direct providers and Court officials to assist with development, and implementation of performance improvement activities for participants
Assess discharge planning needs of individuals being released from local jails, hospitals and other treatment agencies and interact with other personnel as needed for release planning
Communicate in a professional manner, orally, written and/or electronically, as a representative of Monroe County to all Court and community providers and partners
Be culturally responsive to the population being served in the development, design, monitoring, evaluation and/or implementation of programs and services.
Have working knowledge of the local behavioral health and social services network to assist with matching individuals with appropriate resources to promote health, wellness, and recovery
Use team approach in working with Socio-Legal Center staff, Courts and community partners.
Incorporate a recognition of diverse clientele into daily activities.

What You Bring

Ability to satisfactorily complete required background screenings (criminal, SCR, driving, and Monroe County Jail)
Valid Class D New York State Driver License and access to reliable transportation to meet transportation requirements of position

About the Program and Community Based Counselor Position:
Cayuga Centers is the longest-standing provider of Functional Family Therapist (FFT) in the United States, proud leading the way since becoming New York State's first accredited FFT provider in 2000.
Our FFT program serves youth ages 11-18 and their families. This family-focused therapy operates on the belief that all families have strengths and are doing the best they can at any point. With FFT, the entire family unit works together to prevent out-of-home placement, intervene on behalf of family rebuilding, and unify the family unit. Families work together through three FFT phases of engagement: motivation, behavior change, and generalization with our licensed clinicians over 12-16 weeks.

Responsibilities
Provide teaching, monitoring and recovery oriented supports in accordance with OMH 593 and 595 including the following rehabilitative services; Assertiveness/Self-Advocacy Training, Community Integration/Resource Development, Daily Living Skills, Health Services, Medication Management Training, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, and Symptom Management.
Ensure that each resident on his or her caseload receives comprehensive planning in a person centered approach for; admission (including the completion of admission notes), ongoing recovery oriented services, and discharge.
Observe, monitor, document, and report resident behaviors as well as their progress in accordance with agency standards.
Assist Residents in the preparation of nutritious meals, including food preparation when needed.
Assist in planning, implementation, and support of recreational activities.
Provide safe transporting of resident when needed.
Complete household chores assigned by supervisor to maintain cleanliness and safety standards.
Supervise and document resident medications in accordance with agency policies.
Perform any other duties necessary for the effective and efficient operation of the community residence.
Qualifications
RC I: High School Diploma required. Human services experience preferred. Clean driving record that meets Agency's Clean Driving Record Policy. Must be at least Eighteen (18) years of age.
RC II: QMHS Standard. Bachelor's degree in Human Services, or an Associate's degree in Human Services with 3-years FT related paid experience, or a High school diploma and 5 years related paid experience. Clean driving record that meets Agency's Clean Driving Record Policy. Must be at least Eighteen (18) years of age.
Work Environment
The schedule for this position is: Sunday-Thursday, 11pm-7am.
