Responsible to provide services as outlined in a referral, consistent with referral request and individualized care plan. Address the needs of people we support in the areas of participation, health, behavior and safety.
Promptly communicate the people we supports' needs to immediate supervisor and the Individualized Care Coordinator. Responsible to provide services as a vendor service provider to families as requested by the contracted agency. Responsible to provide services to families/people we support who are experiencing emotional trauma and promote positive family strength based interventions.

Job Summary:
The Preventive Services Caseworker provides individual case planning services to families that are referred from Erie County Department of Social Services (DSS). This purpose of the Caseworker is to help children remain safely in their homes and to prevent placement outside of their home. This position works with families of children that are placed in foster care to return home. By partnering and collaborating with the family, this position works to reduce the likelihood that a child will return to a foster care program. The Caseworker maintains strong communication with the family to work towards stabilizing and strengthening the family unit. This is done through counseling, parent training, home management, support and advocacy services. In addition, the Preventive Services Caseworker works closely with referral sources, community service providers, and all family members to ensure there are collaborative efforts towards permanency goals. These stakeholders build networks for each family to ensure adequate and stable supports are in place prior to discharge.
Essential Job Duties:
1. Develop and maintain regular contact and therapeutic rapport with each family. Face-to-face contact in the family home must occur regularly, at minimum two times a month with the caretaker and all the children living in the home. Must meet a minimum of 32 billable units weekly.
2. Communicate and collaborate with stakeholders to monitor service provision and ensure effective communication. Stakeholders may be Case Managers, Physicians, School/Education staff, CSE, Health Home Care Managers, Drug Treatment Court, Family Court, Juvenile/PINS, Community Providers for support with drug/alcohol addiction and mental health treatment.
3. Completes thorough and accurate family/children progress notes, court/permanency reports, court summaries, Individual Service plans, Family Assessment Service Plan (FASPs), monthly summaries, comprehensive assessments, and other case-specific documentation in accordance with regulatory requirements in a timely manner.
4. Teaches models and evaluates effective parenting and family management techniques. These techniques include parenting skills, behavioral management, appropriate discipline, etc.
5. Assists parents/caretakers in addressing safety, behavioral, educational, psychological needs. In addition, will provide crisis intervention to families, as needed.
6. Ensure all court mandated and/or recommended services are offered, utilized, and maintained via referral/linkage or provided directly to minimize risk, while enhancing the family's cohesiveness and overall well-being.
7. Assists with transportation for children and/or families to services/meeting/court and must have a safe, reliable vehicle.
8. Complies with agency policies, federal and state regulations.

Observe, monitor, document, and report resident behaviors as well as their progress in accordance with agency standards.
Assist Residents in the preparation of nutritious meals, including food preparation when needed.

Complete household chores assigned by supervisor to maintain cleanliness and safety standards.
Supervise and document resident medications in accordance with agency policies.
Perform any other duties necessary for the effective and efficient operation of the community residence.
Qualifications
RC I: High school diploma or GED required. Human services experience preferred. Driving record that meets agency's “clean driving record policy”. Must be at least Eighteen (18) years of age.
RC II: QMHS Standard. Bachelor's degree in Human Services, or an Associate's degree in Human Services with 3-years FT related paid experience, or a High school diploma and 5 years related paid experience. Driving record that meets agency's Clean Driving Record Policy. Must be at least Eighteen (18) years of age.
Work Environment
The DePaul Batavia Apartment Treatment Program provides consumers with the highest level of independence in a program licensed by the New York State Office of Mental Health. The program is designed to be a preparatory step before the consumer begins to live independently within the community. An emphasis is placed on increasing personal initiative and self-reliance. Staff make routine visits and are available 24-hours a day in case of an emergency.
There are 2 shifts available:
Sunday and Monday 2pm-10:30pm, Tuesday and Wednesday off, Thursday 10am-6:30pm, Friday and Saturday 7am-3:30pm.
Or
Sunday, Monday, Tuesday 2pm-10:30pm, Wednesday 10am-6:30pm, Thursday 7am-3:30pm, Friday and Saturday off.

The Licensed In-Home Mental Health Counselor will provide counseling and mental health services to youth and families. Mental health assessment and counseling may be provided in the home or in the community for a youth and their family.
Major Responsibilities/Activities:
Provide mental health assessment, diagnosis, treatment planning and psychotherapy services
Provide individual, family, and group counseling to a diverse population with many challenges

Guide families during and following a crisis episode experienced by a child/family with the goal of stabilizing the child/youth in the home and natural environment.
Maintain the crisis phone number, on call, for a week at a time (rotated amongst staff)
Collaborate with other treatment providers and collaterals, as appropriate
Complete and submit required documentation in a timely manner (e.g. progress notes, treatment plans, et cetera). Maintain positive working relationships with other professionals, community agencies and team members
Demonstrates sensitivity to cultural and ethnic norms
Follows agency's Worker Safety protocols
Participates in team meetings
Engage and guide the youth and family through active listening skills and therapeutic engagement methods
Maintain positive working relationships with program/agency staff, collaborating agencies, and stakeholders

Minimum Requirements:
NYS Education Department license in Social Work, Mental Health Counseling, Psychology or Marriage and Family Therapy
At least one (1) year of experience providing direct services for children with one or more of the following primary diagnoses: mental illness, alcoholism, chemical dependency and substance abuse
Demonstrated cultural competence in understanding and working with disadvantaged/minority populations in community-based programs in an urban and suburban setting
A valid NYS driver's license is required
Must have reliable means of transportation to travel throughout Western New York area
Must be comfortable transporting clients in personal vehicle as needed
Computer literacy required, experience with Anasazi preferred
