• Provide service coordination for individual with mental health and substance abuse issues and is responsible for developing, with the other team members, the overall service plan.
• Assist in the development of natural support networks. Individual and group supportive counseling, monitoring of symptoms and medications, linking the individual supported to the appropriate services, agencies and supports in the community, monitoring their development of appropriate behaviors in his/her home and other community settings, teaching the individual supported skills necessary for independent community living and facilitating them in the practice and transfer of these skills to his/her environment, providing support to his/her family.
• Assume responsibility for writing, implementing, evaluating, and revising Person Centered Plan and overall treatment goals and service plans that are based on evaluations and recommendations from assessments by various professionals and input from the individual supported.
• Function as a member of a multi-disciplinary team to develop and implement treatment goals.
• Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis.
• Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources.
• Review treatment progress and monitor and provide continuous consultation with other Assertive Community Treatment Team staff and community agencies to ensure the consistent therapeutic management of the individual's needs.
• Act as liaison with the program after hours staff on a daily basis through exchange of participant information; briefly detailing the presenting problem and after hours plans for the individuals being served by the Assertive Community Treatment Team.
• Meet regularly with the other Assertive Community Treatment Team members to formulate and disseminate information and be available by telephone and/or pager. Meet regularly to review the total caseload, problem-solve, develop plans, and determine daily duty assignments.
• Complete an intake assessment including: medical history, general physical condition, alcohol and drug histories and current use, educational issues, family structure and relationships, vocational history and goals, legal history and issues, psychiatric history (including, onset, course, effect of illness, treatment history, medication history risk behaviors).
• Ensure immediate and appropriate interventions are provided in response to changes in mental status which may put individual at risk.
• Documents participant activities consistent with the service records manual. Write an event note that reflects participant activities, goals, interventions, and outcomes.
• Developing service plans and/or person-centered plans as appropriate, and screens referrals in coordination with the other Assertive Community Treatment Team members.
• Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
• Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
• Follow service definition guidelines for services being provided.
• Complete all other relevant responsibilities as assigned by the supervisor.

The major functions of the school counselor incorporate the schools' mission of successfully educating the whole child to be a lifelong learner and career and college ready.

Essential Functions
Uses data to design and deliver a comprehensive school counseling program based on the NC Essential Standards for school counselors
Operationalizes data to meet student needs
Ensures that direct services to students follows the licensure recommendation of 80% of time worked
Delivers a comprehensive school counseling program:
Counsels students individually and in small groups for specifically identified needs within the three ASCA domains: academic, personal/social, and career.
Collaborates with parents and/or relevant stakeholders individually and in small groups to foster academic success for students.
Prepares and presents subject area content related to academic, career, and social-emotional success for students including sessions regarding personal growth, school adjustment, career development, self-concept, attendance and related matters.
Collaborates with teachers, administrators, parents and outside agency personnel to support the whole child.
Advocates for students, parents, and staff.
Responds to and supports efforts to stabilize student crisis situations on an individual and school-wide basis.
Collaborates with school and/or district personnel to support Cabarrus County's Bullying and Harassment Policy and Procedure.
Consults and collaborates with other specialized instructional support personnel (nurse, school resource officer, social worker, psychologist), MTSS team, and other relevant stakeholders.
Is accountable for the success and outcomes of a comprehensive school counseling program:
Serves on the school's MTSS team as the subject matter expert addressing behavior and social emotional needs.
Serves on the school's 504 team as an advocate for students with mental health, cognitive, and physical needs.
Collaborates with staff working with students in special populations within the scope of the school counseling role.
Analyzes data for school counseling services to ensure alignment with school and student needs.
Analyzes and interprets student academic data (EOG, EOC, course selection)
Coordinates student transitions into new schools.
Participates in and leads professional development opportunities.
Performs other duties as deemed appropriate and assigned by the School Administrator.

Ability to prepare, read and comprehend a variety of job-related forms, reports, spreadsheets, maps, plans, records, documentation and correspondence in all languages required by the job.

Education and Experience
North Carolina Standard Professional II License (K-12) - Level M.
Professional School Counseling License (NC DPI).
Physical Requirements
This work requires the occasional exertion of up to 10 pounds of force.
Work regularly requires speaking or hearing, using hands to finger, handle or feel and repetitive motions, frequently requires sitting and occasionally requires standing, walking, stooping, kneeling, crouching or crawling, reaching with hands and arms, pushing or pulling and lifting.
Work has standard vision requirements.
Vocal communication is required for expressing or exchanging ideas by means of the spoken word
Hearing is required to perceive information at normal spoken word levels.
Work requires preparing and analyzing written or computer data, operating machines, operating motor vehicles or equipment and observing general surroundings and activities.
Work occasionally requires exposure to outdoor weather conditions.
Work is generally in a moderately noisy location (e.g. business office, light traffic).
Special Requirements

Gaston Social Services: Protecting, caring for and empowering children and adults who are temporarily or permanently unable to care for themselves through the administrating of federal, state and county programs focused on improving the well-being of our citizens.
An hourly salary range of $15.48 to $17.99 is anticipated for this position.
The duties listed below are not all of the duties that may be assigned, but are those that are considered as essential for an employee to perform.
* Providing transportation assistance for mental health appointments, substance abuse counseling/treatment, medical/dental appointments, pharmacies, parenting classes, educational facilities, drug testing, seeking employment, locating housing, and other appointments as necessary. Scheduling daily needs to ensure all clients are able to attend appointments timely.
* Provision of in-home aide services (through teaching, modeling, coaching, reinforcement, and encouragement), including budgeting, housekeeping, personal hygiene, meal planning and food preparation, comparison shopping, parenting skills, behavior management, disciplinary techniques, employment seeking, and connecting with educational/vocational training.
* Provide childcare services for clients while they are in counseling sessions, support group, parenting classes, job searching, seeking housing, or other approved appointments.
* On an as-needed basis, other duties to include assisting with the crisis line calls, room cleaning, donation sorting, retrieving mail from the post office, grocery pick-up, tracking activities for monthly reporting, and attending team meetings/trainings. Additionally, staff may be asked to cover other shifts for holidays or to assist teammates.
* Graduation with a high school diploma or GED equivalency
* A valid NC Driver's License is required.
* Experience working with children and/or trauma survivors preferred.
The applicant selected must undergo and pass a drug screening test prior to employment.
