Prototypes Women's Center in Pomona is a 180-bed residential facility for women and women with children. Our interdisciplinary team of professionals provides comprehensive substance use disorders, mental health, and psychiatric treatment that ranges from 60 days to 6 months.
The On-Call SUD Counselor is a registered or certified substance use disorder professional who maintains registration from an approved/accredited California agency. The Counselor works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360's philosophy, where program schedule requires additional staff. The On Call SUD Counselor provides supervision and oversight of the clients and children throughout the facility with shifts ranging to provide 24/7 coverage.
The On-Call SUD Counselor is an integral part of an interdisciplinary team that supports our clients through assessment of their needs and strengths in support of their health and recovery. On-Call SUD Counselor will perform counseling in individual and group setting, deliver curricula in accordance with program specific requirements and case management, and assist clients in navigating systems of care while providing a supportive treatment environment.

KEY RESPONSIBILITIES
Facilitate group counseling sessions, community meetings, and teach classes on various treatment-related topics to assist clients in meeting their treatment goals.
Perform crisis intervention and communicate with treatment team as unforeseen situations arise.
Properly document all group counseling sessions and all individual sessions as needed within 24-72 hours per contract requirements.
Must meet residential service delivery goal of 60%.
Proactively link clients to both internal and external resources based on their treatment needs.
Follow up on the progress or status of clients' treatment.
Document client updates and incidents daily.
Maintain safety and security of the clients in the facility by performing periodic house runs.
Ensure client medications are securely stored and properly accounted for as needed.
Monitor and document client administration of medication.
Assign client duties and sign off upon completion.
As needed, accompany clients to offsite appointments related to their treatment.
Collaborate with client and other internal and external resources to develop or maintain problem lists and/or treatment plans, transition plans, progress notes, and appropriate updates in support of their health and recovery.
Complete discharge paperwork/process and other required agency assessments in a timely manner.
Support intake process such as preparing rooms and maintaining supplies.
Perform property and room searches as needed.
Respond to overdose and other medical emergencies.
Perform tests for substance use such as urinalysis and breathalyzer.
Observe clients every 30 minutes and document on observation log, per program requirements.
Collect, clean, or dispose of biohazard waste.
Disinfect and clean rooms and common areas as needed.
Accurately enter data in various electronic systems for all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Read and respond to emails, calls, and Teams messages in a timely manner.
Complete assigned training as needed and as required by the due date.
Attend and actively participate in meetings.
Plan and facilitate celebrations and special events.
Work on weekends and holidays as needed.
Perform other duties as assigned.
Education, Certification, or Licensure
High school diploma or equivalent required. Bachelor's degree in Health and Human Services, Social Work, Psychology, or other related field preferred.
Must be registered as an Alcohol and Other Drug Counselor or as a Substance Abuse Counselor with a California approved certifying agency (CCAPP, CADTP, or CAADE).
Possess current First Aid and CPR certification or ability to obtain within 30 days of hire.
Possess a valid CA driver's license.
Experience
Experience as a registered counselor or recovery specialist in community mental health preferred.

Minimum Required Education and Experience:
Must possess a high school diploma/GED. No previous experience required.
Position/Program Requirements:
Must possess a reliable mode of transportation to travel to assigned client homes, schools, offices, etc. If personal vehicle is the mode of transportation, must meet Agency's driving requirements.
Must be physically and mentally fit in accordance with the Agency's Personnel Policy: Physical Fitness Standards and Examinations. Must be willing to complete a Tuberculosis (TB) and drug screening test.
Must be willing to complete a personal background investigation conducted by the State of California.

* Experience: Minimum of 2-3 years of relevant experience providing behavioral health services in a clinical or community setting. Previous positions should include responsibilities related to assessment, diagnosis, and treatment planning. Experience in crisis intervention and management is highly desirable. Position Purpose:
The Family Planning Counselor/CPSP Case Manager provides counseling tailored to each family planning patient and educate them on human reproduction, contraceptive methods, pregnancy testing, and pregnancy options. The Family Planning Counselor/CPSP Case Manager provides information to programs and services for prenatal care, health education, and nutrition education.
Family Planning Counselor Responsibilities and Functions:
Screens emotional stability and gives pregnancy test results and provides pregnancy counseling.
Provides educational instruction on contraceptive methods in accordance to Health Education Protocol for Family Planning.
Assesses patient's educational needs and tailors counselors session accordingly.
Reviews tobacco use assessments provided to Family Planning patients.
Conducts teen risk assessment for adolescent patients.
Makes necessary referrals to on-site professional counselors and off-site services as needed.
Performs eligibility screening for all Family Planning patients.

Collects and reports statistical information.

Provide CPSP orientations via one (1) hour face to face session.
Provide Prenatal Combined Assessment/Reassessment Tool and Combined postpartum assessment per CPSP guidelines - minimum of three (3) one hour face-to-face assessments per client.
Based on assessment tool responses, provide one to one nutrition, psychosocial counseling and health education to clients.
Develop individual care plan and conduct appropriate follow-up for clients, in person and via phone calls.
Assist in the development/revision of assessment tool protocol.
Consult with psychosocial and nutrition consultants as appropriate.
Report/document client progress needs in medical charts.
Ensure/assist clients with practical arrangements for meeting medical, diagnostic, specialty consults, nutrition, psychosocial and other recommendations.
Assure that perinatology and obstetrical diagnostic reports are received and shared with provider.
Per provider order, contacts patient regarding follow up visits or other medical issues.
Assist patient with hospital pre-registration.
Assist patient with form completion (disability, FMLA, etc.)
Follow-up on Medi-Cal eligibility issues.
Track patient data, including required UDS reporting, and complete reports as required.
Attend Comprehensive Prenatal Health Worker trainings.
Collects and reports statistical information.
Attends department staff meeting including case conference meetings.
Assists with CPSP audits.
Position Requirements and Qualifications:
Ability to work with adults and youth in a variety of settings.
Basic knowledge of reproduction, contraception and related topics.
CHW Certification and counseling skills.
Bilingual in English/Spanish.
One year prenatal care experience.
Excellent verbal communication skills.
Written/oral fluency in Spanish.

ESSENTIAL FUNCTIONS AND DELIVERABLES

QUALIFICATIONS:
Education: Master's degree in psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field from an accredited institution.
Licensure/Certification: Valid and current license as a Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) issued by the State of California Board of Behavioral Sciences. Current CPR/BLS certification (must be American Heart Association or Red Cross accredited program). As a requirement of this position, you must receive EPIC certification for the module you have been hired into. Valid California driver's license, and auto insurance.
Experience: Minimum of 2-3 years of relevant experience providing behavioral health services in a clinical or community setting. Demonstrated experience working with children, adults, and families, including experience in individual and group therapy. Previous positions should include responsibilities related to assessment, diagnosis, and treatment planning. Experience in crisis intervention and management is highly desirable. Relevant experience from previous positions which demonstrate a strong foundation in behavioral health services and includes specialized training or certification will be considered. Preferred experience: Certification in specialized therapeutic techniques (e.g., CBT, DBT, play therapy, CADC, EMDR) and experience with electronic health records (EHR) and documentation systems.
Essential Technical/Motor Skills: The ability to use a computer on a routine basis and complete required documentation. Use of word processor, spread sheets and internet. Ability to transition to Electronic Health Records.
Interpersonal Skills: Strong clinical assessment and diagnostic skills. Proficiency in individual and group therapy techniques. Ability to develop and implement effective treatment plans. How you will make a difference: The position has primary responsibility for using their lived experience and knowledge of the system to provide outreach and engagement, support, advocacy, and direct services to program clients of all age ranges. Peer Support Counselors are people who have lived experience in the mental health, judicial, and/or social service systems in some capacity. They serve as role models, companions, educators and advocates to our clients. The Peer Support Counselors will support clients in moving toward recovery, creating a meaningful role in their life, which is separate from their illness. Division/Program Overview: * The TAY Crisis Residential Treatment (CRT) facility is a short-term voluntary residential facility. Designed to help Transitional Age Youth (TAY), ages 18-25, who experience a psychiatric crisis to safely stabilize, recover, and transition back to community. Services include: * 24/7 program * Therapeutic and mental health services * Rehabilitation and recovery services (including substance use rehabilitation) * Pre-vocational or vocational counseling * Medication evaluation and support services * Daily exercise and health/wellness education * Crisis intervention.
