What You'll Do: Complete the Integrated Biopsychosocial Assessment and complete intake sessions with clients. Formulate a mental health diagnosis by the current Diagnostic and Statistical Manual of Mental Disorders V (DSM V) standards. Develop an initial treatment plan, incorporating standard practice guidelines for each diagnosis. Review and update the treatment plan with the client and others involved in the individual's care as appropriate. Measures change and adjusts treatment as needed. Document assessments and care provided in the client's Electronic Health Record (EHR) and complete all documentation as required. Complete encounters and associated documentation according to YVFWC requirements and coding and billing rules. Respond to crisis situations with assigned individuals, assesses urgency of individual's needs and help obtain appropriate services as needed. Assist individuals and families in obtaining additional mental health services and other social resources as needed. Document all referrals made to YVFWC case manager or other resources. Work with individuals to transition and maintain a presence in a community setting after psychiatric hospitalizations. Present mental health findings in verbal or written format to appropriate professionals. Provides case presentations for peer review. Perform clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements. Perform other duties as assigned. Who We're Looking For: A licensed therapist who has direct experience treating children or adolescents A team-focused individual who can engage with all levels of an interdisciplinary team A therapist who can handle complex, multi-service treatment plans that may require expertise in evidence-based practices Bilingual (English/Spanish) is strongly preferred but not required Qualifications: Master's Degree in Social Work, Psychology, Counseling, or related field. Basic Life Support (BLS) certification within 45 days of hire. We will hire either level III or level IV, depending on the qualifications of the person hired: Mental Health Worker III Requirements Preferred Experience: Two years' experience in the direct treatment of persons with mental illness or emotional disturbance that was gained under the supervision of a mental health professional preferred. One year's full-time experience in the treatment of children under the supervision of a children's mental health specialist preferred. Owner mentality: you're self-motivated, set your own goals, and can deliver results while working independently from home. Be tenacious, high-energy, able to overcome objections during difficult situations and keep a positive attitude. Outstanding social skills, communication skills and ability to think on your feet. Be extremely attentive to details with consistent error-free execution. Must have a vehicle and ability to travel to child care programs through DC, MD and Northern VA. MAJOR DUTIES AND RESPONSIBILITIES: Provides direct outpatient psychotherapy services to individuals, couples, families and groups, by performing mental health and substance abuse assessments, develops and implements treatment plans and evaluates treatment outcomes with the ability to assess both mental health and substance abuse diagnoses. Develops appropriate treatment plans for clients. Maintains all notes of sessions current with no greater than a 72 hour lag from pt session. POSITION SUMMARY: Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. The master's level social worker represents Holt in their community and ensures the successful adjustment of child and family by conducting an assessment of the family, and providing appropriate services throughout the adoption process. The master's level social worker works cooperatively with families, the Branch Office and Adoption Teams headquartered in Eugene, Oregon. Note: The Adoption Social Worker is primarily a home-based position. The Adoption Social Worker position requires travel (most often by car) to different locations within the assigned area to complete the essential functions of the position. ESSENTIAL JOB FUNCTIONS (to be completed within timeframes outlined by the Branch Director): 1. Determines a family's ability and capacity to parent an adopted child by conducting pre-home study meetings as requested, reviewing and assessing agency-requested information, evaluating the family's knowledge and perceptions of adoption, teaching adoption education courses as needed, and conducting psycho-social assessments of family members. 2. Contributes to the delivery of high-quality adoption services by producing adoption home studies and post placement reports that comply with provided guidelines and meet current agency, state, country and USCIS requirements. 3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database. 4. Prepares family for travel and assists with post placement adjustment by providing training and identifying resources available to help meet the child's needs and aid in the child's adjustment to the family. Conduct home visits as required by state law and country program guidelines, assist in early identification of problematic issues, and make a determination regarding the stability and appropriateness of the placement. 5. Assures child's physical and emotional well-being and eases family's adjustment by providing counseling to the child and family and/or helping arrange for counseling with a clinician outside the agency if needed. In case of an adoption disruption or dissolution, maintain relationship with family to ensure transition plan is developed that meets the child's needs. Assist child and family with transition of child to new placement. 6. Contributes to public knowledge about Holt and supports Holt programs by participating in activities arranged by Holt including picnics or parent groups, representing Holt at adoption meetings, coalitions and organizations, and performing other outreach activities as appropriate. 7.
