Incumbents in this position will be responsible for the following: * Physical assessments of high-risk children in the foster care system. * Conduct multidisciplinary case conferences with providers and managed care plans to resolve barriers to care and authorizations. * Document timely and accurately in CWS/CMS, monitor caseload metrics, and prepare monitoring and audit materials. * Educate families on care plans, medications, equipment, and navigating systems of care. * Referral to services. * Countywide travel to partner sites and clinics. The Stroke Team data registrar RN will evaluate the Stroke Program's performance improvement activities under the supervision of the Stroke Program Coordinator. The evaluation includes data mining, analysis of the data, and data entry into multiple stroke registries which includes but is not limited to Get with the Guidelines (AHA) and Image Trend (Riverside County EMS Agency). This process extends to the evaluation of patient satisfaction with the perception of quality care and the education needs of the facility. Maintains knowledge of current trends in the field of stroke care and quality measures. Ability to recognize, trend, and report opportunities for improvement to staff and providers timely.     Under direction, the incumbent is responsible for providing professional direct or indirect nursing care to patients in both the preoperative and post-anesthesia phases. Preoperative nurses play a critical role in preparing patients for surgery by conducting comprehensive assessments, reviewing medications, coordinating diagnostic testing, and ensuring all necessary consent and documentation are complete. They educate patients about the surgical process and provide emotional support to reduce anxiety, while also confirming physical readiness through proper attire and the removal of personal items to maintain safety and compliance with protocols. Following surgery, post-anesthesia care unit (PACU) nurses care for patients as they recover from anesthesia, monitoring vital signs, levels of consciousness, and overall responsiveness. Because patients may experience confusion, pain, nausea, or difficulty breathing, and may not be able to clearly communicate their needs, PACU nurses must proactively observe and respond to subtle cues. This role demands quick decision-making in a high-pressure, fast-paced environment to ensure patients safely regain consciousness and stabilize after surgery.
