The Hospice Palliative Care LPN is responsible for and provides skilled nursing care reflective of the agency's mission to the ill or injured in the home, hospital or extended care facility. The LPN works under the direction of the Nurse Case Manager and/or Clinical Manager.
Essential Duties and Responsibilities:
Responsible for coordination of care with community and agency resources and with the designated registered nurse on a regular basis.
Provides nursing care as per the established plan of care and documents clinical activities per the agency policy
Recognizes and interprets symptoms and institutes remedial measures within the scope/limits of a Licensed Practical Nurse in the state of CT and reports significant changes in the patient condition to the primary RN Case Manager/Clinical Manager immediately.
Administers medication and treatments as ordered by the physician, with the exception of IV push medication. LPN must pass competency for intravenous/subcutaneous administration of medication via continuous infusion. (LPN may increase or decrease dose, including delivering bolus dose via patient infusion pump, as per instruction and delegation of duty by the Case Mgr.
Reinforces appropriate interventions, medications, effects and side effects to patient, caregiver, volunteers and other as appropriate as directed by the RN Case Manager.
Implements care to achieve outcomes of comfort, symptom management, safe dying, self-determined life closure and effective grieving for patient and family.
Identifies and addresses comfort care needs
Maintains the dignity of the dying patient.
Supports the patients and family's unique spiritual and cultural beliefs.
Provides holistic, family-centered care across treatment settings to improve the quality of life.
Consults and collaborates with the interdisciplinary team and others involved in the patient's care.
Provide care consistent with National Hospice and Palliative Care Organization standards of practice for hospice programs.
Observations of adult and geriatric client's condition are accurately reported to the designated registered nurse and documented appropriately.
Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.
Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.
Performs other duties as assigned by Hospice Clinical Manager/designee.

Education: Graduate of a NLN accredited school of nursing required.

About You:
Pieper Memorial Veterinary Center is seeking a dedicated and skilled Department Manager to oversee the day-to-day operations of our Inpatient veterinary team. Oversees, implements, and coordinates a resident-centered life enrichment program through literature, lectures, movies, cultural events, and other activities available in the community in Caleb Hitchcock Health Center, Dogwood Assisted Living Memory Care, and Social Assisted Living.
Tracks and encourages resident participation in the Enrichment by offering activities and events at varied times across the continuum.
Recruits, trains, and supervises LE staff, volunteers, and instructors.
Attends Resident Care/Interdisciplinary Team meetings, Care Conferences, and other resident care related meetings as appropriate or needed.
Complies with all regulatory requirements related to life enrichment documentation and standards in a CCRC.
Works with individual residents to facilitate ideas and events and resident committees related to specific areas of programming.

Interviews new residents and support them in acclimating to the community.

Education and Experience

POSITION SUMMARY:
Under direct supervision of the Aquatics Director, Lifeguards should ensure a safe, clean and enjoyable aquatic environment for YMCA members and participants. Ensure safety of all individuals in the pool area including people on deck. Lifeguards should be alert and watching all activity in and around the pool, walking around the pool area to see all points in their zone, enforcing and informing members of all pool rules, recognize and respond to emergency situations, perform emergency care and contact emergency assistance when required. Greets everyone by using names whenever possible; extends thanks whenever appropriate; building relationships is always a priority. The position is responsible for ensuring that the mission, purpose, image and core values of the YMCA of Greater Hartford is conveyed in all we do.
ESSENTIAL FUNCTIONS:
1. To ensure the safety of every individual at the YMCA, both in and around the pool as well as the rest of the facility.
2. To enforce regulations pertaining to showers, food and drink, conduct.
3. To help in maintaining the premises in a clean and sanitary condition.
4. To maintain accurate records as required by the CT Health Department Sanitary Code.
5. Be acquainted with other available facilities and activities.
6. To learn and review all emergency procedures.
7. To attend all staff meetings and in-services as required.
8. To report 15 minutes prior to scheduled hours in proper uniform (whistle, trunks and provided lifeguard shirt for men and one-piece suit and provided lifeguard shirt for women).
9. To secure a substitute lifeguard when unable to fulfill assigned duties and notify supervisor of changes.
10. Ability to see and hear individuals and alarms in pool/pool area.
11. Ability to use chemistry test kit.
12. Ability to communicate verbally and in writing.
13. Ability to sit or stand for long periods of time.
14. Ability to perform activities which require sustained concentration and attention
15. To maintain all certifications required by the YMCA of Greater Hartford.
16. All other duties as assigned by supervisor.
QUALIFICATIONS:
Must be 16 years of age or older.
Current YMCA or ARC Lifeguard, CPR for the Professional Rescuer Certification (infant, child and adult), Oxygen Admin and First Aid certification.
In addition to the above requirements, it is expected that a lifeguard will bring to his/her position maturity, responsibility, positive values and sincere interest in working with people.
PHYSICAL REQUIREMENT:
Ability to walk, stand, and sit (including on the floor) for long periods of time.
Must be able to lift and/or assist children up to 30 pounds in weight.
Visual and auditory ability to respond to critical situations and physical ability to act swiftly in an emergency

SSS-STEM Academic Coordinator

The SSS-STEM Academic Coordinator (Student Services Program Administrator) will be responsible for coordinating student support and engagement programming, including supplemental advising, peer mentoring, academic success interventions, and other educational programming to improve college transition, retention, and graduation outcomes for students traditionally underrepresented in higher education. Coordinates academic workshops, academic coaching/mentoring, and individual/group advising sessions to help students reach and maintain satisfactory academic standing and persist/complete their degree in a STEM program of study. Assists with supporting the goals of the Student Success unit. Leads and trains programming staff, advises students, and develops, implements, and coordinates student development programs to improve academic performance and student retention.
This position reports to the Director of Student Support Services-STEM.
DUTIES AND RESPONSIBILITIES
Coordinates and leads implementation of academic support programming to ease students' college transition and improve retention and student success efforts for students traditionally underrepresented in higher education.
Advises students in navigating course options, graduation requirements, schedules, and major/minor selection. Provides personal and non-academic counseling. Assists with advising students in academic and co-curricular pathways. Monitors students' academic progress in cooperation with other university offices. Make recommendations for academic course offerings and STEM enrichment programming based on students' needs.
Manages academic support/success programming, including coordination of peer and professional tutoring, monitoring students' academic progress, and analyzing grade reports.
Oversees peer coaching/mentoring program. Recruits, trains, and supervises peer coaches and mentors. Coordinates workshops, academic coaching/mentoring, and one-on-one and group sessions for students in academic difficulty to help them return to satisfactory academic progress standing.
Provides outreach to students to connect them to campus resources to promote their retention. Facilitates engagement activities for students in academic warning, probation, and/or subject to dismissal. Meets with students to provide strategic academic counseling.
Evaluates programming efforts. Gathers relevant data, compiles statistical reports, and maintains appropriate computerized files and records.
Assists with publicizing and marketing of academic support resources and programming.
Required to work occasional weekends or irregular hours.
Teaches FYE course sections.
Performs related work as required.
MINIMUM QUALIFICATIONS
Bachelor's degree.
Five years of professional experience in counseling, coaching, and/or advising students, with at least one year having served as an advanced or lead team member.
Demonstrated experience in providing/administering academic support services and activities that support student growth, retention, persistence, and graduation.
Experience working with first-generation and/or low-income students from varied educational backgrounds.
Experience working with college students interested in or studying STEM.
Experience coordinating activities and events to promote student learning, engagement, and/or career preparation.
Demonstrated communication, interpersonal, writing, and administrative skills.
Experience or demonstrated ability to provide general educational support, effectively implement recruiting strategies, and interpret educational records and related information.
Experience in data management, analysis, and reporting.
PREFERRED QUALIFICATIONS
Master's degree.
Three or more years of experience in higher education.
Experience supporting and/or advising STEM students in a college setting.
Bilingual (English/Spanish).
