Reports to Physician or Clinical Supervisor any unsafe situations with the patient's home. * Completes Hospice comprehensive assessment and plan of care within regulatory time limits. * Attends and contributes to interdisciplinary team meetings for Hospice. * Reports to the RN promptly any patient information that is outside the vital parameters or a change in condition that poses a safety or health risk, or suggests that patient needs exist. * Will work with bereaved families and see patients in their homes. * Will assist with social work issues that arise in Home Health and Chronic Care Management. * Maintains confidentiality. * Participates in hospital CQI activities. * Attends appropriate in-service, seminars, workshops, and training sessions related to Social Work to maintain and update clinical skills. * Provides formal or informal teaching to other healthcare professionals. * Develops effective working relationships within the Resource Management Department. * Identifies patients requiring social work intervention through established case-finding mechanisms. * Interviews patients/families in order to complete a psychosocial assessment. * Develops a treatment plan responsive to patient/family concerns, age-specific problems and clinical impressions. * Improves patient and/or family understanding of and adjustment to medical diagnosis through counseling and education. * Concise documents need to be identified, services provided, and a discharge plan in patients' medical records.
