CLASSIFICATION: Part Time Hourly (Monday-Friday; 3 hours/day) REPORTS TO: Nutrition Coordinator JOB SUMMARY: The Nutrition Site Manager provides for the safe serving, packing and delivery of meals to eligible seniors aged 60 and older served at our congregate meal site in Bucyrus. QUALIFICATIONS: Valid Ohio Drives License High School Diploma or GED Proof of Automobile Liability Insurance Must be able to pass pre-employment drug test and random alcohol and drug testing. To keep daily menu posted for clients. Responsible for daily documentation of required temperatures Responsible for daily cleaning operations of meal site. Responsible for daily documentation of site activity/making sure each client signs for their congregate meal. Responsible for signing up new congregate clients/updating clients yearly. Responsible for communicating documentation to Bucyrus meal site. To meet the request of client for additional service provided by Crawford County Council on Aging. Perform kitchen, packing and cleaning duties as needed. Accurately deliver meals to clients and obtain any required signatures from client. Must collect donations from specified clients. Responsible for other duties assigned by the Nutrition Coordinator. Requirements: PHYSICAL REQUIREMENTS: 1. Must be able to lift 35 pounds. 2. Serenity Case Manager will oversee the physical and mental wellness of individuals to ensure they are supported and can achieve the best outcomes. They work closely with mental the health team and other health care professionals, as well as family and friends of clients. The Case Manager provides services to clients seeking SUD treatment including but not limited to assessment, referral, monitoring, and follow-up. Must have reliable transportation. Assists passengers as needed in the boarding and disembarking of the vehicle in order to ensure the safety of all passengers, including assistance during vehicle evacuation, as necessary. The House Monitors are responsible for monitoring the safety and security of Serenity intensive outpatient clients in substance abuse treatment. Assist with the day-to-day operations and monitoring both the clients, internal environment, and surrounding grounds. Responsibilities: · Relieving the previous shift and conferencing with them regarding all records, logs, daily reports to ensure accurate and complete reporting. · Officially document any records, logs and so forth that are not complete. · Conduct searches of all residents and their belongings each time they enter the facility. · Assist in or conduct a residential headcount/bed check a minimum of three times per shift or as many times as directed by senior staff. · Monitor the condition and behavior of all residents for intoxication. · Assist in or conduct necessary room searches and inspections. · Assist in the monitoring and collection of resident urinalysis specimens. · May be required to transport residents, staff and/or packages to and from assigned destinations in an agency vehicle. · Answer in-coming phone calls and direct the calls as needed. · Make sure all visitors entering the facility, for either personal or business reasons, are logged in and out on the Daily Report. Under the clinical supervision of the Clinical Supervisor/Director the Mental Health Therapist provides individual, group, or family psychotherapy. The therapist collaborates in a multi-disciplinary setting with other Serenity Health and Wellness Corporation Clinicians to provide comprehensive treatment to the client and his or her family. Responsibilities: Complete diagnostic assessments and other ongoing assessments such as mental status exams and various risk assessments, utilizing the DSM-V criteria to appropriately diagnose mental health and/or substance use disorders.
