    Developmental Disabilities Case Management Job Description The Case Manager Family Consultant is responsible for the up keep of each Individual's chart, which houses complete records of the individual obtained at the time of referral and admission; to include, documentation of the work performed on the individual while in placement, and the charting of the Individual's progress while serving as the provider. · Provide written quarterly service plan reports on all assigned cases. · Participate in agency on-call rotation. This may include staffing coverage, transportation and all other duties assigned · The Family Consultant has monthly contacts with the individual and more if needed, at a minimum of one monthly contact in the community and in the home. · Participate in weekly supervision/follow-up with Family Consultant Supervisor. · Responsible for supervision of the direct care staff and individuals. · Provide copies of Fire Drills and Supervisory check lists to Family Consultant Supervisor · Serve as an advocate and teacher to the individuals. · Must attend Monthly Unit Meetings and Monthly Staff meetings. · Participate in the rotation of the CRT (Critical Response Team) as needed. · Report incidents to CCS Individual Advocate/FC Supervisor within 24 hours. · Develop an understanding of the DBHDD system. · Become familiar with the Office of Regulatory Services rules and regulations related to the DD department. · Provide support to the individual, their family and supervision of direct care staff. · Assist Individual in establishing valued social roles in the community. · Must have the ability to effectively write SOAP notes. · Case notes must be completed on Kaleidacare computer system and must be turned in to Supervisor for review in a timely manner. · Must follow policy and procedure accordingly to Creative Community Services, Inc. precepts · Ensure individuals attend social gathering and CCS events and provide transportation as needed · Coordination and/or providing transportation for individuals as needed. · Family consultant must attend DD social events and CCS company events. · Family consultant must complete 2 full days (9-5) in the office. · Family consultant must meet deadlines for all finance departments requirements which include timesheet, mileage, expense request, leave request and Medicaid billing. · Ensure Medicaid summaries are accurate and complete to deadline. · Filing is to be completed in a timely manner on a monthly basis or as documents is received. · Must adhere to CCS Dress Code. Do not wear jeans while out in the field completing visits. Jeans are to be worn on Fridays in the office only or during a filing/project day. · If coverage cannot be found for an individual on your caseload, you are responsible for providing the coverage. · Provide additional trainings to CA's should they need it. · Must submit daily task list and weekly calendar to Supervisors. · When flexing hours, you are still expected to complete 40 hours of work on a weekly basis. Should you not complete 40 hours of work a week, it is expected that you turn in leave request. · Inclement Weather protocol o In case of occurrence of hazardous weather, all community connections should have your number to contact you in regards to safety measures for the individuals on your caseload. You must contact staff and or families to assist in coordinating coverage, safety measures, precautions ect. · All other duties assigned. Desired Qualifications: The Family Consultant position requires an individual with an understanding and experience with foster care, developmental disabilities and applied behavioral analysis . This position requires a Bachelor's Degree in a Human Services field and/or experience working with special needs children and/or persons with developmental disabilities. This person will be required to analyze and react to difficult situations, often under considerable stress and pressures. Decisions will be guided by established policy and procedure.

Position Summary The TFC Family Consultant is a key member of the TFC Clinical Team and plays a vital role in supporting youth placed in therapeutic foster care. This position serves as the primary coordinator for youth care plans, ensures therapeutic interventions are implemented, and supports foster families to create safe and healing home environments. Core Responsibilities Youth & Family Support Build supportive, therapeutic relationships with youth and foster families. Ensure placements are safe and conducive to each youth's growth and healing. Advocate for the needs of youth and help coordinate services across providers. Case Management Serve as primary coordinator of each youth's Individual Service Plan (ISP). Attend and participate in admission, ISP, and placement meetings. Ensure timely documentation of case notes, logs, ISPs, and incident reports. Monitoring & Documentation Conduct weekly check-ins with youth and foster families; meet jointly at least monthly. Maintain complete, up-to-date case files per CCS and state regulations. Utilize systems such as Extended Reach, SHINES, and Georgia Score for documentation. Team Collaboration Participate in regular supervision, unit meetings, and staff meetings. This full-time position requires travel within an hour of our Norcross office and the ability to work flexible hours as needed. The RN will provide clinical oversight, health care planning, medication training, and consultative support to staff, individuals, and families, while serving as a positive role model aligned with the mission and values of CCS. Essential Duties and Responsibilities Medical Oversight & Clinical Support Maintain a consistent caseload with regular home visits to individuals in Host Home and CA settings. Ensure all medications are properly examined for accuracy, storage, and expiration. Demonstrate sound clinical judgment and strong decision-making regarding the safety and care of individuals. Provide direct training and consultation on medication administration and disposal, health care planning, and recognizing signs of illness. Understand the use and management of psychotropic medications. Educate staff, including Community Assistants (CAs), Host Home Providers (HHPs), and Family Consultants (FCs), on medical best practices and individual care needs. Serve as a resource for the Critical Response Team (CRT) in navigating complex or urgent medical situations. Training & Documentation Conduct onboarding medical training for new HHPs and CAs, including instruction on Health Care Plans (HCPs), medication protocols, and clinical procedures. Complete and update annual Health Care Plans for each individual, collaborating with the individual, HHPs, and Family Consultants. Utilize the IDD/HRST Portal to document and monitor health information. Complete HRST training and perform annual ratings and updates as needed. Communication & Collaboration Maintain regular and responsive communication with individuals, family members, and team members to provide medical education and answer questions related to care and medication. Collaborate with interdisciplinary teams to ensure person-centered health support is integrated into overall care planning. Physical Requirements Ability to lift up to 50 lbs and perform essential job duties that include sitting, standing, walking, speaking, hearing, and driving. Must have a valid Georgia driver's license and reliable transportation. Other Duties Perform other job-related duties as assigned by supervisors.
