The Provider functions as the primary mental health provider of the HBPC team, providing mental health prevention, assessment, treatment, management, and professional consultation services. Specific duties include: * Provides screening, assessment, diagnosis, and treatment of depressive and anxiety-related disorders, as well as other Axis I conditions, with an emphasis on the application of time-limited, evidence-based approaches. * Assesses and provides treatments for subclinical symptoms of dysphoria and anxiety, adjustment difficulties, and excessive use or misuse of alcohol or other drugs. * Provides psychological prevention services to patients at significant risk for psychological problems. * Provides services to the families of veterans with dementia being served by HBPC, when such care is directly related to the overall treatment plan for the veteran and will allow better care for the veteran and allow the family to sustain the veteran in the home environment. * Provides clinical services through the use of Telehealth technology. * Provides screening for cognitive deficits to address specific functional questions or to determine if a referral for a complete neuropsychological evaluation or dementia assessment is needed. * Performs capacity/competency assessments when there are questions about a patient's ability to make medical decisions, perform other specific functions, or live independently. * Assesses patients' understanding of advanced directives and other forms of consent and assisting staff in determining how to present treatment options and other such material to patients in a format they can understand. * Provides individual and group interventions to support patients that are coping with feelings of grief and loss associated with disability and other life transitions. * Provides psycho-educational and supportive interventions, in conjunction with the veteran, for spouses, family members/caregivers, and others that are crucial to keeping the veteran at home. * Provides effective interventions with couples or families, when appropriate, to relieve relationship difficulties, provide support to caregivers of veterans with dementia or other significant frailty, and/or promote collaboration with the HBPC team, for the benefit of the veteran. At all times, the necessity for and benefit of the intervention for the veteran should be documented and included as part of the veteran's treatment plan. * Facilitates transition and adjustment to new living situations (e.g., nursing home, assisted living facility), and providing seamless intervention services, as appropriate. * Provides behavioral medicine interventions to manage pain, disability, address sleep problems, facilitate weight loss, promote smoking cessation, and enhance medical compliance. * Promotes communication/interactions between medical team members, patients, and their families to facilitate the medical treatment process. * Provides clinical supervision to students, Interns, fellow, and residents. Assesses residents to evaluate needs and specific requirements. Involves the client, family, staff and referral sources in the treatment planning process through the presentation and discussion of the Treatment Plan in the treatment planning conference. Revises, as needed. Works with external agency staff (probation officers, case workers) to understand case history and identify specific treatment needs. Coordinates the implementation of treatment related activities. Major Activity: Performs Group, Individual and Family Therapy. May perform crisis intervention as needed Works and counsels with individual residents to effect and monitor treatment and progress. Utilizes group-counseling techniques to effectively monitor and enhance treatment progress. Major Activity: Maintains necessary documentation. Ensures compliance with clinical programming. Enforces facility rules and standards of behavior. Maintains and completes required documentation prior to the end of the shift. To include: Communication Log, Incident Reports and Progress Notes. Prepares reports as required by Abraxas and external sources. To include: Assessment Reports, Progress Reports, Court Reports, Safety Plans, Discharge Summaries, etc. Maintains necessary documentation as it relates to individual and group therapy. REQUIRED MINIMUM QUALIFICATIONS: At least 21 years of age. Fluent English and Spanish speaking Experience in social work, crisis intervention, and counseling within the substance abuse and mental health, to include juvenile sex offender and sexual abusing behavior youth. Thorough knowledge of current health/mental health issues and problems , available community/clinical resources and best practice therapeutic intervention skills. Excellent oral and written communication skills. A social worker licensed by the state of Ohio counselor, social worker and marriage & family therapist board and whose professional disclosure statement includes juvenile sex offender assessment and treatment; Bachelor's degree in the social science field and at least two years of related experience; A professional counselor licensed by the state of Ohio counselor, social worker and marriage & family therapist board and whose professional disclosure statement includes juvenile sex offender assessment and treatment; Bachelor's degree in the social science field and at least two years of related experience.
