The six core components of clinical case management include: processes and relationships, health care management, community resources and support, service delivery, psychosocial intervention; and rehabilitation case management. Work is full time during the weekdays Monday- Friday from 7:30am-4:30pm and requires some on-call and overtime work.Your key responsibilities will include: Collects information about the patient's situation and functioning to identify individual needs, and assessment of that information to develop a comprehensive case management plan. Plans the specific objectives, goals and actions designed to meet the patient's needs. This plan should be action-oriented and time- specific. Establishes, communicates, and facilitates initial program issues and goals with appropriate designated staff. Communicates with external stakeholders and families to ensure coordination of services. Title: Music Therapist (Director of Recreation) Job Duties: The role of the Music Therapist is to create and lead the recreational program with recreational activities, hobby & interest building, physical activities, and other events & holidays throughout the year. In our holistic approach, recreation is just one facet of the wellness of our clients along with behavioral skill building, medication support, dietary teaching, and pre-vocational training. Our overall goal is to support our clients to create lives worth living and be successful as independents in their community. Schedule: Full Time Qualifications: Must have a bachelor's degree as an occupational therapist, music therapist, dance therapist, art therapist, or recreation therapist. Must be registered or eligible for registration as an occupational therapist, music therapist, dance therapist, art therapist, or recreation therapist. Must have a valid California driver's license. Must have a current CPR certification is preferred. Must have two (2) years' experience in a social or recreational program within the past five (5) years, one year of which was full-time in a client activities program in a healthcare setting.
