DEPARTMENT: Women's Residential Services
REPORTING TO: Clinical Manager
Provides case management services to a target population of Ohio residents with active/historical use of substances which may include mental health disorders.
SCOPE OF RESPONSIBILITIES
Provides clinical therapeutic and case management services for adults with substance use disorders and/or psychiatric disabilities to assist them in improving their current level of functioning in the community.
Completes comprehensive case management assessments for identification of patient needs, level of functioning, support network, adequacy of living arrangements, financial status, physical health, level of self-care.
Works from and contributes to the care plan established by the primary therapist.
Ensure member has applied for benefits and health insurance such as SSI, SSDI, Medicaid or Medicare. Complete or assist in that process.
Ensure access to local resources, including psychiatric and medical care, housing, rehabilitation programs, drug/alcohol services, socialization activities, providing transportation and accompanying the client when necessary. Communicate regularly with other treatment providers.
Actively outreach clients on caseload who have not been in contact and ensure engagement in services in their home, transitional housing placement or on the streets.
May refer to or conduct support groups and teach classes on topics such as money management, vocational or job coaching, and life skills training.
Complete and enter all documentation into the patient/client's medical record according to The Crossroads Center's standards of practice in Electronic Health Record - CareLogic and/or Methasoft within proscribed timeframes as outlined in TCC documentation policy.
Meet weekly, monthly, and quarterly billing quota as established by the organization.
As time allows or as requested, assist in the daily operations of the Map, and/or residential programs.
Attend scheduled staff meetings, supervision and training as requested.
Work cooperatively with other team members.

Position Requirements

Two (2) years' experience providing services to adults who have been diagnosed with a substance use disorder and/or mental health diagnosis required.
Knowledge of the effects of trauma. Proven proficiency with both oral and written communication skills.
Organizational skills and the ability to complete multiple tasks a must.
Strong interpersonal skills and the ability to deal effectively with the public, other team members and elected officials.
A flexible work schedule is required in order to respond to clinical needs and other emergency situations.

The Behavioral Health Treatment Liaison serves as the primary link between community churches and other organizations, and the youth served in the TCC Adolescent Behavioral Health Program. This position is responsible for assessing behavioral health needs, facilitating access to appropriate mental health and substance use treatment, and coordinating care transitions to ensure youth clients receive comprehensive, continuous support.

Responsibilities:

**About the job:**
The **Registered Respiratory Therapist (RRT or CRT)**, within the Ohio Scope of Practice, provides therapeutic procedures and diagnostic testing to chronically and acutely ill patients requiring respiratory procedures.

Job Description
Position Information: The Treatment Manager provides support, coaching, mentoring, input, training and direction for the daily provision of clinical services based upon evidence-based practices for clients participating in services at The Crossroads Center (TCC). This includes residential services, outpatient services, medication assisted treatment, office-based opioid treatment and/or intensive outpatient services, outpatient group services and aftercare services. The Treatment Manager is responsible for the development, implementation and the standardization of services including curriculum and special programs for clinical services offered at The Crossroads Center.
Essential Duties and Responsibilities:
Works in conjunction with Chief Clinical Officer and Human Resources Coordinator to hire and terminate employees, complete performance reviews, corrective actions, performance improvement plans and conflict resolution.
Provides documented clinical supervision on a consistent basis for employees providing therapeutic services and peer support services.

Supports training efforts for clinical and non-clinical personnel.

Ensures coverage during absences to ensure consistent service provision to the clients.

Promotes services through establishing external relationships.

Qualifications:
Master's degree in social work and/or counseling.
Minimum of (3) three years of leadership experience in a clinical role with progressive leadership responsibilities serving clients with mental health and/or substance use disorders is preferred.
Experience in multiple treatment settings, e.g., inpatient, partial, outpatient, and/or residential a plus.
LISW/LISW-S or LPCC/LPCC-CS required.
Skills:
Demonstrated knowledge, use and implementation of evidence-based practices.
Experience with providing work/clinical supervision, and knowledge of applicable regulatory guidelines affecting the operations of a treatment program.

SCOPE OF RESPONSIBILITIES
Provides clinical therapeutic and case management services for adults with psychiatric disabilities to assist them in improving their current level of functioning in the community.
Outreach and engage potential members to evaluate appropriateness and desire for services in jails, hospitals, or street settings.
Completes comprehensive initial and ongoing assessments for diagnosis and evaluation of level of functioning, support network, adequacy of living arrangements, financial status, physical health, and level of self-care.
Assist clients in identifying needs, setting goals, establishing concrete objectives, and developing a coordinated care plan with a set timeframe from enrollment and annually.
Ensure the member has applied for benefits and health insurance such as SSI, SSDI, Medicaid, or Medicare. Complete or assist in that process.
Ensure access to local resources, including psychiatric and medical care, housing, rehabilitation programs, drug/alcohol services, socialization activities, providing transportation and accompany the client when necessary. Communicate regularly with other treatment providers.
Actively reach out to clients on the caseload who have not been in contact and ensure engagement in services in their home, transitional housing placement or on the streets.

Work cooperatively with other team members including sharing responsibility for 24 hour on-call coverage.
