    Profiles Needed: 9/5 (up to 8 candidates) Family Nurse Practitioner (FNP) - Emergency Department & Ambulatory Care Contract Type: Non-personal services Period of Performance: 10/01/2025 - 09/30/2026, with option year through 09/30/2027 (Assignments may include Emergency Room, Adult Ambulatory Care, Thoreau Health Clinic, Pueblo Pintado Clinic) Shift Details: • ED shifts: 10-12 hours, typically starting 8:30 or 9:00 a.m. (daily, including weekends/holidays) • Clinic hours: 7:30 a.m.-6:00 p.m. or 8:00 a.m.-4:30 p.m. • No on-call required • Must be flexible for additional hours if needed Scope of Work: • Provide urgent/acute care in ER Fast Track • See adult outpatients in ambulatory clinic • Same-day clinic coverage at Thoreau or Pueblo Pintado (all ages) • Manage diabetes, hypertension, chronic care needs, refills, referrals • Perform minor procedures (laceration repair, splinting, I&D, toenail removal) • Enter referrals and documentation in EHR • Complete charts within 72 hours (pay may be held if records are incomplete) • Attend daily 10 a.m. ER staff huddles • May assist with triage, COVID-19 testing, and vaccinations Qualifications: • Graduate of accredited NP program • Unrestricted U.S. state NP license and DEA • Board certified (AANP or ANCC) • Current BLS, ACLS, PALS (PALS strongly preferred) • Experience across ED and outpatient care preferred Support: • Physicians always available in ED for consultation • Support staff: RNs, MAs, NAs, MDs/DOs (staffing dependent) • Specialty services at Crownpoint: optometry, dental, speech, PT, inpatient unit, women's health, pediatrics • No specialty services at Thoreau or Pueblo Pintado clinics Housing/Travel: • Government housing may be available, subject to vacancy • Otherwise, provider responsible for housing, per diem, and travel
