DUTIES
Conduct home visits and phone contacts with the dual purpose of assessing the safety and appropriateness of the child's placement with their caregiver (sponsor) and connecting the child to services in the community to address risk factors.
Ongoing case management with a focus on screening and making appropriate referrals to address child's needs in the following domains: Placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, and child enrichment activities.
Provide crisis intervention services directly, as needed, or connect the family to services to ensure the minor's safety and well-being in a crisis situation.
Submit detailed and thorough documentation in compliance with Global Refuge policies and procedures.
Utilize social work best practices for thorough assessments of the child's progress and services provided.
Maintain regular and timely communication with the supervisor around any issues concerning the child's safety, stability, or well-being in the placement.

QUALIFICATIONS
Bachelor's degree in Social Work, or other relevant degree or equivalent experience

Job Description
TITLE: MA Level Supportive Counselor SUD
RESPONSIBLE TO: Clinical Director
SUMMARY: The MA Level Therapist's primary role, under the direction of the Clinical
Director, is to utilize their specialized clinical knowledge and clinical skills in counseling,
diagnostic interviews, assessments, ASAMs to assist individuals struggling with a substance use
disorder in their recovery journey. The Substance Use Disorder Therapist will provide therapy
sessions for individuals, groups and families.
DUTIES AND RESPONSIBILITIES:
Conduct thorough initial assessment and complete the initial ASAM to determine the extent
of the substance use disorder and to assure proper placement within the level of care for
which the client is recommended.
Provide evidence-based individual and group counseling utilizing various therapeutic
approaches tailored to the client's needs.
Establish clear goals and strategies for the welfare and recovery of the client and maintain
documentation of services provided and client's progress/regress.
At each session the Therapist will assess for safety, imminent risk of harm and relapse
potential and make referrals to higher levels of care if needed.
Work with the client to develop an individualized treatment plan (ITP) according to OMHAS
and CARF person/family-centered standards, review and monitor services and activities,
documenting progress/regress.
Complete Continuing ASAMS with clients every 30 days or when a significant event occurs
to determine client is placed within the appropriate level of care.
Offer education and guidance on the physical, psychological, and social effects of
substance use disorder, as well as develop strategies for relapse prevention.
Collaborate with a multidisciplinary team of health professionals, including physicians,
psychiatrists, psychologists, social workers, and nurses, to provide integrated care to
clients with co-occurring disorders.
Collaborate with courts, probation/parole officers, law enforcement, and child protective
services agencies to assure client safety and progress/regress within the client's recovery
journey.
Maintain accurate, up-to-date client records, documenting treatment interventions,
progress, and outcomes in compliance with legal and ethical standards.
Conduct regular reviews and consultations with clients to assess treatment effectiveness
and address any challenges or concerns.
Provide crisis intervention services to clients in urgent situations, coordinating appropriate
care and support as needed.
Provide random instant urine drug screens as needed.
Engage in continuous professional development by attending workshops, seminars, and
training sessions to enhance knowledge and skills.
Uphold ethical standards and confidentiality in all interactions with clients and colleagues,
in accordance with relevant state and federal laws.
Make appropriate referral if identified deficiency is outside of the Therapist's scope of
practice. (MH/ Physical Health).
Identify behaviors that interfere with treatment and recovery.
Day-To-Day Team Member Responsibilities:
Willingly serve in a professional and collaborative capacity with all other employees of Path

QUALIFICATIONS:
Education: Master's level degree in Human service/Behavioral Science related field.
Experience: Candidates must have:
Minimum of 1 years' experience in an outpatient setting.
Previous experience with the treatment of substance abuse disorders.
Application of the Substance Abuse and Mental Health Services' (SAMHSA) best practice
indicators.
Physical Effort: This position requires the ability to work under stressful conditions and to work
irregular hours. Requires sitting for extended periods of time, viewing computer monitors, and
keyboarding.
Communication Skills: Candidates must possess excellent verbal and written communication
skills to communicate professionally via telephone and in writing. Strong grammar and writing skills
are crucial for clear and concise communication with clients, colleagues, and other healthcare
professionals.
Essential Technical/Motor Skills: Candidates must achieve competency in the Electronic Health
Record (EHR) program, telemedicine services, and any additional software programs utilized by the
company, ensuring efficient access and updating of client information. The role requires fine
