We're looking for an experienced Area Vice President of Business Development to lead and inspire high-performing sales teams, drive new market growth, and strengthen referral networks across multiple territories.
What You'll Do
Direct and coordinate sales operations across assigned territories, including pipeline development, referral relationships, and performance outcomes.
Build, coach, and mentor regional sales teams to achieve and exceed growth goals for admissions, census, and revenue.
Lead the design and execution of market-specific sales strategies, leveraging data and industry trends to identify new growth opportunities.
Foster and maintain referral relationships with physicians, hospitals, skilled nursing facilities (SNFs), assisted living communities (ALFs), and other healthcare organizations.
Support recruitment, onboarding, and ongoing training of business development staff, ensuring they are equipped with effective sales techniques.
Collaborate with clinical and operational leaders to align sales strategies with patient care goals and operational capacity.
Monitor and report on team KPIs, sales funnel health, market share, and territory performance metrics.

Ensure compliance with company policies, industry regulations, and ethical standards in all sales and marketing activities.
Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity. About You
Education & Certification
Bachelor's degree required (Business, Marketing, or Healthcare preferred).
Valid driver's license and automobile liability insurance required.
Experience
5+ years of direct sales experience in the healthcare industry, including home health or hospice.
2-4 years of sales management experience in a multi-site or multi-territory environment.
Proven track record of meeting or exceeding admissions and census targets in a healthcare sales role.
Experience working in a metrics-driven sales environment with CRM, EMR, and data tracking tools.
HomeCare HomeBase experience preferred.

Key Responsibilities:
Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes.
Document clinical findings accurately and attest F2F encounters in patient medical records.
Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements.
Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians.
Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families.
Provide clinical recommendations to the IDG based on F2F assessment findings.
Conduct assessments in the patient's place of residence, including home, skilled nursing, or other settings.
Notify the hospice agency promptly if unable to complete F2F visits as scheduled.
May provide F2F coverage for multiple provider numbers with approval.

Key Responsibilities
Patient Engagement and Outreach

Obtain verbal consent and enroll eligible patients into CCM programs.
Care Coordination and Documentation
Develop, update, and maintain individualized care plans based on patient needs and provider direction.
Track and document all interactions, care activities, and care plans in the EHR or care management platform to meet documentation and billing standards.
Coordinate follow-ups, referral tracking, and patient reminders for appointments and preventive services.
Identify social determinants of health and connect patients with appropriate community resources.
Collaboration and Quality
Work with primary care providers, nurses, and care teams to share updates and escalate clinical concerns.
Participate in care team meetings and contribute to continuous quality improvement efforts tied to chronic care measures.
Maintain compliance with CMS CCM requirements and organizational protocols.
Required Qualifications
Clinical background (Medical Assistant, LPN, RN) or equivalent experience in care coordination or healthcare support roles.
Experience with chronic disease management, patient education, or care coordination.
Strong communication skills and comfort engaging patients by phone and digital tools.
Ability to work independently in a remote environment with reliable internet access.
Proficiency with electronic health records and care management platforms.
Preferred Qualifications
· Prior experience in Chronic Care Management, Remote Patient Monitoring, or similar population health programs
· Knowledge of CMS CCM billing, documentation, and time tracking requirements
· Experience working in an electronic medical record (EMR) system, preferably in a primary care or ambulatory setting
· Comfort navigating multiple technology platforms while delivering remote patient care
· Bilingual skills a plus
Work Environment
Fully remote, regular business hours with occasional flexibility based on patient needs.
