As a result, NYAP is seeking a fulltime CME Moderate Care Coordinator which will cultivate flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around model of care coordination covering all of Darke County and overlap into Shelby and Miami Counties as needed.
Under direct supervision of the Care Coordination Supervisor, this employee will aim to achieve the ultimate goal to keep youth in their homes, communities, and schools by assessing and delivering the appropriate services needed and reducing unnecessary out-of-home placement and potential custody relinquishment.

* Cultivate flexible, family-focused, community-based responsive services based on the High Fidelity Wrap Around model of care coordination
* Develop and maintain the Wraparound Team, including coordinating and leading team meetings

(i) three years with a high school diploma or equivalent; or

Essential Duties & Responsibilities:
You will be responsible for receiving and responding to web based electronic inquiries (leads), working with our existing customer data base, incoming sales phone calls that result from prospects and customers visiting the dealership website, associated web sites and other lead sources. The Internet Sales/Business Development Representative will be evaluated based on performance standards that measure how many leads were converted to showroom visits and how many of those showroom visits resulted in a sale. You will utilize and/or learn both traditional and Internet-specific automotive sales processes.
As a Business Development Representative with LaFontaine, you can expect an hourly pay rate plus commission-based compensation, as well as training and a benefits package. Execute customer communications via email, telephone, and text and at times face-to-face, in line with standards and processes.
Set dealership appointments, achieve lead-to-Dealership conversion and sell products based on department standards.
Enter all prospect communications, notes, records, and customer appointment data into the CRM.
Maintain tracking boards showing Internet sales updated each day of the month.
Prepare reports showing appointments scheduled, customer appointments kept, and customer sales based on leads received.
Attend regular sales meetings for both the dealership and the eSales department.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer.
Must be able to lift 15 pounds at times.
Must be able to access and navigate each department at the organization's facilities.
