Role Description:
The purpose of a CommunityHealthWorker (CHW) at Oak Street Health is to act as the bridge between our patients, community, and medical systems in order to remove barriers and increase wellness across all life domains. A CHW is a patient's advocate or liaison, accompanying patients through proactive in-person and phone outreach based on their care needs to promote health literacy and increase access to resources needed to live healthier lives. High levels of flexibility, problem solving, strong communication, and an intimate knowledge of the community served are required to be successful.
CHWs work closely with Medical Social Workers to manage patient care plans, support care team decision making, and coordinate clinical and complementary services needed to provide high quality health care and improve the quality and cultural competence of service delivery. CHWs are expected to work within their scope of practice. There is no expected clinical license for this position.
Core Responsibilities:
Establish and maintain strong interpersonal relationships with patients, community organizations, team members, and partners to coordinate patient needs
Manage patient referrals defined by the care team & collaborate with the Medical Social Worker on action plan
Facilitate communication between all identified parties involved in patients' care as needed (e.g., family members, caregivers, medical providers, community-based organizations)
Form relationships with and build an inventory of local community organizations that may benefit our patients
Connect patients to state and local community resources related to housing, transportation, food, and activities of daily living among other social and physical barriers to health.
Assist patients with completion of applications for accessing eligible benefits and resources
Promote goal setting and achievement to improve patients' quality of life and self efficacy with patients. Goal definitions are agreed upon by the care team
Meet with patients in patient-centered and patient-preferred locations (e.g., Oak Street Health center, patient's home, external medical provider facility, community setting)
CommunityHealthWorkers should plan to spend about half of their time outside of the center in patient-centered locations; this means having access to a reliable means of transportation to do so is required
Drive engagement with high risk individuals (e.g., completed specialty appointments, adherence to Post Discharge Visits) may include accompaniment to appointments
Complete referrals to organizations and agencies as needed
Deliver culturally appropriate health education in the areas where OSH has provided competency training to the CHW
Support care team decision making through participation in interdisciplinary team meetings
Document interactions with patients in electronic medical record in a timely manner while maintaining HIPAA standards and confidentiality of protected health information
Manage time, set priorities, work independently, and collaborate effectively with an interdisciplinary medical team

What we're looking for
Required:
Minimum of 1 year of experience in healthcare, community-based, case management, or social service environment
Strong oral and written communication skills
Ability to manage multiple priorities while maintaining a positive attitude

The typical pay range for this role is:
$18.50 - $31.72
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Summary Description:
Coordinates the oversight of surgical instruments and supplies between the OR and Central Sterile Processing (CSP) to ensure that requested items are available and to help facilitate efficient case flow. Responsible for assisting in special projects and process improvement initiatives when needed. Promotes customer service and satisfaction through training, education, communication and resolution. Perform duties in accordance with hospital, TJC, CMMS, FDA, AORN, AAMI and other appropriate hospital and department specific policies and procedures.
General Duties:
1. Assists the Operating room staff in preparing the OR suite for surgery by verifying instrument trays are available prior to case start times.
2. Assists in the transportation of sterile instruments and supplies to the operating room using appropriate standards and techniques. Perform package integrity checks on all items to make sure that the package and/or wrapper is not compromised.
3. Keeps track of case flow during the day, monitor instrument needs and notify CSP personnel in advance of any possible addition / emergent instrument needs. Must be able to resolve issues or offer alternatives when necessary.
4. Assist in removing soiled case carts or un-sterile instrumentation and scopes from the operating room to be returned to CSP for processing.
5. Assist with planning and coordination of instrument turnarounds with the CSP staff. Help communicate to the surgical team essential instrumentation information that may affect or delay surgery start time.
6. Verifies the instrument needs for the next day and that they have been cleaned, wrapped, sterilized and available for use.
7. Demonstrate a working knowledge and understanding of surgical instruments and be able to identify when instruments and broken, damaged or not functioning as designed. Report these issues to the CSP management team.
8. Communicate to the OR coordinators suggestions related to surgeon preference card changes based upon set usage to help ensure availability.
9. Working with the OR staff identifies instrumentation related to tray additions and reconfigurations, building surgeon specialty sets, etc. Report recommendations to the CSP Manager.
10. Responds to problem calls from Surgery throughout the day as required.
11. Perform required daily audits of completed and post-op case carts. Provide completed audit sheets to the CSP data entry clerk.
12. Participates in education of health care providers related to receipt, inspection, inventory, storage & distribution of instrumentation and medical supplies in Processing, function of specialty instrumentation, problem resolution of frequently encountered instrument function issues and instrument maintenance program.

Shift information: Flexible Day Shift, at least one evening a month (11-7), one weekend day every 4 months, 2 of the 6 major holidays
Clients: Adults within the CPR program who join Friends in Action Clubhouse
Location: Onsite at the Friends in Action Clubhouse. Our EWH (evening, weekend, holiday program) may be offsite at a park, in the community shopping/out to eat, at a parade, etc.
Who will this person interact with on a regular basis? All Clubhouse members daily who attend, we have about 35-50 members a day at the Clubhouse.
MUST HAVE skills for this position: self-starter, ability to lead a large group, ability to move from task to task, ability to guide/cheerlead a Clubhouse member to get that person started on a task and then move on to the next member, then back to the first member. This open position is for the Business Unit so would prefer computer skills including Excel, Publisher, etc.

Management of Clubhouse Units- Organize and complete daily unit work with membership side by side/oversee assigned areas- Apply and model the Clubhouse Standards, as stated through the Clubhouse International - Creatively encourage members to participate in their rehabilitation through the WOD by making Clubhouse work urgent and meaningful - Plan and participate in the EWH program (Evening, Weekend, and Holiday activities) - Actively participate in the fundraising efforts of the Clubhouse - Complete frequent outreach of members who are not in attendance - Assist to meet the community needs of members - Provide crisis intervention services as needed

Position Summary
The Provider Data Services Senior Coordinator will have responsibility to support loading all Medicaid new and revised participating provider data transactions within Aetna/CVS source systems.
What you will do:
Daily activities include writing simple SQL queries and pull reports on both an ad hoc and production basis for analysis of data present in QNXT/Source systems
Proactively define, identify and recommend process enhancements or modifications to support business strategic goals
Candidate must be proficient in SQL, able to write and modify queries, interpret results, conduct root cause analysis (RCA), develop short and long-term solutions following through to completion
Ability to adapt swiftly to ever changing data sequences and business needs
Perform data compares
Experience working with Large Data Sets
Follows area protocols, standards, and policies to provide effective and timely support.
Ability to learn quickly and work with minimal supervision
Supports a positive team environment
Required Qualifications
1-3 years experience using Microsoft Excel (VLOOKUp, XLookup, concanation, formulas, etc.)
1-3 years experience using SQL
Proficiency with desktop software applications such as Microsoft Word, Teams and OneNote

1 year of experience in Provider Data Services systems such as QNXT or Salesforce

The typical pay range for this role is:
$18.50 - $38.82
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
