POSITION SUMMARY: A Supports Coordinator provides family-focused services to children with developmental disabilities for the purpose of assisting the child in building on their strengths and helping the family to remain safely together in the family home. The Supports Coordinator will work in an environment convenient to the child and family to assist with linking, monitoring, and coordinating services. ESSENTIAL RESPONSIBILITIES: To provide support and resources to families and children diagnosed with developmental disabilities. Able to meet clients and their families in non-traditional settings, (i.e. client's home or public place that can accommodate privacy) during hours that meet the family's needs. Participate in at least bi-weekly supervision at minimum and participate in bi-weekly team meetings. To effectively coordinate, educate and train family members and support systems on individual needs of each child. Demonstrate self-care strategies and model healthy coping mechanisms when exposed to emotionally challenging circumstances To monitor individuals for their progress on goals as well as to provide additional support and/or resources. Demonstrate sensitivity to cultural, ethnic and linguistic differences in all interactions with youth and families. Work skillfully and tactfully with other people significant to the client including families, schools, doctors, and other systems within the client's life. Support families in navigating systems that may not reflect their cultural values, advocating for culturally responsive care. Acts as a cultural bridge between youth, families and community groups from different backgrounds. Maintain program compliance in timeliness of services and required paperwork completion. Provide crisis intervention and on-call response services as assigned by supervisor. Maintain coordination for treatment planning with multiple services providers. Supervision Received: Specific Direction - Follows established work procedures. Receives periodic checks for performance. Refers unique questions/conditions to immediate supervisor. Supervisory Responsibilities: No supervisory responsibilities. MINIMUM QUALIFICATIONS: Minimum education requirements include a 4-year degree in a human services field. One year's experience working with people who have been diagnosed with a developmental disability preferred. Possess a knowledge of community resources and support for children and families, and the skills necessary to facilitate and encourage positive change towards treatment goals. Valid driver's license, good driving record and adequate insurance with a car available always. Basic computer skills with knowledge of Microsoft Outlook, electronic record systems and other software programs. The ability to articulate ideas and prepare written reports and correspondence that are of a high professional standard, are grammatically correct and are timely. POSITION SUMMARYThe Clinical Case Manager II provides intensive targeted and coordinated planning, linking, training and advocacy support to youth ages 0-18 and foster families to preserve placement. The focus of services is to stabilize the child(ren) who is/are experiencing significant emotional and/or behavioral regulation issues leading to consideration of residential treatment OR to facilitate their discharge from residential treatment. The Clinical Case Manager is responsible for ensuring children and foster families are assessed, monitored, and engaged in a service plan designed to enhance stability, wellness, and growth. This includes facilitated referrals for and monitoring of behavioral health or other therapies for the child, information, skill building and support for foster parents, and coordination with education and other community systems. As caseload permits, this level II position also assists with therapy and case management cases in the Behavioral Health Team. The EFC Clinical Case Manager II reports directly to the Clinical Treatment Coordinator. ESSENTIAL RESPONSIBILITIES Provide services to a maximum caseload of eight EFC-authorized children, according to an Initial Service Agreement (ISA) Complete monthly face-to-face contact expectations with families based on level of care. Complete Clinical Pathways screening process for assigned children. Screen and assess for behavioral health or other service needs, facilitate referral for necessary services, and help to remove barriers to service access and engagement. Able to meet clients and their foster families in non-traditional settings, (i. e. client's home or public place that can accommodate privacy) during hours that meet the family's needs. Maintain weekly contact with the assigned Behavioral Supports Specialist (when applicable) to support his/her role in achieving the child's goals of improved and appropriate behavioral, emotional, social, and/or educational functioning. Work collaboratively with foster care staff, agencies, programs, services, and individuals who have important roles in the child's life situation. Communicate with each service provider to monitor a child's progress in treatment or other services and assure coordination of all services to the child. Coordinate and collaborate with the educational system, maintaining regular contact with the child's school. Provide information, skill building opportunities and support to Foster Parents to increase their confidence and capacity caring for the unique needs of the child(ren) in their home. Identify and analyze any identified issues in services, school and/or the foster home and develop a plan of response; engage foster care staff, service providers, foster parents, school personnel and others as necessary to facilitate successful resolution. Participate in weekly team/peer meetings and at minimum monthly individual supervision with demonstrated ability to implement feedback from supervisor. Participate in 24-hour system of crisis intervention and support, including detailed information to access after-hours support; assist with safety planning; assure on-call staff have relevant and up-to-date information to assure appropriate and effective response. Convene and facilitate Care Coordination sessions as necessary, but no less than monthly. Facilitate Initial Service Agreement with all involved persons and agencies no less than monthly. Provide recommendations regarding changes in level of care. Complete all necessary initial documentation and quarterly reports according to agency, accreditation, contract, licensing, and other regulatory stands by due dates. Attend and testify, as requested, at all court hearings on assigned cases. If a youth is placed in an Enhanced Shelter Home, clinical case manager will do the following: Participate in weekly ESH call and provide updates on placement efforts. Maintain daily phone or email contact with caregivers and youth. Conduct a minimum of two face-to-face visits with the caregiver and youth per week. Complete a safety plan for both the youth and the caregiver within 72 hours of admission to an ESH, documenting the plan on the Initial Safety Plan Form. Complete Assessment Report, which includes individual youth caregiver safety plans, by day seven and weekly thereafter Tasks required for Behavior Support Specialist Role of EFC: Implement designated portion of the service agreement. Create and maintain a schedule with assigned children and families; provide schedules to assigned EFC CCM. Communicate with assigned EFC CCM to each case at least weekly to discuss goals, progress and/or concerns specific to each case. Contact Clinical Treatment Coordinator and EFC CCM with any urgent safety or service concerns. As needed by Behavioral Health team and based on capacity, clinical case manager will service Home-Based, Case Management, and Outpatient cases from network 180. When this occurs, the following tasks will need to be completed: Meet monthly unit expectations for assigned level of care. Complete intake forms, interim plan, assessment, individual plan of service, service plan reviews, and discharge summaries by contractual due dates. Complete progress notes within 7 days of service delivery Utilize a variety of therapeutic treatment models, skills, and interventions that best fit a client's individual needs and/or family's needs based on your level of training. A preference should be demonstrated towards the use of evidence-based therapies (i. e. Applied Behavior Analysis, Cognitive Behavioral Therapy, Motivational Interviewing, and Dialectical Behavior Training). If contacted during business hours, therapist would assess and complete a crisis screen for safety planning and/or potential hospitalization. Additionally, therapist would be expected to occasionally assist with covering on-call during afterhours when there are unfilled weeks on on-call schedule because of vacancies. Complete agency required expectations such as timesheets and mileage reimbursement by required deadlines. Additionally, the case manager should keep Outlook calendar up to date with client appointments. It is expected that the case manager will work at least 40 hours per week. Will complete a minimum of 24 child and family training hours per calendar year. Other responsibilities shall include those set forth in agency personnel policies as amended. Supervision Received: Specific Direction - Follows established work procedures. Receives periodic checks for performance. Refers unique questions/conditions to immediate supervisor. Supervisory Responsibilities: No supervisory responsibilities. MINIMUM QUALIFICATIONS QUALIFICATIONS Master's degree required in Social Work, Counseling, Psychology, or Marriage & Family Therapy. License to practice mental health therapy in the State of Michigan. If temporary or limited licensed, must be completing steps towards full license. The supervisor supports programming that addresses a wide range of behavioral health needs-such as depression, anxiety, trauma, self-harm, aggression, and other emotional or behavioral challenges-and promotes strong collaboration with schools, residential teams, and community partners. In addition to direct supervision, the Behavioral Health Supervisor maintains responsibility for monitoring caseloads, outcomes, and contractual requirements across service lines, ensuring consistent practice standards and efficient program operations. The position provides coaching, reflective supervision, and professional development to therapists; oversees data collection and reporting; and helps strengthen integrated care pathways for youth receiving services through Child Welfare, Residential, and community programs. ESSENTIAL RESPONSIBILITIES Assist the Clinical Program Manager in the hiring of new staff assigned to the team Employ an ongoing process which evaluates the functioning, needs and effectiveness of assigned staff members on at least a quarterly basis. Support the TF-CBT service by providing guidance to model adherence for staff in training and staff who have completed model training. The initiative goals are to increase support for students, families, and schools; increase access to health, mental health, and community resources; increase parent/community engagement and involvement; to promote opportunities that support student's educational, social and emotional development; and to offload the non-education issues from the principal and teachers to allow them time to focus on educating the children. The Team Clinician focuses on meeting the mental health needs of the children and families by providing in home or outpatient therapy and/or connecting families to other mental health resources that best meets their need. These issues can include depression, anxiety, child abuse effects, harming oneself, slowness in learning, eating disorders, phobias, aggression and other symptoms or difficulties in behavior and feelings. The Clinician helps children assess their issues by observing their behaviors and assessing what treatments will best benefit the child. The KSSN Site Team Clinician provides two levels of services and acts as a Network 180 Access center to refer to others. The KSSN Site Team Clinician provides both outpatient and home-based services to clients that are referred. Group therapy is also utilized as needed. The STC may also be involved with the school's Attendance Team and Community School Leadership Team. ESSENTIAL RESPONSIBILITIES Assess, counsel, educate, advocate for, and support referred children and families. Complete all necessary Agency, CMH, and other miscellaneous paperwork requirements within deadlines per program and contract requirements. Able to meet clients and their families in non-traditional settings, i.e. client's home, restaurant, or public places. Reviews resources and materials for cultural relevance and accessibility for specific children and families. Meet with clients a minimum of required contacts per month as set by contract and program requirements (Work closely with the KSSN Community School Coordinator to meet the needs of the school as deemed appropriate by school staff and supervisor. Complete nonbillable contact hours per week in your school building per KSSN contract. Acts as a cultural and linguistic bridge between children, families and community groups from different backgrounds. Provides direct service to include intake, assessment, individual/family therapy, and crisis interventions to client's and provide proper and timely documentation of these services in the client record. Manages a mixed caseload of Outpatient, Outpatient Bundle, Case Management, Trauma Focused Cognitive Behavioral Therapy (as certified), and Home-Based services and maintain monthly unit expectations for each service as deemed by contract and program requirements Provides services both on-site at the school facility and off-site to include the client's home support network per caseload requirements as deemed by program Maintain monthly contact with all parents/guardians of each client. Completes initial treatment plans, treatment plan updates, treatment authorizations, quarterly reports, and discharge summaries within required deadlines. Participate in family education, training, and support group activities when needed. Utilize a variety of treatment models, skills, and interventions that best fit a client's individual needs and/or family's needs. Participate in required case-related activities such as school conferences and court hearings. Advocates for children and family needs at an organizational and facility level. Agency-related duties include required attendance at team meetings, cooperation with other departments within the Agency, timely completion of agency documents (to include expense voucher, ADP weekly time record and PTO, Outlook Calendar, etc., monthly data, etc.). Rotate the 24 hour / seven day a week on-call cell phone on an ongoing basis with fellow Site Team Clinicians and Behavioral Health Staff. While on call: Will cover the on-call phone Friday to Friday, 24 hours Will be within 1 hour of Network 180 Will remain drug and alcohol free Staff will be accessible, maintaining flexible hours, seven days a week.
