**Primary Duties and Responsibilities:**
1. Practices as a pharmacist in accordance with all federal, state, DFCI and Joint Commission policies, regulations, and standards.
2. Ensures medication orders and/or prescriptions for appropriateness, accuracy and completeness based on patient-specific parameters and clinical data.
3. Provides clinical consultation and evidenced based recommendations for treatment plans, supportive care, dose adjustments and monitoring plans to clinicians regarding pharmacotherapy needs for patients based on literature, clinical practice guidelines and institutional standards.
4. Collaborates as a member of the pharmacy and interdisciplinary team through patient care related meetings including rounds, huddles, and case conferences as applicable.
5. Provides comprehensive education to patients and caregivers receiving oncology related therapy including participation in the oral anticancer education program and review and maintenance of patient education materials.
6. Documents clinical care activities in the EHR and effectively communicate all changes in care plan to the staff involved in patient care.
7. Participates in scholarly and educational activities according to departmental expectations (e.g., in-services, drug use evaluation, resident precepting, clinical research, quality/process improvement, presents continuing education programs, guideline development).
8. Participates in clinical administrative duties such as pathways, beacon validation and active patient management, formulary request process and presentation to P&T committee(s), evaluate off label/exception review, REMS programs and other departmental or strategic initiatives.
9. Participates in the development of policies, procedures, teaching documents, education and/or monitoring tools, workflow design, and other initiatives that will improve and/or expand the program.
10. Other duties and projects as assigned.

**Minimum Job Qualifications:**
The Clinical Services Pharmacist II must meet the following requirements to be hired and maintain their position in the Pharmacy Department at DFCI.
Minimum Education:
All Pharmacists require a Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of Pharmacy.
Minimum Experience:
PGY2 residency in area of specialty (preferred) or PGY1 plus 2 years of clinical specialist experience (such as oncology, palliative care, anticoagulation) or 5 years of clinical specialist experience (such as oncology, palliative care, anticoagulation) required.
**License/Certification/Registration Required:**
Must be licensed to practice pharmacy in the Commonwealth of Massachusetts. Pharmacists must attain board certification in their specialty area within 18 months of employment.

Job Summary
Under direction of the Senior Operations Manager, PET Production and Cyclotron, the nuclear pharmacist provides a variety of services, ensuring compliance in the procurement, preparation, compounding, manufacturing, quality control testing, dispensing, distribution, and monitoring of radiopharmaceuticals. The nuclear pharmacist also must provide support for the safe and efficient production and use of radioactive drugs for research and clinical diagnostic use at MGH. The incumbent is involved in all aspects of the day-to-day operation of the PET Production and Nuclear Pharmacy radiopharmaceutical facilities. In addition, the nuclear pharmacist provides consultation regarding all aspects of the use of radiopharmaceuticals and non-radioactive adjunct imaging medications, as well as health and safety issues in patient care.
Duties and Responsibilities for Nuclear Pharmacists in the MGH PET Nuclear Pharmacy include but are not limited to:
1. Assumes overall responsibility for quality oversight of the FDA regulated, cGMP PET manufacturing facility and PET nuclear pharmacy for the manufacturing, compounding, quality control testing, dispensing and packaging of PET radiopharmaceuticals.
2. Serves in a quality assurance (QA) role to ensure compliance with all departmental procedures, FDA guidance and regulatory filings.
3. Reviews and releases batches of PET radiopharmaceuticals manufactured under 21 CFR 212, cGMP PET drug manufacturing regulations.
4. Ensures compliance with all state, federal, and local regulations concerning these activities.
5. Maintains inventory and ensures suitability of all raw materials and components used in the production of PET radiopharmaceuticals
6. Performs set up, operation, and documentation of computer controlled automated synthetic devices dedicated to the production of PET radiopharmaceuticals.
7. Performs complex analytical testing on manufactured and/or compounded radiopharmaceutical including, thin layer chromatography (TLC), high performance liquid chromatography (HPLC), gas chromatography (GC), radionuclide identity and purity testing with high purity germanium solid state detectors, sterility and pyrogen testing.
8. Works with PET radiochemistry staff to troubleshoot radiopharmaceutical production and optimize yields.
9. Works with cyclotron facility staff to ensure quality and availability of PET radionuclides for manufacturing and compounding of PET radiopharmaceuticals.
10. Maintains detailed records of operation and validation of all processes taking place in the nuclear pharmacy and PET production facility.
11. Ensures that all shipments of radioactive material meet all regulatory requirements.
12. Works with MGH radiation safety staff to ensure that all tasks are performed in the PET nuclear pharmacy are in compliance with regulatory and prudent radiation safety principles.

Job Summary
GENERAL SUMMARY/ OVERVIEW STATEMENT:
Massachusetts General Hospital is currently seeking highly motivated candidates to join our clinical transplant pharmacy team. This clinical position is for a Cardiothoracic Transplant Clinical Pharmacist.
The Cardiothoracic Transplant Clinical pharmacist will rotate through the inpatient heart and lung transplant service lines, serving as an expert of transplant pharmacotherapy for the interdisciplinary transplant teams. This position requires application of advanced transplant training to lead: immunosuppressive and opportunistic infection discussion on interdisciplinary rounds, transplant education for patients and healthcare professionals, transplant protocols and guidelines, quality improvement and research projects, and transplant training for the PGY-2 transplant resident and additional learners. The Cardiothoracic Transplant Clinical pharmacists are recognized as exceptional resources to the Pharmacy, Transplant, Cardiology and Pulmonology Departments, with whom they collaborate closely. This is a weekday position (no evenings) with every 10th weekend rotation plus three holidays per year (rotates every year).

* Complete critical patient monitoring and review patient chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, subtherapeutic dosage, over dosage, adverse drug reactions, drug interactions, untreated indications, medication use without an indication, and treatment failures
* Follow established standard work parameters and maintains standard work environments
Participates in pharmacy operations and medication dispensing:
* Maintain proficiency in and actively engage in operational programs, central dispensing pharmacies, satellite dispensing pharmacies, and specialty pharmacy areas as appropriate for job assignment

Job Summary
The MGBMG Pharmacy Department aims to expand clinical pharmacy services across the Mass General Brigham Medical Group (MGBMG) locations in a way that enhances the patient experience, health outcomes, and quality. One of the key goals is ensuring that our patients with chronic diseases are supported in achieving clinical outcomes known to reduce morbidity and mortality from these conditions. Pharmacists can play a key role in this goal by offering higher touch services (medication prescribing, monitoring, education) to patients that are struggling with chronic disease management. Working with primary care providers, population health coordinators, high-risk nursing programs, and community health workers, the pharmacist has the potential to positively impact patients through virtual, in-person appointments, and telemedicine.
In collaboration with the MGBMG team, the Ambulatory Clinical Pharmacist will monitor the medication use process in accordance with the laws and regulations governing credentialed pharmacists and the practice of pharmacy by the appropriate state and federal agencies. They will work relatively independently, and within the limits of established pharmacy/ambulatory care practice site policies and procedures, as a member of the patient care team. The Ambulatory Clinical Pharmacist will report to the Clinical Pharmacy Manager at MGBMG and be responsible for working at the top of their licensure in collaborative drug therapy management (CDTM) work, as well as in-service presentations, clinical consult work, collaboration with our pharmacy team, and development of work to assist in the expansion of our department.

* Graduate of ACPE accredited college of pharmacy
* Doctor of Pharmacy (Pharm.D.) required

Principal Antineoplastic Stewardship Duties and Responsibilities

d. Developing guidelines, policies, and other monitoring and intervention strategies for antineoplastic therapies
e. Compiling data and metrics on antineoplastic use
f. Working with the Oncology/Antineoplastic Subcommittee to maintain and update the network/institutional guidelines,
policies, and protocols
g. Reporting data and findings to the Oncology/Antineoplastic subcommittee and other stakeholders.
h. Coordinating, facilitating, and promoting education, training, competency assessment and performance improvement of
pharmacists, pharmacy residents and other healthcare professionals in the appropriate use of antineoplastics

The Massachusetts General Hospital is seeking highly motivated pharmacists to join our expanding team of Primary Care Pharmacists. This position offers a rare and exciting opportunity to contribute to the strategic development and implementation of new clinical pharmacy services within a primary care setting! Qualifications
Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage the appropriate use of medications according to professional and regulatory standards. Job Summary
Responsible for the optimization of drug therapy by identification, resolution, and prevention of drug related problems and improving therapeutic outcomes. Also responsible for providing patient care according to the laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies. May be responsible for prescribing medications under specific collaborative drug therapy management agreements (CDTM).

Job Summary

The Pharmacy Intern may also be involved in the dissemination of drug information.
Essential Functions:
* Fills and delivers medications requested by nursing units and as needed for Omnicell automated dispensing machine after complete check by a registered pharmacist.
* Assists IV Pharmacist in sterile compounding. Assembles and manufacture IV Admixtures, small and large volume parenterals, irrigation solutions, skin tests, and other sterile dosage forms.
* Assembles and prepares chemotherapeutic agents if needed including the aseptic transfer of chemotherapy agents to large volume parenterals using aseptic technique to assure sterility and negative pressure technique to assure no drug contact.
* Prepare extemporaneous dosage forms not available from commercial sources.
* Assembles and prepares medications for unit dose prepackaging.
* Performs and records daily temperature checks of nursing unit refrigerators.
* Inventories and restocks supplies on shelves in immediate fill area on a daily basis.
* Performs monthly stock checks for expired medications.
* Performs good customer service skills via telephone or in person at Pharmacy window and while rounding on patient care areas.
* Adheres to HIPPA regulations regarding confidentiality and patient privacy.
