The Universal Nurse Home Visiting (UNHV) Nurse Home Visitor will provide home visiting services using the Family Connects model. The Family Connects model is an evidence-based and successfully demonstrated program that connects parents of newborns to the community resources they need through postpartum nurse home visits. As a community-based program, Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parents' needs, and helps to identify community services or resources that can meet those needs. In addition, through this connection to resources, Family Connects helps to identify gaps in critical community-wide resources with the goal of working toward increasing needed services locally. The Nurse Home Visitor will provide home visiting to all families with newborns in the determined catchment area. The goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The home visiting nurse will work in a team environment of clinical and nonclinical staff in order to schedule, visit, and support families in the community using the evidence-based Family Connects home visiting protocol. The Orthopaedic/Spine Nurse Navigator Team Leader facilitates the multidisciplinary clinical program through the skills of advanced leadership resulting in exceptional patient care and clinical outcomes. The Neurosurgical and Orthopedic Nurse Navigator Team Leader oversees program development by championing the Navigator mission to improve patient preparedness for treatment through education and psychosocial support while also facilitating interaction between patients, family members, their physician(s) and referrals as appropriate. The Neurosurgical and Orthopedic Nurse Navigator Team Leader assists in the development of pathways for use in the EPIC platform appropriate educational materials and program monitoring to promote coordination of care and patient satisfaction. The Neurosurgical and Orthopedic Nurse Navigator Team Leader serves as the lead Navigator and liaison with the clinical and leadership team as respects program development, expansion and successful achievement of programmatic goals. Is self-directed and innovative in clinical practice. Ensures that the resources, processes of delivering care, and demonstrating the foresight to anticipate problems and solutions result in favorable outcomes. Creates and addresses all scheduling and coverage needs for the Navigators that practice at Yale New Haven Hospital and cover Bridgeport and Milford Hospitals. 1,1 Assists physician leader and Director with ongoing management of the Neurosurgical and Orthopedic Program. 1.2 Assists the Optimization Program Manager with the development, ongoing management, quality and outcomes of the Neurosurgical and Orthopedic Program. 1.3 Serves as a clinical leader of the navigator team and role model for the Neurosurgical and Orthopedic Program. Supports the work of the multi-disciplinary provider team members as it relates to the accomplishment of positive patient outcomes. 1.4 Maintains excellent understanding of medical knowledge regarding Orthopaedic/Spine prodedures. Is certified as an Orthopedic Certified Nurse. 1.5 In collaboration with the medical team, coordinates the clinical care of patients. Utilizes excellent leadership skills to prioritize needs in an ever-changing patient care environment.. 1.6 Maintains knowledge of evidence based medicine. Acts as a clinical resource for navigators and clinical nursing staff related to transitions of care issues and educational needs . 1.7 Provides accurate, comprehensive assessment of relevant medical history and condition by using the optimization screening tool and follow up protocols. 1.8 Assesses patient and family understanding of patient condition, goals of care and ongoing management 1.9 Provides patients and families with education regarding their disease process, medications, treatment and plan of care. 1.10 Has direct responsibility for predetermined population of patients to optimize. 1,11As a member of the Orthopedic/spine team, assists with establishing protocols and guidelines 1,12 Performs additional duties as requested. * 2. 2. Provides support to patient caregivers through guidance, direction and support in both the inpatient and outpatient settings. Evaluates patients pre-admission health status to plan for discharge needs. Serves as a liaison for all EPIC-related pathways, consult orders and documentation for the Navigators 2.1 Serves as mentor for all navigators. Participates in orientation of new navigators. 2.2 Assists patients and families in understanding their treatment options, pre and post operative care and available resources. 2.3 Serves as an essential link between patients and all other care providers. 2,4 Continuously monitors and measures progress of interventions and outcomes. 2.5 Partners with patients and families in addressing their concerns, issues, desires and needs. 2.6 Facilitates the assessment, initiation and evaluation of patient and family education. 2.7 Enhances the patient/family quality of life through assessment, teaching, communication, emotional support, spiritual support and the practice of family centered care. 2,8 Facilitates appointments for consults and support services within established service standards, including social work, dieticians, physicians, etc. 2.9 Assists with planning and implementation of a safe discharge and transition of care. 2.10 Maintains responsibility for post discharge phone calls and follows up as appropriate. 2.11Conducts follow up calls related to patient satisfaction and survey completion. * 3. 3. Assists physician and administrative leaders with Orthopedic/Spine quality improvement process. Serves as the ad-hoc representative of the Navigators on the Safety and Quality Committee. Supports the efforts of the Navigator staff to create unique approaches to individualizing patient care. 3.1 Participates in department specific performance improvement activity relevant to program and role. 3.2 Assists in achieving objectives for the optimization program and supports an action plan to accomplish set goals. 3.3 Assists with educating staff regarding the Optimization programs quality improvement process. 3.4 Participates in continuous performance improvement activities. 3.5 Collaborates with multidisciplinary team to develop comprehensive plans to systematically document indications, appropriateness, complications and success of treatment. 3.6 Supports YNHH High Reliability organization initiative. * 4. 4 Collaborates with physicians, nurses and staff to assess educational needs of patients, families and community. Ensures that projects related to patient and family education materials are culturally sensitive and are offered at the appropriate literacy level for the diverse patient population. Demonstrates the ability to orchestrate group projects by promoting collaboration and preventing communication breakdown among staff. Creates and supports a caring environment in the clinical and administrative settings that fosters collegiality and trust - that ultimately results in patient and family confidence 4.1 Assesses needs and collaborates with physicians and clinical staff to develop and provide ongoing Orthopedic/spine education. 4.2 Shares results of research and PI studies with YNHHS and the Orthopedic community at large 4.3 Functions as a resource for providers and community agencies. 4.4 Functions as a clinical resource for residents and nursing staff maximizing opportunities in the clinical setting for informal education of staff, patients and families. * 5. 5. Leadership: Leads the Navigator team by example and demonstrates both clinical and behavioral excellent to all members of the multi-disciplinary team. 5.1 Demonstrates ongoing quest for knowledge 5.2 As requested, assists the Program Manager with data abstraction and statistics. 5.3 Documents all patient and family interactions in the electronic medical record according to established protocols. 5.4 Establishes a rapport with office staff and providers from various delivery networks * 6. 6. Maintains and prepares for compliance readiness related to ongoing Joint Commission certification of several sub specialties. Assists with data abstraction, aggregation of data and action plans as it relates to the patient and family experience 6.1 Maintains knowledge of Joint Commission certification requirements. Qualifications EDUCATION Bachelor's Degree in Nursing required; Master's degree in Nursing or Healthcare preferred. EXPERIENCE Five (5) years experience in acute care or healthcare management required. Experience in a combination of the following is essential: clinical care in neurosurgery or orthopaedics, outcomes management, physician relations. Knowledge and understanding of standards related to the Joint Commission certification process and the Procedure Pass and Care Companion on EPIC. Excellent oral and written communication skills. Serves as the lead liaison related to all marketing and community events. LICENSURE RN License State of Connecticut SPECIAL SKILLS Candidate must be able to thrive in a fast paced, complex academic medical environment where the Neurosurgical and Orthopedic Nurse Navigator Team Leader works as a valued member of the multidisciplinary team.
