The LicensedPracticalNurse - Medical/Surgical Unit - is responsible for providing safe, high quality care to general care patients as a team member in an assignment supervised by a Registered Nurse. Demonstrates commitment to patient/family engagement and effective communication. Practices consistently with applicable state nursingpractice regulations, applicable NLH policies and procedures. The LPN may administer intravenous medications upon successful completion of NH Board of Nursing approved training and with specific delegation by a Registered Nurse.
Responsibilities
* Provides and documents nursing care assessments, nursing interventions and patient/family education to stable patients under the supervision of a Registered Nurse in accordance with the patient's plan of care. Alerts Registered Nurse to any observed change in patient presentation or unexpected assessment findings
* Reviews assigned patient plan of care and pertinent active orders in the patient electronic medical record prior to providing care. Assists patients with activities of daily living, nutrition and hydration according to orders and goals of care
* Performs and documents focused nursing assessments of assigned patients, including but not limited to vital signs, assessment of pain, skin and wound assessment, functional status assessment, assessment of drains and ostomies, assessment of fluid balance, assessment of nutritional status
* Administers oral, topical and inhaled medications. May administer intravenous medications upon successful completion of NH Board of Nursing approved training and with specific delegation by a Registered Nurse.
* Applies dressings and assistive devices as indicated by assessment or as ordered by the interdisciplinary team, if applicable
* Reports relevant assessment findings and suggestions to enhance patient care directly to members of the interdisciplinary team in addition to the supervising Registered Nurse
* Complies with policies, procedures, and area-specific expectations related to maintaining a safe environment of care. Initiates reporting or action when needed to address safety concerns
* Complies with policies, procedures and area-specific expectations related to medication safety; patient identification; specimen collection and specimen labeling activities; documentation of patient information and patient care activities; as well as dependability, attendance and scheduling
* Participates in staff meetings, required learning, attestation, competency, and training activities according to area-specific and organizational expectations. Independently monitors email and other communications as necessary to meet these expectations
* Performs other duties as required or assigned.
Qualifications
* NH Board of Nursing Approved IV Therapy Certification required upon hire or within 6 months.
* Graduate of an accredited school of practicalnursing

Responsibilities
* 1. Orientation and Onboarding
* Coordinates and implements unit based orientation programs for nursing staff (RNs, LPNs, LNAs, patient care assistants, and others) new to DHMC or transferring to assigned unit
* Delivers training and ensures new staff competency regarding organizational systems, including, but not limited to, point of care testing, medication dispensing systems, electronic health record
* Assigns preceptors in collaboration with unit leadership

The coordinator conducts assessments of post-discharge and post-transition needs, presents suitable options to enrollees and their caregivers, develops individualized care plans, and ensures thorough documentation of all assessment updates.
This position is integral to reducing hospital readmissions, improving continuity of care, and providing essential support to enrollees and their families at critical points in their healthcare journey. This position requires a hybrid work model, including practice-based responsibilities, remote work, and facility or community visits as needed.
The population of focus will primarily include enrollees residing in Essex, Middlesex, Suffolk, Norfolk, Plymouth, Bristol, Dukes, and Nantucket counties.

Under the direction of the Transfer Center Medical Director, Nursing Director, and Nurse Manager, the Transfer Center Nurse (RN) is the single point of contact for facilities/providers requesting access to services or patient transfers to Concord Hospital. They are also responsible for patient placement and flow within the organization, balancing bed request with priority of need. Decides placement of all admissions and transfers by following established guidelines and criteria, and by collaborating with the Transfer Center Medical Director, Nursing Director, and Nurse Manager.
Education
Bachelor's Degree in Nursing or enrolled into a program within 6 months.
Certification, Registration & Licensure
Current and active New Hampshire Registered Nurselicense.
American Heart Association Healthcare Provider BLS required on hire
Experience
Minimum five years of Acute Care Nursing experience.
Previous supervisory/ leadership experience desired.
Demonstrate leadership ability and management skill including proficiency leading diverse teams in a highly complex, matrix management structure.
Strong organizational skills including but not limited to time management, ability to effectively multi-task, and prioritization.

Must be able to work a flexible schedule that may include evening, night, weekend, and holiday assignments.
Responsibilities
Anticipates and responds to all Transfer Center calls. Coordinates all admissions and transfers. Communicates with referring facilities regarding transfer and bed assignment. Develops and promotes long-term relationships with referring facilities and providers.
Monitors, evaluates and improves quality of care and services provided.
Monitors current institutional capacity to assess impact patient flow, resources, and capacity management.
Actively engages in the daily implementation of strategic goals as they relate to capacity management, patient flow and service-line development. Interprets and supports policies, procedures, and safety standards to facilitate daily operations.
Supports the overarching goals of the Health System.
Performs other duties as assigned.

The Surgical Specialties unit (6 North-South) is a fast-paced inpatient unit. We specialize in the post-operative care of multiple disciplines including, general surgery, urology, GYN, plastic surgery, vascular, ENT, orthopaedics, pelvic medicine, and post trauma care. Due to the complex nature of these injuries, the Surgical Specialties staff is trained to provide skilled nursing care of chest tubes, multiple drains, VACs, colostomies, ileostomies and complex wound care. As a member of 6 North-South, you will be part of a passionate, collaborative and highly skilled team of clinicians. Through our collaborative partnership with our surgeons and multidisciplinary team we provide excellent, individualized and holistic care to each of our patients.
Flexible schedule. Shifts are 12 hours in length, starting at 6am or 6pm. Every other weekend.

Summary

Under the direction of the Administrative Director, Cardiovascular Services, the Medical Director of Electrophysiology Lab and the Manager of Invasive Cardiovascular Services, the Electrophysiology Registered Nurse will perform duties necessary for caring for patients undergoing electrophysiology, cardiac rhythm management and select structural heart procedures at a technical and professional level. Under direct supervision of the physician in the electrophysiology lab, the RN will provide pre, intra and post procedural care. They will assist with a variety of technical procedures that require independent judgement. The RN will provide procedural care for patients, ages 18 years old and up, undergoing Electrophysiology, CRM and select Structural Heart procedures in the electrophysiology lab.
Education
Graduate from a state accredited school of Nursing. Bachelor's in nursing preferred
Certification, Registration & Licensure
Current AHA BLS for Healthcare Providers certification, or equivalent course credentialed by AHA
ACLS within 3 months of hire
RCIS or RCES preferred within 2 years of hire and renewed as required

Experience
