Former inmates come typically everyday to receive services such as GED preparation, substance abuse and mental health treatment, or assistance with employment. Responsibilities Provide effective clinical evaluation in completing the Human Services Needs Assessment Formulate appropriate Case Plans along with the consumer to document each goal/problem and how it will be addressed within one week of completing the HSNA Provide advocacy and referrals to community resources to assist consumers in advancing towards recovery and re-entry Provide didactic groups designed to teach skills and monitor consumer progress in recovery and re-entry Provide monitoring services to consumers towards progress/achievement of goals. Effectively collaborates with team members and other service providers in the delivery of clinical services.
