Davis Behavioral Health (DBH) is actively seeking a part-time Medical Records Retention Specialist to join the clinical support staff. This position ensures that all clinical records are maintained and stored in accordance with the DBH records retention policy.
The Medical Records Assistant is responsible for the following essential job duties:
Gather patient demographic and personal information.
Maintain quality and accurate records by following procedures.
Work within DBH's electronic medical record system and have a working knowledge of HIPAA and other
privacy laws.
Make sure all medical records are protected and kept confidential.
Ensure client charts, paperwork and reports are scanned and completed in an accurate and timely manner.
Working on caseload reports and any other reports that are necessary.
Complete clerical duties including responding to emails
Other duties and responsibilities as needed and assigned.
The clinical support staff at DBH provide the first impression for its clients and play an important role in helping clients who may be struggling with their mental health, which is why providing excellent customer service in a prompt manner is critical.
In addition to the essential job functions outline above, the following responsibilities are assigned to your specific position.
Scanning, importing, and organizing medical documents while adhering to HIPAA regulations.
Prepares medical records and loose documents for scanning.
Scans medical record documents.
Indexes medical record documentation.

Location
This position will work in the Davis Behavioral Health Main Street Clinic located at 934 S Main St Layton, UT 84041.
Shift
The successful candidate will be expected to work 20-25 hours per week during regular business hours. Exact hours will be determined by the hiring manager and successful candidate after hire.
Compensation
Starting pay for this position is $18.25 per hour.
Required Qualifications
Must be at least 18 years old.
Education/Licensure/Certification
High school diploma or equivalent required.
Experience
Previous experience working in a medical office and/or familiarity with medical terminology preferred.
Experience with business office equipment (credit card machines, fax machines, printers, etc.) preferred.
Skills & Abilities
Computer knowledge, including keyboarding (typing) skills, are required.
Must possess proficient working knowledge of Microsoft Office Suite and other computer programs.
Strong attention to detail with excellent organizational skills.
Must be a self-starter and have the ability to work independently with minimal supervision.
The Medical Records Assistant must possess strong verbal communication skills and have demonstrated professional customer service experience.
The successful candidate must also demonstrate compassion, courtesy and respect to all clients and their families.
Exceptional interpersonal skills with the ability to establish immediate rapport with clients.
Spanish speaking is a plus.

Essential Job Functions
The Adult Mental Health Outpatient Case Manager must be able to complete the following essential job functions:
Complete DLAs (or other agency identified case management needs assessments) at the required frequency in order to create individualized goals and objectives.
Use information from the DLA to complete individualized service plans in support of overall mental health treatment goals.
Maintain agency standards for productivity and documentation.
Engage with clients' treatment teams to support clients and coordinate treatment objectives.
Maintain an active caseload.
Coordinate and advocate for care that ensures basic needs are met. This may require coordination and advocacy with multiple local and State agencies.
Work with clients and their families or other support systems to ensure basic needs are met.
Be knowledgeable about the crisis system and know when to activate it.
Able to balance independent work while appropriately consulting with supervisor and taking directions when given.

Strong preference will be given to candidates who graduated from a college or university with a bachelor's degree in social work (BSW) AND active licensure as a Social Service Worker (SSW) in good standing in the State of Utah.
Employment Classification & Schedule
This position is classified as full-time, non-exempt.
The proposed schedule for this position is Monday through Friday from 9:00 am to 5:00 pm.
Location
The DBH Main Street Clinic is located at 934 South Main Street, Layton, UT 84041.
While this position will have office space within the agency, case management activities are expected to be performed across multiple settings including client homes and community locations. Significant fieldwork is required, including frequent travel throughout the community to conduct outreach, attend meetings, and provide direct client services. The position may require the use of a personal vehicle; mileage reimbursement is provided in accordance with organizational policies.
Compensation
The hourly rate for this position is outlined below:
BSW Degree + SSW License
$26.00 per hour base wage + 1% per year of licensure
Degree without SSW License
$22.00 per hour base wage + 1% per year of direct social work experience, up to 10 years.
Introductory Period
All new employees at DBH have a 6-month introductory period to assess whether they can perform the essential functions of the job at the expected level, within the specified timeframes, and are a good fit for the job.
Required Qualifications
Education
Bachelor's degree in social work or a related field is required.
Licensure/Certification
Strong preference for active licensure as a Social Service Worker (SSW) in good standing in the State of Utah. BHCM certification is not required for licensed individuals, OR
The ability to obtain Behavioral Health Case Management (BHCM) certification within 90 days of employment is required, if unlicensed.

DBH utilizes an electronic medical health records system, so computer literacy and familiarity with Microsoft Office products is required.
Meeting monthly face-to-face service expectations and productivity standards is also required.

Clinical Oversight:
Oversee and manage all aspects of the team's interventions
Provide clinical supervision of counselors/therapists who report to this program.

The IPS Job Coach/Case Manager is a full-time position with most hours Monday through Friday. DBH reserves the right to require occasional overtime, weekend work, and/or assignments to evening or night shifts.
Work hours must be during the regular business hours where the employee is available to meet with community employers and members of the treatment team.

These activities include being an active member of the Cleaning Services team to clean DBH facilities, ordering and stocking required cleaning supplies, coordinating responses to urgent cleanup requests, and acting as the liaison between DBH and external services providers that assist with cleaning needs. The Cleaning Services Supervisor is directly responsible for personnel within the Cleaning Services program, including supervision, training, recruitment, and scheduling for custodians.
The Cleaning Services Supervisor must be able to complete the following essential job functions:
Perform all supervisory functions for the DBH Cleaning Services program, including developing a schedule to meet DBH's cleaning needs, completing all aspects of the recruitment process to fully staff the program, participating in personnel actions, ensuring that all required training is completed by all members of the cleaning team, and providing regular training to team members.
Manage inventory of cleaning supplies through regular ordering and stocking each facility, as needed.
Maintain cleaning equipment. Perform regular inspections to ensure that equipment is operating as expected. Conduct minor repairs and/or coordinate regular maintenance of equipment.
Develop and implement cleaning checklists for each facility to include tasks that must be completed daily, weekly, monthly, and quarterly.
Perform all cleaning tasks on cleaning checklists at assigned facility.
Develop and maintain relationships with external cleaning service providers who assist with facility cleaning assignments.
Coordinate responses to urgent cleaning needs outside of regular cleaning schedule and task checklist.

Clinical Oversight:
Oversee and manage all aspects of the team's interventions
Provide clinical supervision of counselors/therapists who report to this program.
Provide leadership and act in accordance with professional, ethical guidelines and code of conduct.
Regularly evaluate training needs and facilitate clinical training.
Research current best practices for providing treatment to individuals with psychotic disorders and lead efforts to train the team on the most up to date techniques
Manage referrals into the program, including processing referrals, scheduling and conducting screening meetings with potential clients, determining appropriateness for the program, and scheduling intakes
Ensure all staff have adequate supervision and staff needs are immediately addressed
Proactively involve and communicate with collateral supports, including family, significant others, probation officers, judges, caseworkers, etc.
Fulfill on-call responsibilities and ensure the 24/7 on-call coverage is responsive to the needs of program participants
Clinical Care:
Appropriately diagnose mental health and substance-related conditions using the DSM-5
Assess, diagnose, use & interpret relevant clinical tools (RANT, LS/CSI, SASSI, ASAM, Etc.) to inform best practice decisions relating to client admission, treatment plan development and revision, termination of client services, and reducing the level of care.
Conduct individual, family, and group psychotherapy as assigned.
Coordinate client psychiatric evaluations and other wrap-around services to provide effective care.
Support clients to perform independent activities of daily living & basic life skills.
Support clients with their efforts to maintain legal adherence, medication adherence, and other supportive services to maintain better-quality mental health function.
Utilize motivational techniques to retain and support client progress.
Conduct groups, crisis intervention, and individual and family therapy for participants.
Conduct feedback meetings where appropriate client feedback is presented, providing clinical supervision for attending counselors.
Provide crisis management services and support a culture of safety for clients and staff, including impromptu support for both clients and staff.
Develop an adequate level of rapport with clients on caseload while maintaining healthy boundaries.
Complete service documentation within the appropriate client management system within the same day of service.
Complete thorough and effective assessments in required timeframes, including funding authorization requests
Management:
Supervise all program staff, both directly and indirectly through supervision of managerial staff
Support the Program Director to oversee the program's compliance with legal guidelines, policies, and quality standards.
With the support of Program Director, maintain responsibility for hiring, orientation, scheduling, development, evaluation, and termination of staff.
Manage all routine operational activities and identify areas for program improvement or program development.
Proactively resolve program issues and conflicts, including issues related to staff performance, client relations, medication issues, staff coverage, and the team's relationship with community providers
With support from the Program Director, coordinate and respond to program evaluations and audits conducted by outside agencies
Support the Program Director in maintaining adherence to the program budget.
Oversee Quality Assurance within the program, including regular and formal evaluation of service, documentation, paperwork, and outcomes achieved through fidelity to the program model.
Document all incidents immediately and report as required.
Coordinate with the Program Director and Program Manager to ensure that the program is managed effectively and that licensing requirements are being met.
Create and maintain the staff on-call schedule
Assist with community networking, fundraising, relationship building, and speaking engagements.
Attend relevant partner meetings as needed.
Maintain active and appropriate licenses for the role and duties assigned.

Job Responsibilities:
Become familiar with each child's treatment plan goals and oversee their daily progress
Learn processes, methodology, technologies and consistently follow them
Communicate with Lead Teachers and Clinical Staff as necessary
Produce accurate progress notes on children's daily development
Follow the classroom schedule during the shift
Follow client safety and facility procedures
Learn and adopt client de-escalation techniques and help children express their emotions in an age-appropriate manner.
Document daily feedback logs for parents and communicate any pertinent information to parents and the clinical team regarding the child's needs and progress

Cleaning Services Supervisor

The Cleaning Services Supervisor is responsible for daily oversight and administration of facility cleaning throughout DBH and developing and implementing a regular schedule of activities to maintain the expected level of cleanliness throughout the company. These activities include being an active member of the Cleaning Services team to clean DBH facilities, ordering and stocking required cleaning supplies, coordinating responses to urgent cleanup requests, and acting as the liaison between DBH and external services providers that assist with cleaning needs. The Cleaning Services Supervisor is directly responsible for personnel within the Cleaning Services program, including supervision, training, recruitment, and scheduling for custodians.
The Cleaning Services Supervisor must be able to complete the following essential job functions:
Perform all supervisory functions for the DBH Cleaning Services program, including developing a schedule to meet DBH's cleaning needs, completing all aspects of the recruitment process to fully staff the program, participating in personnel actions, ensuring that all required training is completed by all members of the cleaning team, and providing regular training to team members.
Manage inventory of cleaning supplies through regular ordering and stocking each facility, as needed.
Maintain cleaning equipment. Perform regular inspections to ensure that equipment is operating as expected. Conduct minor repairs and/or coordinate regular maintenance of equipment.
Develop and implement cleaning checklists for each facility to include tasks that must be completed daily, weekly, monthly, and quarterly.
Perform all cleaning tasks on cleaning checklists at assigned facility.
Develop and maintain relationships with external cleaning service providers who assist with facility cleaning assignments.
Coordinate responses to urgent cleaning needs outside of regular cleaning schedule and task checklist.

Schedule

The starting pay for the Cleaning Services Supervisor is $20.00 to $22.00 per hour based on direct experience.

Demonstrated experience in the cleaning services industry.

5+ years of commercial cleaning experience.
