The Clinical Educator at Transitions Care is responsible for the overall development of the hospice staff as well as assisting in maintaining the hospice education program. The Clinical Educator coordinates clinical education for the hospice staff in alignment with the organization's standards of care, competency requirements, regulatory standards and promotes an environment of quality of care for patients.
Major Responsibilities
Develops and provides orientation and training programs for newly hired staff to educate them on the requirements of their position as well as company expectations.
Develops, coordinates and presents clinical educational programs, including basic care, patient services, new equipment and product training, as well as required training or certification programs.
Works with the Regional Director of Nursing and leadership to assess the training needs of clinicians; assists in providing ongoing training and staff development activities to address identified performance improvement opportunities.
In collaboration with the Director of Education, updates and coordinates annual compliance training.
Serves as a resource to departmental directors in the development of annual education plans for their staff.
Assists the Medical Director, Physicians and Regional Director of Nursing in providing continuing education units (CEUs), in-service trainings and continuing education opportunities to employees to update their skills.
Supports nursing leadership in identifying policies, standards and practices not in compliance with current standards and implementing changes, as needed.
Develops, coordinates and presents in-service trainings in SNFs, RCFEs and medical offices, when able.
Develops patient and family education materials as needed.
Participate in staff meetings, department meetings, team meetings, briefing, in-services, and other related activities as needed
Assure for compliance with local, state, and federal laws, Medicare regulations and established company policies and procedures.

Requirements
Education and Experience
RN with current licensure to practice professional nursing in the State you are employed
Minimum of three years clinical experience including one year in a managerial role in a healthcare environment preferred.
Proficient PC skills-Microsoft Word, PowerPoint & Excel
Excellent organization skills required
Proficient in principles of adult learning and providing education/training to clinicians.
Bachelor's degree in nursing.
Comprehensive knowledge of business management, state/federal regulations and all organizations accrediting bodies.
Hospice and Palliative Care Nursing Certification (CHPN) preferred.
Has excellent observation, verbal and written communication skills, public-speaking and presentation skills.
