About the Role
We're looking for a builder - someone who wants to take a messy zero-to-one commercial motion and turn it into something that works, fast. As our first Head of Provider Growth, you'll own provider adoption end-to-end: testing channels, refining targeting, knocking on clinic doors with our first reps, and scaling the system once we know it works.
This role is perfect for someone who has built growth engines before (in startups, agencies, or their own companies), loves rapid experimentation, and wants to have massive impact in a short amount of time. What You'll Do - Impact in Your First 3-6 Months
Own the growth target. Build and execute a plan to reach meaningful PA volume across provider settings.
Run fast experiments. Test messages, channels, outreach methods, and install flows to identify what moves the needle fastest.
Get your hands dirty. Shadow field reps, visit clinics, talk to staff, learn the real workflows, and refine our pitch based on firsthand insight.
Target smarter. Partner with our Head of Strat/FinOps to map specialties, geos, and practice types with the highest PA density and frictionless install paths.
Create momentum. Build a simple but effective engine: outbound sequences, enrichment, list building, mini‑landing pages, CRM flows, funnel dashboards.
Produce early proof points. Deliver data that shows what works, what doesn't, and where we should double down.

You've built zero-to-one growth engines before - maybe at a startup, maybe at your own company, maybe as the first GTM hire somewhere.

Summary: Responsible for administrating and assisting the MSSP Team in the care management process under the supervision of the Care Manager to frail seniors to avoid premature placement of persons in nursing facilities, while fostering independent living in the community.
Essential Functions:
⦁ Provides case management services to seniors. Specific responsibilities include:
i) Conducting intake/Screen on MSSP applicants.
ii) Completing the intake and application process.
iii) Providing information and assistance to the participant as needed.
iv) Purchasing and delivering items requested by the primary care manager.
v) Completing monthly care management contacts.
vi) Completing quarterly home visits.
vii) Documenting activities in the participant record.
⦁ Maintains complete client files and completes all required data collection forms and reports.
⦁ Reports incidents affecting case management clients to appropriate bodies including Adult Protective Services.
⦁ Conducts outreach and community education to seniors, their family members, and service providers regarding the issue of elder abuse or other related social issues in the community.
⦁ Assumes responsibility for own professional growth and continuing education to upgrade case management skills; gives input to the supervisor in developing the training plan to meet individual training needs.
⦁ Participates in case conferences, in-service trainings and other meetings as required.
⦁ Participates in evaluation and quality assurance activities to determine the effectiveness of the Case Management Program as required.
⦁ Performs other duties as assigned.
Qualifications:
⦁ Two years of experience working with frail older adults or a Bachelor's degree in a human services discipline.
⦁ Knowledge, skills, and experience necessary to assess the consumer's need for services and perform the core functions of case management. Training in and/or experience with elder abuse issues is strongly preferred.
⦁ Ability to communicate effectively with consumers or their representatives and staff of other service agencies, and to work as part of a multidisciplinary team of service providers on behalf of consumers.
⦁ Sensitive to the needs of the elderly and disabled adults from diverse ethnic and minority cultures.
⦁ Ability to work independently and in a team environment.
⦁ Bilingual and bi-literate preferred (English, Chinese, Spanish, Vietnamese, Tagalog, etc.).
⦁ Must be able to travel via public transportation or personal vehicle for the performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
