What You Will Do - Essential Functions
As a Provider at Emcara Health, you'll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care.

Required Qualifications:

* 1+ year of experience in primary care or a related field such as family practice, general practice, or outpatient care.

Position Summary: The RCA nurtures & supports the Hawthorne Residents by providing excellent care in a family environment. The RCA recognizes & responds to Residents needs appropriately. Assists Residents with completing activities of daily living & personal grooming & comfort, while encouraging Resident independence & respecting Residents wishes for care. Responsibilities include:
Support emotional, physical and social well-being of elders.
Encourage independence of elders with decision making.
Look for daily opportunities to establish meaningful relationships with the elders by valuing individuality.
Assist in developing and carrying out lifestyle programs and activities that meet the needs of the elders.
Assist in maintaining clean common spaces and laundry rooms.
Attend monthly staff meetings as well as quarterly organization wide meetings. Assist with program selected routine procedures such as personal care, dressing, grooming, transfer, assist with ambulation, toileting to assist elder to maintain good personal hygiene in accordance with the Individual Service Plans (ISP's).
As necessary, assist elders with positioning and transporting in and out of chairs, beds, bathtubs, cars, buses, etc…. Will also include transporting to other areas of the community.
Report any physical or mental status changes or concerns to LPN, RN/Case Manager or Program Coordinator.
Communicate with co-workers at all levels to meet the needs of the elders.
Maintain a safe, hazard free environment by observing and monitoring the community and reporting unsafe conditions to the RN/Case Manager or Program Coordinator.
Follows procedures for documentation.
Ability to work with individual residents or groups in special activity programs.
Ability to respond to common inquiries from residents, resident's families, regulatory agencies, and the public.
Must be able to provide redirection to confused or distressed residents.
Ability to maintain a high level of confidentiality with staff and peers.
Ability to remain calm, reassuring and effective in an atmosphere that often times is chaotic.
Follow established protocol for accidents and incidents.
Attend and participate in 12 hours annually of training programs and other educational classes.
Complete additional tasks as requested by Program Coordinator, RN/Case Manager or LPN.
Responsible for reporting to RN/Case Manager any observations or changes in residents' behavior, activity or physical appearance.
Responsible for following infection control guidelines.
Attend and participate in disaster preparedness training and drills.
Assist residents during meals with direction, cueing and assistive devices.
Monitor elders during meals.
Provideassistance to elders with medical equipment.
Qualifications:
High School diploma or equivalent required.
Certification as a NYS Home Health Aide (HHA) required.
Medication assistance training desirable.
Med Tech experience desirable.
Willingness to complete onsite RCA training.
Minimum of 2 years of past experience providing direct resident care required in home health or long-term care.
Proficient in writing and reading English.
Physical Requirements:
Standing, walking, sitting, lifting and the ability to assist elders with transferring and all ADL's.
