Provides clinical site director and team leaders with information regarding standards that are not met. In addition to routine independent auditing of charts, team leader will provide oversight for the concurrent review process to ensure that timelines are followed according to agency policy. Provides appropriate recommendations for follow up and linkages to such as needed to consumers who have received services and require follow up to ensure they maintain sobriety/abstinence and are able to access appropriate community services. Other duties as assigned by the supervisor Consults with other local providers and referral sources by telephone as needed to insure coordination and transition of services when necessary. Units Billed Minimum Requirement: There are 25 units of productivity standard to meet. The clinician is expected to collect the necessary information needed for reimbursement for the assessments that are done by the clinician. The clinician is also expected to turn in appropriate paper work so that assessments, intakes, evaluations, screenings, assertive outreach and case support may be billed when appropriate. Special Attributes: Strong evaluative and clinical assessment skills needed. Must be able to get along well with other professionals to build collaboration in the work environment of multiple providers, community agencies, and a variety of community stakeholders. Pre-Crisis Support Assist the consumer with using the crisis plan and provide relapse prevention support. Housing Supports the consumer to maintain stable housing, improve housing situation, and enhance independent living skills. Education/Employment Assists the client in gaining information about going back to school or job training Developing and implementing a Mental Health Wellness Recovery Action Plan (WRAP) If with an approved WRAP facilitator or trainer, this can include skill maintenance and enhancement around meals and social activities to build peer relationships where eating is not the core activity. Units Billed Minimum Requirement: The direct service guideline is 75% of time spent in specific billable contacts per week in the community. (120 units a week) Qualification Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Peer Support - Requires Peer Support Certification. AP -requires a bachelor's degree in social work, Counseling or another Human Services field with no experience. Tech 2: QP- requires an associate or bachelor's degree in social work, Counseling or another Human Services field. Qualified Professional (QP) required.
