PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations. · Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses. · Performs ongoing, systematic collection and analysis of patient data pre - during - post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed. · Assesses, collaborates, and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family. · Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients. · Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness. · Initiates or assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections. · Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. · Ensures patient awareness related to transplant and treatment modality options. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. The Executive Director (Registered Nurse, RN) in Hospice supervises all aspects of patient care, all activities of professional staff and allied health personnel, regulatory requirements compliance, financial performance, and quality assurance performance improvement activities for agency. Responsible for compliance with all regulations, laws, policies, and procedures applicable to hospice and Medicare/ Medicaid issues when applicable daily. Directs the daily operations of the agency and acts as the Care Management Process driver. The Executive Director or designee RN, such as a Patient Care Manager, will receive daily reports on patients from staff. This process also includes review of patient care paperwork, referral information, recertification processes, discharge information, and event reporting, etc. daily. Reviews monthly financial results relative to all aspects of the operation to assure that quality patient care is delivered in the most cost-effective manner. Participates in the QAPI planning and processes, reporting, and improvement action plans as indicated. This includes chart audits, patient satisfaction, financials, contracts, patient/family complaints, etc. and appropriate follow-up. License Requirements Must be a licensed physician; licensed registered nurse; licensed social worker; or a Bachelor's degree college graduate with at least three (3) years of documented success in discipline/field of study and a minimum of one (1) year of full time experience in a hospice, home health, or other health care delivery system setting. Three (3) years of health care leadership preferred. Current CPR certification required for Executive Directors who may provide services to patients in the field. ABOUT THE ROLE As a Oncology Nurse Navigator at Daymark Health, you will be a key member of an integrated Care Team, which includes a Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will work closely with the Clinical Operations team to ensure that optimal care pathways are identified and followed. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities. WHAT YOU'LL DO In this role, you will provide direct patient care in the home and through virtual platforms. Additionally, you will conduct health assessments, educate patients, and deliver high-quality care to meet each patient's unique needs. Within your first three months, you will: Master Daymark systems and tools. Become familiar with Daymark's systems, tools, technology, and partners. Collaborate closely with Clinical Leadership. Work closely with Nurse Leaders and Medical Directors to ensure alignment with clinical protocols and best practices. Build Trusting relationships. Establish strong, trusting relationships with patients and their care networks by prioritizing empathy and active listening in every interaction. Adhere to Operational Standards. You're following Care Team policies, procedures, and documentation standards, contributing to operational efficiency and maintaining high-quality care with comfort. Support Patients Across the Care Continuum. You'll provide comprehensive support throughout the oncology care journey, from screening to survivorship or end-of-life care, offering clinical and care coordination as needed. WHAT WE ARE LOOKING FOR: Experience: A Bachelor of Science Degree in Nursing is required, along with a compact unrestricted registered nurse (RN) license and licensure in Rhode Island (or are willing to be licensed in RI) Additionally, you have at least 3 years of nursing experience, including 3 years in oncology nursing, and have spent a significant amount of time delivering in-home care. You are passionate about helping patients understand the benefits of palliative care. Availability: You can work with our team and patients full-time from our office in Providence, Rhode Island, five days per week. Qualifications: Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) are strongly preferred or will be obtained within 2 years of hire. Strong Clinical Skills. You're comfortable administering IV fluids, accessing ports when needed, and conducting point of care testing. Comfort with Technology. You're comfortable using Slack, Zoom, Microsoft Office, and EHR systems. Strong relational skills.
