The CPN is a vital member of the team who partners with the Deaconess Care Group and Deaconess Health System leadership to engage and educate the physician team, optimize performance on defined key metrics, and ensure collaboration with the care team. They will also be involved in various hospital committees and will present information about the team's quality data to hospital leadership as requested. While this position does not involve providing direct patient care, they are a part of participating in daily workflows, advocating for patient goals in the care plan, identifying cohorts of patients eligible for specific initiatives, and ensuring communication throughout the care team as the patient moves through their hospital stay. Success in the role will be defined by measurably improved patient outcomes and experience of care, as well as improved utilization of resources. As a CPN, you will work in a fast-paced environment while maintaining great attention to detail and accuracy in all you do. Education and Experience * Graduate from a nationally accredited school of professional nursing. If graduate of non-nationally accredited school of professional nursing, then at least one year of professional nursing experience is required. * BSN is preferred, or an education action plan in place. Newly hired RNs on or after October 1, 2014 with five (5) years or less nursing experience are required to receive their BSN within five (5) years from date of hire. Initiates or assists with emergency response measures. Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections. Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. Ensures patient awareness related to transplant and treatment modality options. Assists in the identification, evaluation, selection and education of Home Dialysis candidates and Home Partners. Performs assessment and identifies barriers of the Home Dialysis candidate's home environment and partner/ family readiness and ability to perform dialysis treatments in the home. Trains home dialysis patients and / or Home Partners on the safe, effective operation and maintenance of all Home Dialysis equipment and treatment supplies through a formal standardized Home Dialysis Training Program. Participates in education and quality improvement projects at the facility and area level as directed by Supervisor. May serve as a Preceptor to new employees. Required to complete CAP requirements to either maintain or advance. Performs all other duties as assigned by Supervisor.
