The Human Trafficking Therapist (HTT) provides individual, family and group therapy and case management to human trafficking clients within agency policies and procedures and practices. Responsibilities include providing direct and family therapy and a variety of casework duties. The HTT works with Human Trafficking Specialist to assess for social service needs of clients and makes appropriate referrals. The HTT reports to the Human Trafficking Program Manager. Primary Responsibilities:
Provides direct individual and family therapy to a minimum caseload of12- 15clients including comprehensive evaluation, treatment planning, effective treatment implementation, and discharge planning within the guidelinesestablishedby the Family Hope Center.
Facilitates a minimum of two to four therapy or psychoeducational groups (child, parent, combined child and parent, or family) per year.
Performs a variety of casework duties, including obtaining,analyzingand evaluating data necessary to implement client services.
Actively participates in reflective, clinical and administrative supervision with the clinical supervisor at a frequency to be determined by the therapist andsupervisor together.
Activelyparticipatesin Family Hope Center department meetings and case consultations.
Activelyparticipatesin peer consultation process.
Participates in Family Hope Center file reviews.
Provides linkage to other appropriate resources, including intensive case management services, based upon the needs of clients and families.
Maintainsaccurate,completeandtimelyclient records as required by the Family Hope Center and ChicagoCAC.
Conducts crisis assessments and responds appropriately to client crisis needs.
Participates in Family Hope Center community events and actively fosters supportive community environment within Family Hope Center for clients and families.
Promotes trauma resiliency among the Family Hope Center team,ChicagoCACstaff, and MDT staff byparticipatingin and contributing to projects to foster the trauma informed environment, e.g., Breathing Room.
Participates in MDT case review, case consultation, or staff meetings to provide clinical support related to Human Trafficking Cases.
Participates in Human Trafficking Collaborations and Task Force Meetings.
Facilitates trainingswith Human Trafficking Specialist.
Performs other duties as assigned.
Discretion
The Human Trafficking Therapist has a mid-level range of discretion and independent decision-making.
Supervisory Responsibilities
The Human Trafficking Therapist has no direct supervisory responsibilities.
Relationships
The Human Trafficking Therapist works closely with the Human Trafficking Program Manager, Human Trafficking Specialist, Director of Clinical Services, Family Hope Center team, Advocacy Team, and Family Support Specialists.
Qualifications
Minimum requirement of a Master's Degree in Social Work, Counseling, Psychology, or another human services related field. Must be licensed LCPC, LCSW, or PsyD. Minimum of three years of experience providing trauma-focused evidence-based therapy to children and adolescents. Strong problem-solving, organizational, written, oral, and communication skills needed. Demonstrated awareness of multiple cultures and cultural impact on the delivery of service a must. Flexibility in schedule based on the needs of clients is required. Travel to client's homes or schools for delivery of service is required. Evenings required.

General Description

Mental Health Therapist Responsibilities include, but not limited to:
Evaluates behavioral and emotional clients' issues.
Provide directs client supervision to those clients with chronic or acute mental disorders who require active treatment.
Interacts meaningfully with clients.
Provides individual, family, and group counseling under the direction of a clinical supervisor as required and in accordance with client's individualized treatment plan.
Conducts psycho-educational assessments under the direction of a clinical supervisor.
Makes observations and documents client treatment interventions, behavior, and progress.
Provides direct client supervision and interacts therapeutically.
Interacts with educational team as needed to ensure appropriate educational placement.

Develops and updates clients' individualized treatment plans.
Minimum Requirements:
The following educational requirements are acceptable for this classification including:
Master's Degree from an accredited university in a clinical mental health discipline, such as: Psychology, Counseling, ClinicalSocial Work, Psychiatric Nursing, and Marriage and Family therapy. Master's Degree must include clinical coursework in the foundations of behavioral health and the provisions of mental health services; field work practicum is preferred. At least one year verified post-master's experience providing mental health treatment.
Participation and completion of mandatory in-services.
Participation in at least ten hours of documented additional in-service per year.
At least twenty-one (21) years of age
Valid Driver's License
Physical exam that includes TB and drug testing.
Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors.
Satisfactory completion of background screening and applicable pre-employment checks, including but not limited to employment and/or personal reference and driving records.
Ability to work with computers and the necessary software typically used by the department.

Summary: A Therapist I provides therapy services to a variety of agency clients. Works cooperatively with other community agencies and resources.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Promotes good public relations within the community; sustains good working relationships with interrelated agencies to facilitate referrals and cooperation on individual cases.
Attends appropriate continuing education workshops and seminars to update techniques and maintains necessary levels of program specific knowledge and enhance counseling skills.
Attends and participates in weekly clinical supervision with their licensure supervisor.
Ensures that all documentation is reviewed and approved by their clinical supervisor.
Implements clinical feedback provided in relation to work with clients.

Responsibilities
MAJOR DUTIES
1. Patient Care and Documentation
Functions as a full-time integrated team member in the primary care clinic using the Primary Care Behavioral Health (PCBH) model of service delivery. Screens and assesses patients, and performs PCBH model appropriate biopsychosocial evaluations using expert knowledge of general and specific concepts, principles, procedures, and practices relating to clinical, clinical health, and primary care psychology. Provides targeted assessment and evaluation of functional status focused in the context of the presenting problem. Conducts brief assessments and formulates evidence-based interventions appropriate to a primary care setting for a wide range of behavioral health conditions, chronic medical conditions, and health behaviors affecting medical conditions. Provides evaluation, intervention and relapse prevention services in appointments lasting no more than 30 minutes for initial and follow-up visits. Uses a stepped-care approach to maximize delivery of care within the PCMH. Triage and refers patients to Specialty Behavioral Health Care when services required exceed the BHC scope of practice. Assists with implementation and monitoring of treatment plans with Primary Care Managers (PCMs) and other primary care team members. Use objective and measurable outcomes to evaluate treatment effectiveness and make adjustments if appropriate for increased adherence and efficacy. Provide high level expertise in human behavior as a primary care team member to develop and support specific clinical pathways or best practice programs for targeted patient groups (e.g., patients with acute stress symptoms, chronic medical conditions, insomnia). Provides concise documentation of care and recommendations in patient's medical record beginning in the appointment and completed in no more than 72 hours after the patient is seen.
2. Consultation and Team Based Care
Provides timely and succinct feedback to PCMs regarding consultation findings and recommendations as directed by the current DHA PCBH Administrative Instruction and DHA Procedures Manual. Determines if patient is appropriate to receive PCBH model services in the primary care setting. Provides ongoing consultation services for a subset of patients who would benefit from periodic, regular behavioral health monitoring and intervention for sustaining improvement and preventing relapse (i.e., continuity consultation). Participates in all PCMH activities to include huddles, provider meetings, peer review, and trainings. Actively works with PCMH team to identify patients who are appropriate for BHC services and facilitates processes to increase same-day “warm handoff” of patients from PCMs to BHC. Works to increase overall penetration into the enrolled clinic population by targeting common conditions with behavioral components that are amenable to BHC intervention. Performs daily activities to increase BHC clinical productivity (e.g. scrubbing PCM appointment lists, circulating in the clinic and interacting with staff, being vocal in morning huddles etc.)
3. Educational Activities
Develops, teaches, and provides oversight for classes that promote education and skill-building to enhance psychological and physical health, and promote behavioral change related to improved health status. Conducts trainings for the primary care clinic to increase referrals and promote understanding of the role of the BHC.

Responsibilities
MAJOR DUTIES
Provide expertise in Behavioral Health, Psychosocial and Crisis Intervention for Individuals seeking treatment and care in an emergency room setting.
Develop individualized treatment plans for each case based on psychiatric, cognitive, psychosocial, environmental, and medical needs.
Utilize social work systems, theories, and principles that impact patient care.
Work with highly complex cases in which individuals are severely impacted by moderate to severe psychiatric diagnoses to include a history of trauma, physical abuse, neglect, exploitation, co-occurring conditions, and suicide.
Select, administer, and interpret instruments for the purpose of assisting in the diagnosis and assessment of treatment plans with behavioral/psychiatric disorders.
Collect bio-psycho-social information about patients using interviewing, case history, observation, and various assessment methods.
This position has a requirement to lift up to 35 lbs. in the regular performance of duties.
PERFORMS OTHER DUTIES AS ASSIGNED.

Basic Requirement for SocialWorker (Behavioral Health Consultant):
Degree: Master's degree (or higher degree) in social work from a school accredited by the Council on Social Work Education.
Licensure: Current, active, valid, and unrestricted clinical license to practice Social Work independently offered by a U.S. State, District of Columbia, Commonwealth, territory, or jurisdiction.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Specialized Experience: One year of specialized experience which includes conducting behavioral health assessments; providing individual and group treatment; and working as part of a multidisciplinary team. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-12).
Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted.
