SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY * Clinician provides services for consumers with severe and persistent behavioral health disorders. * Facilitates skill-building and evidence based groups. * Conducts face-to-face comprehensive assessments to identify problems and service needs. * Writes individual service plans that meet Medicaid, insurer, CARF, and other required standards for diagnoses, content, justifications for treatment, evidence of consumer input, and treatment time frames. Writes service plans that are clear, concise, complete, grammatically correct, timely and properly coded. * Secures or provides necessary signatures required to secure authorization for services. * Follows all Medicaid, state, insurer, and other regulations related to the credentials required to provide authorized services. * Arranges appointment schedule at optimal intervals to maximize use of work time. * Provides service linking and monitoring. * Establishes linkages and coordination of services with community agencies. * Informs consumers and families of the availability of community advocacy and support groups. * Other critical services provided include symptom management, behavior management planning, teaching of relevant skills, supportive community-based interventions, and crisis intervention services. CLIENT'S AVAILABLE HOURS *29 hours per week. *A set 3 days each week. *Contractor only exceed 29 hours if they pick up a shift from a call off. *Required 15 min unpaid break. b>CLIENT'S REQUIRED SKILLS & EXPERIENCE *Master's Degree in social work, psychology, or related field. Current license as a LCSW, LPC, OR LMFT in the State of Georgia, or master's with LAPC, LMSW, or LAMFT and ability to obtain LPC/LCSW/LMFT licensure within 3 months of hire in the State of Georgia. *Minimum of two years' experience in clinical social work. Experience working with moderate to severe and persistently mentally ill populations.
