Job Summary: The Lead HR Generalist works closely with the Hospital and District leadership team to coordinate activities related to the Hospitals personnel. The HR Manager provides oversight and consultation for one or multiple hospital facilities within in a market including, employee relations, policies and procedures, employment law, retention, compensation, HRIS/management reporting, new hire procedures, and benefits. Consults with and updates Senior/District Director of Human Resources regarding all Human Resources functions and activities. In collaboration with area management and under the direction of the District Director of Human Resources, communicates, facilitates/coordinates implementation and monitors new and/or enhanced HR programs, policies and procedures. Promotes proactive, positive employee relations practices and environments in all areas and understands and appropriately balances the roles of both employee and company advocate. Serves as key member of Kindred Human Resources team, providing valuable input and feedback and assuring implementation and maintenance of division and corporate HR initiatives. Ensures compliance with federal, state and local employment laws, as well as applicable regulations and standards.
Essential Functions
Promotes and fosters ongoing collaborative communication with healthcare professionals and VNSNY to establish coordinated VNSNY Hospice services. Functions as a liaison between external customers, patients, families and VNSNY Hospice. Builds relationships, obtains referrals and reviews patient information referred to VNSNY Hospice to determine initial appropriateness of admission. Facilitates the safe and timely transfer of patients from a hospital, skilled nursing facility, physician office, or other Home Care site to the care of VNSNY Hospice. Works under moderate supervision. Education: Associate's Degree in Health Care, Business or a related field or the equivalent work experience required. Experience: Minimum of two years of experience in a business, operational or customer service role in a healthcare setting, preferably Hospice, required. Ability to understand medical terminology required. Experience entering data, navigating and retrieving information through a mainframe or similar computer system required. Ability to adapt to change and work in a team based environment required. Ability to be persuasive, negotiate to win-win outcomes and demonstrate exceptional customer service skills required. Demonstrated capability to gain commitment, build rapport with others (patients, families, physicians, clinicians, institutions etc.) required. Strong follow up skills required, as well as the ability to manage multiple priorities. Knowledge of managed care, Medicare/Medicaid and insurance authorization/billing helpful but not required. Bilingual skills may be required, as determined by operational needs.
Provides direct psychosocial services to mentally ill adults and/or children in the community who are experiencing psychiatric and/or psychosocial difficulties. Utilizes the Needs Adapted Treatment model of care. Applies psychotherapeutic and family systems theories and techniques in the delivery of professional services to participants and their families of choice. Assists in the overall functioning of the Parachute Program. Works under general supervision. Licensure: Valid driver's NY State license may be required, as determined by operational/regional needs. Current registration to practice as a Licensed Master Social Worker in New York State required. Education: Master's degree in Social Work required. Completed Needs Adapted Treatment Model (NATM) training or must complete within the first nine months of employment. Experience: Minimum of one year as a social service provider with clients in a mental health setting required. Bilingual skills may be required, as determined by operational needs.
