PURPOSE AND SCOPE:
The registered professional nurse (CAP RN 1) position is an entry level designation into the Clinical Advancement Program for Registered Nurses. The CAP RN 1 is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients.
As a member of the Chronic Kidney Disease (CKD) health care team, the CAP RN 1 participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations.
Performs all essential functions under the direction of the Clinical Manager (CM) and with the guidance from the Educator, Preceptor or a CAP RN 2, 3, 4 or 5 for the first 6 months of employment.
Expected to complete requirements to advance to the next CAP level, for which they qualify, within 24 months from date of hire.
Performs ongoing, systematic collection and analysis of patient data pre - during - post hemodialysis treatment for assigned patients and documents in the patient medical record, adjusts or modifies the treatment plan as indicated and notifies Team Leader, Charge Nurse, Clinical Manager or Physician as needed.
Recognizes aspects and implications of patient status that vary from normal and reports to or collaborates with appropriate health team members for input.
Assesses daily assigned patient care needs and collaborates with direct and ancillary patient care staff as needed.
Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of chronic hemodialysis therapy to assigned patients.
Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness.
Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
Ensures physician orders for assigned patients are entered and acknowledged in the Medical Record
Initiates or assists with emergency response measures.
Delegates appropriate tasks to direct patient care staff including but not limited to LVN/LPNs and Patient Care Technicians.
Monitors patient care staff for appropriate techniques and adherence to FKC policy and procedures.
Promotes infection control, equipment and environmental safety.
Assesses, collaborates and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family.
Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.
Ensures patient awareness related to transplant and treatment modality options.
Makes referrals to Social Worker and Registered Dietitian as appropriate for individual patient education and counseling.
Checks availability of adequate emergency equipment and supplies as assigned.
Assists with medication tracking and inventory as assigned.
Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.
Participates in all required staff meetings as scheduled.
Supports and promotes the FMCNA's mission, vision, values, and customer service philosophy.
Performs all other duties as assigned by supervisor.
PHYSICAL DEMANDS AND WORKING CONDITIONS:

SUPERVISION:

Successful completion of 3 months experience in chronic hemodialysis.
EDUCATION:
Graduate of an accredited School of Nursing.
Current appropriate state licensure.
Must meet the practice requirements in the state in which he or she is employed.
EXPERIENCE AND REQUIRED SKILLS:
Entry level for RNs with less than 2 years of nephrology nursing experience within the last two years or for RNs with a BSN and less than 1 year of nephrology nursing experience within the last two years.
Minimum of 9 months experience as a Registered Nurse (preferred).
Chronic hemodialysis experience (preferred).
Successful completion of FKC Education and Training requirements for new employees.
Successful completion and maintenance of CPR BLS Certification.
Meet all conditions of employment including Ishihara's Color Blindness Test.
Successful completion of all annual required education/training/competencies.
Successful completion of on-going training and education as assigned or required.
The rate of pay for this position will depend on the successful candidate's work location and qualifications, including relevant education, work experience, skills, and competencies. Hourly Rate: $36.00 - $57.00

Role Description

Project Execution
Work hands-on with the field staff to execute the project scopes of work, including, but not limited to:
Site prep and grading
Hardscape and masonry
Softscape installations
Irrigation, lighting, and drainage
Ensure both the quality and timeliness of work performed
Develop the skills of the team through on-the-job training
Demonstrate best practices for organization, care of equipment, work ethic, and client communication
Provide foremen with clear, executable plans for their jobs
Perform layout of job sites and properly set elevations and grades
Manage handoff between hardscape and softscape crews
Monitor crews for proper execution based on Outerland's SOPs through regular daily and weekly check-ins until project completion
Personnel Management
Hire, manage, and maintain a staff of high-performing team players who uphold the company values and contribute to the high performance of the construction team. Included within this responsibility is:
Hire staff to fulfill the needs of the construction team, while monitoring the crew's average wage to make sure it stays within budget
With the support of the PM, ensure all HR paperwork is completed in a timely manner in conjunction with the Director of HR. Paperwork includes
Manage staff's compliance with Outerland Policies, including but not limited to, attendance, safety, and care of equipment
Equipment and Fleet Management
Oversee the utilization of equipment to ensure it is properly used and maintained, and that equipment expense is carefully controlled within budget.
Manage the care of shared construction equipment
Verify that all equipment is being properly maintained through regular inspection
Ensure all foremen are completing DOT circle checks daily
Train all employees in the proper use and maintenance of equipment prior to use.
Snow Management
Fulfill a management role in snow removal. This includes working at all hours and days of the week as needed.

The Manager of the Clinical Resource Department is responsible for the day to day operations and clinical management of the Case Management, Utilization Review and Social Work, with a secondary focus on the Clinical Documentation team. This position will oversee functions of the Clinical Resource Department as they pertain to discharge planning, utilization management, and care coordination/high risk patient process while maintaining the integrity of daily operations. This position is responsible for developing and conducting ongoing education and process improvement for Case Management/Social Work staff.
PRIMARY DUTIES AND RESPONSIBILITIES:
Day to day management of the case management and social services program staff and workflows, including scheduling and performance evaluations. Assists the Manager of Operations with the Clinical Documentation team where necessary
Rounds on patient units on a regular basis to identify problem areas and assist in the facilitation of the discharge process
Ongoing development and completion of continuing education programs for Clinical Resources Management staff, including case managers and social workers specifically related to role functions and implementation of new processes.
Provides ongoing program education for new staff, including new Case Managers, the Trainee program hires, social workers, physicians, nurses and allied health professionals as it relates to discharge planning and utilization review.
Tracks and trends Clinical Resource Management program compliance. Creates a dashboard to see where opportunities for improvement are and implements process improvement activities around areas identified.
Develop, mentor and coach continual improvement in the collaborative practice between physicians, nurses and clinical resource team, and within the clinical resource department itself
Analysis and review of program metrics using working knowledge of the systems used in the departments.
Stays current with regulatory changes in the case management field.
Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization's culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence.
EDUCATION/EXPERIENCE/TRAINING:
Bachelor level degree in Nursing required.
Master's Degree preferred
Current Massachusetts Registered Nurse license
Certification in Case Management or Clinical Documentation Integrity preferred
Two years' experience in process improvement
Minimum 5 years Case Management experience in an acute setting.
Experience in development and presentation of professional continuing education programs
Knowledge of CMS regulations
Excellent computer skills - familiarity with Windows based software programs
Organizational, analytical, writing and interpersonal skills

The Operations Superintendent position is a key member of the management team and reports to the Utility Manager. The Kennel Assistant will
* Assist the client care team by answering telephones and helping callers with questions, emergencies, or any need they may have
* Admit and discharge patients to the hospital efficiently while maintaining the hospital flow.
* Restrain pets for medical treatments and diagnostics according to doctor orders.
* Ensure treatment areas, exam rooms, and labs are clean, stocked, and organized to ensure they are ready for incoming patients and emergency needs.
* Assist with radiographs and ultrasounds.
* Disinfect pet cages after use
* Perform in-house blood work using the appropriate lab machines and prepare samples for outside lab processing.
* Laundering and restocking patient bedding using proper cleaning protocols and maintaining hospital cleanliness
* Be willing to cross-train in other areas when necessary.
