Supervision may be direct (on-site) or indirect (available by phone), depending on the complexity of the task and setting. Consults with the client and multi-disciplinary treatment team about treatment options and preferences. Monitors progress of the person-centered or family centered ITP for all psychiatric department clients Coordinates care (routine, urgent, crisis) among primary healthcare, behavioral healthcare, dental, pharmacy, and specialty services inside and outside the agency, including, but not limited to: Scheduling and monitoring appointments Scheduling follow-up services and post-hospitalization appointments Lab work Prescription prior authorization, appeals, adherence Secure releases of information from past or current providers and obtains relevant information Facilitates and monitors internal and external treatment referrals.
