The program is focused to aid in serving the oral healthcare needs for the underserved population, with emphasis on children. Although this role is noted as WFH doing telephonic outreach and closing care gaps over the phone. The expected travel will be occasional for an event and outreach support. Locations include but are not limited to: Dauphin, Lebanon, Berks, Schyukill & Northumberland. Job Duties: Applies protective topical agents, sealants and fluoride and related procedures within the scope of practice for a Public Health Dental Hygiene Practitioner. Assesses dental condition and needs of member using patient screening procedures including medical history and medications. Contacts parents and guardians via telephonic outreach to provide oral health educational and counseling. Develops dental health program and strategy in conjunction with Quality Improvement, Medical Management staff and Dental Benefit Vendor within the various areas identified as having gaps in coverage. Develop outreach strategy. Maintain records of all outreach and communications. Provide education material and effective outreach to identify eligible members as well as work with private practice pediatricians, Federally Qualified Health Centers and Rural Health Clinics, or schools to promote oral health care and overall wellness. Refers members for follow up with a dentist for an annual dental exam and assists member to obtain an appointment with a local dentist. Work with local agencies such as WIC, head start, nurseries, pre-schools, and child care programs and other agencies in the target areas to identify parents and programs for offering education, enrollment and treatment opportunities; provide comprehensive age appropriate education programs related to all aspects of good dental care; work with the community navigators to identify schools and agencies to recruit for our education and enrollment treatment programs. Schedules for and attend community-based events. Work is typically performed in a clinical environment.     The Central Texas VA Health Care System, Temple, Texas is seeking a full-time Podiatrist (Home Base Primary Care Medical Director) to provide clinical oversight of all Non-Institutional Care (NIC programs to include all Home and Community Based Services, Contract Nursing Homes and State Veterans Home Programs) in collaboration with NIC program Manager. The HBPC Medical Director is the senior clinician leader and is responsible for the overall medical care delivered by the HBPC team. He/she works in collaboration with the HBPC Program Director to meet the requirements of the program. The incumbent may have a faculty appointment and be involved in academic activities with affiliated Medical Schools, residency and fellowship programs. The incumbent provides patient care directly and/or through the supervision of licensed independent practitioners (LIPs), residents, fellows and/or medical students in the home setting. In addition to their role as clinician, the HBPC Medical Director will have administrative responsibilities for leadership and direction of HBPC development and programs, as well as effective interface with the HBPC staff. The HBPC Medical Director will participate fully in all accreditation processes, reviews, and site visits. The incumbent will provide direct patient care which includes assessments, diagnosis, treatment, and outcome measures of healthcare problems. This includes the following: initiating and adjusting therapeutic regimens, performing histories and physicals, ordering diagnostic and therapeutic tests/studies, initiating consults and referrals, and prescribing medications/supplies in accordance with VA policy, VA formulary, Federal & state regulations, and scope of practice.
