**The Position**
This position is responsible for providing treatment to patients in the facilities served by working with the DDS, clinical coordinator, and staff at facility. Registered Dental Hygienists complete on-site hygiene services, as well as work with their clinical support managers and staff at nursing homes to ensure that the patients and facilities needs are met.
**Essential Duties & Responsibilities**
* Review patients lists prior to visit and ensure all supplies/equipment are available for all patients.
* Arrive on time, check in with dental contact, and review list of patients to be seen.
* Transport, set-up and take down equipment and supplies in room designated as workspace by facility.
* Work with contacts at nursing home to coordinate transfer or patients and their chart to and from the treatment room. If issues with transport of patients/charts and all options have been exhausted, call clinical support manager for assistance.
* Perform required care including taking X-rays, cleaning teeth and dentures, applying sealants, assess patient's oral heath to report finding to dentist, following all ADA and Company standards of care.
* Explain procedures as they are completed to patient. Follow up with staff at facility to review recommendations for patient.
* Enter chart notes for treatment provided into Salesforce at the time of treatment.
* Ensure that OSHA safety and cleanliness regulations are followed during patient treatment.
* Ensure that disposables are handled correctly before leaving facilities.
* Check out with dental contact at facility, including review of chart notes. Leave printed copies of chart notes with facilities for patient charts.
* Work with clinical support manager on schedule and follow up of patient care and treatment.
* Maintain dental equipment through regular cleaning and maintenance routines.
* Inventory supplies and order as needed.
* Follow regulated OHSA guidelines in handling and sterilization of instruments.
* Utilize personal vehicle for equipment/supply transport and traveling to scheduled facility visits.
* Be available via cell phone during workday.
* Willing and able to expense purchases for office needs, unforeseen supply purchases and other reimbursable business-related expenses.
**Location**
This position covers their home market which includes driving up to a 3-hour radius. For locations greater than 2.5 hours, the company offers overnight lodging and meals. Mileage reimbursement is included, as well as paid time for all driving that exceeds normal commute time.

* Current Registered Dental Hygienist license in the state in good standing and 5 years' experience.
* Valid Driver's License with no moving violations in the last 5 years.
* Willing to travel within a 3-hour radius.
* Must have own reliable transportation.

* Ability to lift equipment weighing up to 50 pounds in and out of your vehicle and roll in and out of home/facilities.

Position Summary: The Dental Hygienist is accountable directly to the Director, Dental. The Dental Hygienist performs clinical oral hygiene procedures as delegated and directed by a dentist in accordance with department protocols, accepted professional practice and in compliance with applicable DC law. The Dental Hygienist documents all relevant patient information and service in the dental practice management system. This is a full-time position.
The salary for this position ranges between $95,000 - $105,000 annually and the offer amount is determined by the candidate's education, qualifications, and experience. Indeed provides their own estimated salary calculator and is not affiliated with COH's range.
Highlighted Duties and Responsibilities:
* Performs patient screening procedures; such as assessment of oral health conditions, review of the health history, oral cancer screening, head and neck inspection, dental charting and taking blood pressure and pulse;
* Records patient-hygienist transactions as they occur in the patient's dental record so that the dental record accurately and completely reflects the nature of the contact, the condition of the patient and the care or treatment provided;
* Integrates appropriate patient dental hygiene care with other health care professionals involved in the patient's health care management.
* Removes calculus and plaque (hard and soft deposits) from all surfaces of the teeth via use of a cavitron, piezo or by hand instrumentation.
* Performs taking and mounting diagnostic dental radiographs (x-rays);
* Applies preventive materials to the teeth (e.g., sealants and fluorides);
* Counsels patients about good nutrition and its impact on oral health;
* Participates in the Quality Improvement program of the Dental Department of the center;
* Teaches patients appropriate oral hygiene strategies to maintain oral health; (e.g., tooth-brushing, flossing and nutritional counseling); Incorporates different modalities to educate patients about proper oral care including but not limited to distribution of educational pamphlets, playing videos, demonstration on dentiform, etc.
* Coordinates with Dental Director to maintain and update provider schedules as needed
* Provides coordination with the Dental Office Manager as needed.
* Maintains ongoing training and licensing and other credentials as required by DC and COH;
* Remains compliant with Community of Hope policies and state and federal regulations governing confidential information, including HIPAA and HITECH.
* Completes and locks all progress notes within 3 days after patient visit.
* Documents all patient encounters, including face to face, telephone and electronic communications in the health record of the patient in a timely fashion.

Requirements
Minimum Qualifications:
* Diploma from an accredited dental hygienist program required;
* Requires an active DC Dental Hygienist license and Local Anesthesia Certification.
* 2 years experience in a dental office practice preferred;
* CPR required;
* Must have the ability to work in a non-profit environment and a commitment to work with under-served populations.

As part of the WorkCare Incident Intervention team, contract physicians help support WorkCare's 24/7 Incident Intervention program. The primary responsibility of this position is to facilitate appropriate and timely health care with the focus on employee return-to-work and return to maximum functionality. This is accomplished by conducting telephonic consultations with the injured worker. The physician conducts screening evaluations of workplace injuries and provides an assessment and guidance to the injured worker. This allows the employee to receive the right care at the right time in the right setting.
Our Occupational physicians have knowledge of workplace hazards, the workers' compensation systems, as well as clinical medicine and may assist clients by contacting the receiving clinic or emergency department to be a resource to the receiving medical facility. Peer to peer conversations can help relay clinical information in form of a hand off and communicate other medical concerns, red flags in the case, or work-relatedness concerns. We also may receive information regarding the outcome of the clinical visit that can be communicated with the client (i.e. work-restrictions or other OSHA recordable results).
Essential Duties and Responsibilities
Contract Physicians utilizes evidence-based medical guidelines employs best practices that require early intervention and setting of realistic expectations for recovery and return to work. This position is responsible for performing injury management consultations, peer-to peer communication, initial injury intake, and other client services.
Injury Management “On-Call”
Triage employees to appropriate level of care based on symptoms, history, photos, and clinical judgement.
Provide first aid suggestions, if able to keep at self-care.
If clinic visit is recommended, provide recommendations of clinic type and timing.
Provide timely peer-to-peer calls.
Provider to Provider contact to discuss injured employee and/or injured employee return to work.
Document all patient transactions in accordance with company policies and procedures in appropriate electronic format as required.
Use appropriate systems per policies and procedures to provide service during scheduled shift.
Assure that an alternate physician is available to cover if unable to fulfill a scheduled shift, unless an emergency.
Communicate and work with medical staff to ensure coordinated efforts for the provision of high-quality medical triage and client service.
Assist Clinical Lead in identifying, analyzing and resolving work problems.
Evaluate the effectiveness of existing clinical methods and procedures and suggest improvements.

Education and Experience

* Assists dentist with all DA functions as outlined by the Dental Board of California
* Takes Quality radiographs and organizes them in the digital x-ray system.

* Excellent collaboration skills to work seamlessly with the clinical team and support staff for a seamless patient experience
