Lodging is provided for visiting providers while they are offering services in Yakutat. Scope of Services Yakutat Community Health Center (YCHC) is issuing this Request for Proposal (RFP) to solicit proposals from qualified Dentist service providers to deliver services to Children ages 0-17 in Yakutat, Alaska. The functions noted below must be performed consistently and expertly and include, but are not limited to: Preventive and restorative pediatric dental care, including fluoride varnish, sealants, exams, fillings, extractions, and emergency care. Collaboration with YCHC's hygienists and participation in school-based screenings or community outreach as appropriate. Frequency of Services: Quarterly, 5 full clinical days. Patient volume expectations are contingent upon staffing levels. YCHC currently has two dental assistants on staff, including one who is in training. Billing and Compensation Services YCHC will handle insurance billing. Providers must comply with becoming in-network with accepted insurance plans. Dentists will invoice YCHC for professional services at the agreed-upon daily rate. The contracted dentist will establish and maintain a collaborative agreement with YCHC's two licensed dental hygienists in compliance with Alaska Laws. Rules Governing Proposal Proposers should carefully examine the entire RFP and become fully aware of the nature of the work and conditions likely to be encountered. All proposals must be complete and irrevocable for 60 days following submission. Proposals must include a dated signature and be received by YCHC before the deadline. Emailed proposals are accepted. Proposal and Submission Requirements To ensure a uniform review process, proposals should include Cover Page, Services to be performed, Experience, References, Methodology and Approach, Payment Rate, Addenda acknowledgment, Debarment/Suspension Statement, and Authorized Signature.
