INTRODUCTION
Yakutat Community Health Center (YCHC) is issuing this RFP to solicit proposals from qualified vendors who agree to provide professional general dentist services to patients as designated by our organization and in accordance with generally accepted industry standards. Services will include diagnosis, treatment and follow-up care, with an emphasis on high-quality, culturally responsive care.
Background
The Yakutat Community Health Center is a 22,000 sq-ft health center which provides primary and ancillary healthcare to the Yakutat Service Area.
YCHC provides primary, urgent, and emergency care services and offers locum providers for ancillary and specialty services on a rotation basis. Lodging is provided for visiting providers while they are offering services in Yakutat.
Scope of Services
Yakutat Community Health Center (YCHC) is issuing this Request for Proposal (RFP) to solicit proposals from qualified Dentist service providers to deliver monthly dental services to approximately 380 patients in Yakutat, Alaska. The functions noted below must be performed consistently and expertly and include, but are not limited to:
Dental exams; restorative and prosthodontic care including crowns, bridges, dentures, and dental implants; bone grafting; simple and surgical extractions; treatment planning for implant placement and restoration; collaboration with YCHC hygienists and medical staff to ensure continuity of care; and provision of emergency dental services as needed. Frequency Services: At least 3 clinics per year, each consisting of 4 full clinical days.
Patient volume expectations are contingent upon staffing levels. YCHC currently has two dental assistants on staff, including one who is in training.
Billing and Compensation Services
YCHC will handle insurance billing. Providers must comply with becoming in-network with accepted insurance plans. Dentists will invoice YCHC for professional services at the agreed-upon daily rate. The contracted dentist will establish and maintain a collaborative agreement with YCHCs two licensed dental hygienists in compliance with Alaska Laws.
Rules Governing Proposal
Proposers should carefully examine the entire RFP and become fully aware of the nature of the work and conditions likely to be encountered. All proposals must be complete and irrevocable for 60 days following submission. Proposals must include a dated signature and be received by YCHC before the deadline. Emailed proposals are accepted.
Proposal and Submission Requirements
To ensure a uniform review process, proposals should include Cover Page, Services to be performed, Experience, References, Methodology and Approach, Payment Rate, Addenda acknowledgment, Debarment/Suspension Statement, and Authorized Signature. Dentists must hold an Alaska state license.
Evaluation Criteria and Process/Selection Process
Proposals will be evaluated based on the following criteria:

Past performance/ reference - 20 points
Presentation of proposal - 20 points
YTT Tribal Member/ Native Preference - 20 points
