What You Will Do
Collaborates with program management to ensure the provision of the highest quality health services to patients. Participates in the formulation and update of clinical and administrative policies and procedures and the preparation of policy and procedure manuals.
Collaborates with Medical Directors to maintain current evidence-based clinical policies, procedures, and standards of clinical practice. Supports the integration of evidence-based care practices for a specific program into protocols/policies, consultation strategies, staff development plans, continuous quality improvement initiatives and research for clinical nursing staff.
Ensures program services address patient needs. Establishes and communicates protocols and standards of care for a cultural and demographic diverse population; provides intervention guidelines based on these population health needs.
Analyzes utilization to ensure visits are made according to guidelines and clinical outcomes best practices. Ensures audits are done per regulatory requirements.
Participates on interdisciplinary and interdepartmental work groups that focus on new and existing clinical projects and programs that provide services to patients in the Palliative Care and Annual Wellness Visits.
Supports and provides comprehensive advanced nursing care guidance to ESPRIT clinical staff, including physical examination, comprehensive history, screening for physical and/or psychological conditions, emergent interventions, pharmacological and non-pharmacological interventions, ordering treatments and DME, preventative health maintenance activities, care management, referrals, discharge planning, counseling, and patient education.
Provides clinical supervision and oversight for field staff, including onsite observation of direct patient care to ensure quality of services. Qualifications
Licenses and Certifications:
Current ANCC or AANP certification as an adult, family or geriatric nurse practitioner required
Licensure as a Nurse Practitioner in the State of New York required

Education:

What You Will Do

The Biomedical Technician is responsible for overseeing the overall operational condition, maintenance and repair of all water treatment equipment, all medical equipment and mechanical/electrical systems to ensure patient safety and the safety of all staff operating such equipment for their assigned clinic(s).
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and tasks may be assigned.
Troubleshoot, repair and maintain equipment as recommended by the manufacturer.
Repair, calibrate, test, clean and disinfect dialysis machines, water distribution loop and RO's according to protocol.
Perform water quality checks to include but not limited to: free chlorine, total chlorine, and water hardness testing.
Perform safety checks and tests on all pretreatment water equipment including but not limited to carbon tanks, water softeners, RO units, UV lamps and ultrafilters.
Collect samples of RO water and dialysate for microbiology testing according to protocol.
Collect water samples for AAMI analysis testing according to protocol.
Disinfection of central bicarbonate system and mixing tank according to protocol.
Disinfection of RO, distribution system, and hemodialysis machines according to protocol.
Perform duties at all times within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations.
Perform electrical safety on dialysis machines and related equipment.
Be familiar with instruction manuals and operation for equipment in accordance with manufacturer's recommendations. Be capable of starting up and shutting down facility's equipment if necessary.
Maintain established preventative maintenance programs and required support documentation.
Maintain accurate maintenance records for the facility's equipment.
Have knowledge of the function and safe operation of water treatment equipment and related mechanical and electrical systems.

We are currently seeking a BC/BE glaucoma or cornea trained ophthalmologist to join our busy practice in Kingston and Rhinebeck, NY. We would also consider a comprehensive ophthalmologist with broad practice interests, including medical glaucoma, diabetic disease, and cataract surgery. Both offices are currently staffed by two comprehensive ophthalmologists and one medical retina specialist. We share group call coverage with our offices in Poughkeepsie, making for a 7 MD call rotation. A recently retired ophthalmologist with 30 years of practice in the community has left a large need for continuing care. The other three physicians on staff have a combined 40 years experience in the community.
Our department is supported by a team of clinical & non-clinical staff, including COAs and COTs. We have a strong referral relationship with our community optometrists and also work closely with our in-house regional subspecialty partners in Cornea, Glaucoma, Oculoplastics, Neuro-Ophthalmology and Retina. We have 8 exam lanes, the most up-to-date OCT technology, and imaging/angiography systems in each of our offices, as well as the latest IOL Master for our cataract surgery patients. We utilize our community hospital as well as a regional outpatient eye surgery center for surgical procedures. The surgery center offers Femto cataract surgery and ORA technology. We have the advantage of a shared EMR with our colleagues in Optum, on-site labs and imaging services for our patients, and coordinated care with primary care providers and other medical and surgical specialties in the group.

About the Position:
Plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation. Collaborates with interdisciplinary team.
Qualifications:
Current state license as a Physical Therapist.
Two years of experience as a Physical Therapist.
Current CPR and Hepatitis profile.
Knowledge of durable medical equipment.

Assist the DNS with management responsibilities and in maintaining the quality of care
Responsible for Wound/Skin Rounds weekly
Assist with compliance and ensure effective communication with all levels of nursing staff
Meet the nursing medical needs of all residents
24 hours on call
Coordinate and direct the total planning for nursing services
Maintain a comprehensive knowledge of general nursing theory and practice
Responsible for the accurate and timely completion of medical records
Assist with interviewing, hiring and orienting of new nursing staff
Oversee daily staffing process

Maintain flexibility with work schedule to address unpredictable needs
REQUIREMENTS:
Current State RN license required
Min. 3 yrs. exp. in Long-Term Care settings
2 years of management experience preferred

Evaluate and test individuals served and develop effective treatment plans while providing quality service.
Administer treatment programs for individuals served and communicate treatment needs to staff.

Maintain documentation including report of services, weekly schedule to meet therapy goals of staff and individuals served.

Collaborate with physicians, staff, professionals/paraprofessionals, consultants, families, and community resources to ensure effectiveness of occupational therapy program.
Qualifications:
Bachelor's Degree in Occupational Therapy and one-year human services experience
One year experience in Neuro-rehabilitation preferred
Must be licensed OT/L in state and a member in good standing with American Occupational Therapy Association (AOTA)
Current driver's license, car registration, and auto insurance
