The Summer Youth Leadership Program is a summer employment opportunity for the City of Norwalk residents. The youth participates in leadership and job skills development programs and actively participates in weekly workshops related to job training and leadership programs. They attend trips geared towards team building and relationship development. Performs various unskilled tasks involved in the maintenance, custodial services, or upkeep of City buildings and facilities, parks and recreation buildings, grounds, and open spaces; performs related duties as required. ESSENTIAL DUTIES Depending upon assignment, essential duties include but are not limited to, the following: Public Services Learned the tasks and care required to maintain city facilities, parks, and infrastructure. * PARKS - Maintains and cleans parks and open space areas around City buildings and park facilities, including picnic areas, barbecues, restrooms, and athletic fields; performs routine landscaping, including planting, trimming, weeding, and fertilizing; assists with graffiti removal in parks. * FACILITIES - Provides custodial services for facilities, including floors, cleaning restrooms, and removal of trash; sweeps, mops, scrubs, strips, polishes, and waxes floors; cleans rugs and carpets; dust, polishes, and waxes furniture and woodwork; cleans and stocks restrooms; empties and cleans waste receptacles and ashtrays; washes walls, windows, mirrors, and blinds; moves furniture or other heavy objects for set up of City functions. * STREETS - Cleans up work sites upon completion of jobs; observes safe work methods and safety practices, including the coning and flagging at work sites to secure them from traffic. Administration Learned the fundamental elements of City Government and operation. Transportation Will have exposure to transit operations for the City's Transportation Division Public Safety Learn the procedures of the Public Safety office and basic information about how Public Safety operates within the City of Norwalk. Social Services/Senior Center Will be introduced to various City Services, including Senior Center Recreational and Social Services. Recreation Will assist with facility operations and park programming, interacting with families at the parks and various Recreational facilities. OFFICES Assists with routine clerical tasks, such as photocopying, filing, and typing. This position provides case management, counseling, life skills training, referrals to educational and employment and medical resources. This position closely supervises a staff of 4 to 5 Youth Advisors to ensure that residents receive the structure, attentiveness and training needed to become productive, independent adults. ESSENTIAL DUTIES AND RESPONSIBILITIES Service Delivery Monitor the progress and activities of transitional housing youth by conducting weekly case management meetings. This will assist the youth to establish: Employment or other income eligibilities Budgeting skills School enrollment (if applicable) Connections to medical, mental health, substance abuse resources as applicable Affordable housing or permanent housing Promote improved basic living skills amongst residents by: • Coordinating weekly life skills groups. • Assisting residents to complete housing and employment applications. • Coordinating tutoring for residents who are in need of assistance with homework assignments. • Coordinate community partner engagement and awareness. Promote positive resident interactions and healthy living environment by: • Initiating resolution intervention techniques to handle conflicts between residents as needed. • Promote good health, in case of illness, and ensure that residents receive the proper medical attention. • Promote and maintain a clean work environment and ensure that office area and equipment cleaned daily. • Work with the youth to ensure they are partnered with other local • organizations to ensure they have healthy community and social involvement. Adult Protective Services (APS) is a state-mandated program administered by the City and County of San Francisco (CCSF), HSA, and DAS. Under general supervision by the APS program, the Protective Services Worker (PSW) receives and responds to reports of suspected “elder” and “dependent adult” abuse including neglect, self-neglect, and financial abuse. The PSW performs professional investigations and case management services to victims of abuse. This includes activities related to evaluating the level of harm or risk of harm, to the person and the estate of an “elder” or “dependent adult;” assessing biopsychosocial functioning, including the medical condition, cognitive/psychological status, and adequacy of care and support system; orchestrating ongoing services to maximize the use of community resources, and to help protect victims from further harm once the case is closed. Conducts unannounced visits to see elders and dependent adults in their living environment. Investigates the allegations and conducts a risk assessment through interviewing the victim, family, relatives, neighbors, and others to assess for immediacy. Conducts a needs assessment for clients in need of protective services. Develops a Service Plan and organizes the initial care plan as appropriate, including supported decision-making activities. Provides counseling for clients and significant others to alleviate the identified problems and to implement the service plan to restore optimum social functioning, and health adjustment within the client's capacity; assists them in order to address personal and environmental difficulties, which perpetuate poor health and dysfunction, and interfere with self-care and self-protection. Monitors the Service Plan and follows up through field visits and telephone contacts with clients, relatives, and other concerned individuals until the service goals have been achieved. Assists with multidisciplinary team meetings with healthcare providers, hospitals, and the Public Health/Mental Health Department, and other City agencies involved to further their understanding of significant social and emotional factors underlying the client's health, social, or environmental problems. Job Description Under general supervision, positions in class 2940 Protective Services Worker within the Office of the Public Conservator in the Department of Aging and Adult Services provide a broad range of clinical/advocacy services to temporary/permanent conservatees and collateral contact with a multidisciplinary team. In general, an average caseload is approximately 65 clients and can be more or less depending on case intensity and program capacity. The target population of this office is individuals who are gravely disabled, (unable to provide for food, clothing and shelter as a result of chronic mental illness and/or chronic alcoholism), as a result of major mental illness as defined by the Diagnostic Statistical Manuel (DSM). The population also includes individuals who may have involvement with the Criminal Justice System and are being held in jail and State Hospitals or other facilities designated for those who have committed, or are alleged to have committed a crime. The Office of Conservatorship Services has an average caseload of 700, with approximately half of these individuals placed outside of San Francisco County in either locked psychiatric facilities or residential care homes. Therefore travel is required in order to meet the needs of the conservatee and our service mandate by State and County regulations. The average number of miles driven per month is 350. The 2940 is responsible for managing an independent caseload and is assigned mental health (LPS) conservatorship investigations as well. This involves interaction with Mental Health Court, including Public Defender and District Attorney, Department of Public Health, hospitals, jails and community agencies. In addition to assessments and making recommendations for levels of care, the conservator is also responsible for advocating these recommendations, which may at time be at conflict with the interests of other stakeholders. Basic computer skills are required to perform charting on electronic systems (Avatar and Panoramic), emailing (the bulk of inter and intra agency communication is done through e-mail). Role description Under general supervision, positions in class 2940 Protective Services Worker within the Office of the Public Conservator in the Department of Aging and Adult Services provide a broad range of clinical/advocacy services to temporary/permanent conservatees and collateral contact with a multidisciplinary team. In general, an average caseload is approximately 65 clients and can be more or less depending on case intensity and program capacity. The target population of this office is individuals who are gravely disabled, (unable to provide for food, clothing and shelter as a result of chronic mental illness and/or chronic alcoholism), as a result of major mental illness as defined by the Diagnostic Statistical Manuel (DSM). The population also includes individuals who may have involvement with the Criminal Justice System and are being held in jail and State Hospitals or other facilities designated for those who have committed, or are alleged to have committed a crime. The Office of Conservatorship Services has an average caseload of 700, with approximately half of these individuals placed outside of San Francisco County in either locked psychiatric facilities or residential care homes. Therefore travel is required in order to meet the needs of the conservatee and our service mandate by State and County regulations. The average number of miles driven per month is 350. The 2940 is responsible for managing an independent caseload and is assigned mental health (LPS) conservatorship investigations as well. This involves interaction with Mental Health Court, including Public Defender and District Attorney, Department of Public Health, hospitals, jails and community agencies. In addition to assessments and making recommendations for levels of care, the conservator is also responsible for advocating these recommendations, which may at time be at conflict with the interests of other stakeholders. Basic computer skills are required to perform charting on electronic systems (Avatar and Panoramic), emailing (the bulk of inter and intra agency communication is done through e-mail). Adult Protective Services (APS) is a state-mandated program administered by the City and County of San Francisco (CCSF), HSA, and DAS. Under general supervision by the APS program, the Protective Services Worker (PSW) receives and responds to reports of suspected "elder" and "dependent adult" abuse including neglect, self-neglect, and financial abuse. The PSW performs professional investigations and case management services to victims of abuse. This includes activities related to evaluating the level of harm or risk of harm, to the person and the estate of an "elder" or "dependent adult;" assessing biopsychosocial functioning, including the medical condition, cognitive/psychological status, and adequacy of care and support system; orchestrating ongoing services to maximize the use of community resources, and to help protect victims from further harm once the case is closed. Job Description Under general supervision, positions in class 2940 Protective Services Worker within the Office of the Public Conservator in the Department of Aging and Adult Services provide a broad range of clinical/advocacy services to temporary/permanent conservatees and collateral contact with a multidisciplinary team. In general, an average caseload is approximately 65 clients and can be more or less depending on case intensity and program capacity. The target population of this office is individuals who are gravely disabled, (unable to provide for food, clothing and shelter as a result of chronic mental illness and/or chronic alcoholism), as a result of major mental illness as defined by the Diagnostic Statistical Manuel (DSM). The population also includes individuals who may have involvement with the Criminal Justice System and are being held in jail and State Hospitals or other facilities designated for those who have committed, or are alleged to have committed a crime. The Office of Conservatorship Services has an average caseload of 700, with approximately half of these individuals placed outside of San Francisco County in either locked psychiatric facilities or residential care homes. Therefore travel is required in order to meet the needs of the conservatee and our service mandate by State and County regulations. The average number of miles driven per month is 350. The 2940 is responsible for managing an independent caseload and is assigned mental health (LPS) conservatorship investigations as well. This involves interaction with Mental Health Court, including Public Defender and District Attorney, Department of Public Health, hospitals, jails and community agencies. In addition to assessments and making recommendations for levels of care, the conservator is also responsible for advocating these recommendations, which may at time be at conflict with the interests of other stakeholders. Basic computer skills are required to perform charting on electronic systems (Avatar and Panoramic), emailing (the bulk of inter and intra agency communication is done through e-mail).
