Assumes primary responsibility for coordinating and performing services necessary to provide socialization skills. 1) Conduct interviews and evaluate prospective residents to determine appropriateness; tour families and applicants and oversee the complete admission process for all new admissions. 2) Works with residents to encourage healthy connections with family and community as well as develop new interests and form new support systems. 3) Maintains accurate and timely records of case management interventions, appointments, recommendations and follow-up in each resident's file. This includes developing of a Person-Centered Service Plan upon admission and reviewed/updated every 6 months and as needed in conjunction with the resident's responsible party, resident's physician and home care agencies. 4) Provides one-to-one support for residents related to emotional, behavioral, social and emotional concerns. 5) Works with the Resident Care Director to ensure that the residents obtain/maintain a primary physician of their choice and have access to other services as needed to maintain their optimal health/mental health. 6) Actively networks and maintains links with other health care and human service organizations. Make appropriate referrals according to residents' needs. 7) Assist residents in need of alternate living arrangements to make and execute sound transfer and/or discharge plans. 8) Assists with annual rent increases, coordinating paperwork along with the administrator. Coordinates paperwork when there is a room/rent change. 9) Perform other duties as assigned and is available as needed to ensure that all residents' needs are ESSENTIAL FUNCTIONS: Ability to communicate with residents and staff in English. Ability to read and write English. Ability to lift up to 30 pounds unassisted. Ability to visually observe residents, to hear emergency systems and verbal needs expressed by residents. Ability to turn, stoop, bend and stretch in order to assist residents. Ability to stand for prolonged periods. Ability to use equipment competently. Ability to respond to emergencies, i.e. resident choking, evacuation of residents and visitors in the event of fire. Qualifications Must be at least 18 years of age. Bachelor's degree with major course work in human services plus one (1) year full-time work experience in a related field; or an Associate's degree with major course work in human services plus three (3) years of full-time work experience in a related field, or a High School diploma or GED plus five (5) years of full-time work experience in a related field Well-developed organizational skills. Ability to work effectively with inspectors, monitoring and licensing agencies. Must have the required annual Tuberculosis (TB) testing and annual Health Assessment completed Must have the required references and criminal record check. Must have a valid New York State driver's license that meets agency's clean driving record policy.
