Responsibilities
Area of Responsibilities:
Develops the overall Residential Service Plan with the resident and his or her support network by identifying the individual's needs and desires in an effort to maintain placement and / or progress to more independent housing.
Provide teaching, monitoring and recovery oriented supports in accordance with OMH 595 and 593 including the following rehabilitative services: Assertiveness / Self-Advocacy Training, Community Integration / Resource Development, Daily Living Skills, Health Services, Medication Management / Training, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, Symptom Management.
Ensure that each resident receives comprehensive planning in a person centered approach for; admission (including the completion of admission notes), on-going recovery oriented services, and discharge. Ensure that each plan is complete and up-to-date in accordance with standards of practice and the New York State Office of Mental Health.
Participates in monthly meeting with Primary Counselor and Resident to monitor recovery efforts, progress and changes needed in plans and ensure delivery of services in accordance with each resident's service plan.
Conducts at minimum quarterly reviews of the Residential Service Plan with the individual in an effort to ascertain the degree of progress and to make revisions when necessary.
Provide guidance and oversight to Counselors to ensure Counselors observe, monitor, document, and report resident behaviors as well as their progress in accordance with agency standards.
Provide guidance and oversight to Counselors to schedule or ensure that annual appointments for physicals, eye exams, and dental appointments are scheduled. Ensures that transportation is arranged for these appointments (when appropriate).
Works collaboratively with the resident's community service providers to ensure continuity of care (care managers, clinical, medical, educational, vocational, and entitlement specialists, etc.). Makes appropriate referrals to these services when needed.
Serves as the key liaison between the resident and his / her clinical provider with regard to the monitoring of the individual's mental health and well-being. Works with the Primary Counselor to keep the resident engaged in clinical services through regular attendance.
Help residents to plan for the purchasing of clothes and other necessities.
Works with residents to encourage a healthy connection with family and community ties and to develop new supports and interests.
Respects and maintains resident confidentiality and demonstrates a caring, positive attitude toward all residents, staff, guests, and service providers.
Provide safe transporting of residents when needed.
Follow all safety rules and regulations for self, residents, and staff.
Performs any other duties necessary for the effective operation of the program.
Qualifications
Education & Experience: QMHS Standard Bachelor's degree in Human Service field
or
an Associate's degree in Human Service field plus three years of FT related experience
or
a High School Diploma plus five years of FT Human Service experience. This position also requires a clean NYS driving record as outlined in DePaul's personnel policy. Must be at least Eighteen (18) years of age.

Responsibilities
The Care Transition Coordinator's primary responsibility is to facilitate a seamless transition for patients

SPECIFIC DUTIES & RESPONSIBILITIES:

THE OPPORTUNITY
The Staten Island Justice Center (SIJC) seeks to re-engineer the experience of criminal court in Staten Island, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Richmond County Criminal Court along with community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant's life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling.
SIJC seeks a Case Management Coordinator for the borough's Supervised Release Program (SRP). Reporting to the SRP Director of Practice, the Case Management Coordinator is responsible for employing quality assurance measures to ensure the program is adhering to the program model and employing best practices aligned with organizational values, including a commitment to Operational Excellence. The Case Management Coordinator will provide direct supervision to Supervising Case Managers and group supervision to Case Managers and Senior Case Managers. Additionally, they will ensure that case management staff regularly engage in program-specific training and receive consistent and appropriate task supervision. The Case Management Coordinator will facilitate effective communication between this team and across all other SRP teams to ensure that the program builds on successes and proactively addresses challenges.
This position leverages significant direct practice and supervisory experience to support staff in the execution of their responsibilities. Through trauma-informed supervision, case conferencing, and interdepartmental communication, the Case Management Coordinator will ensure case management and peer staff adhere to the program model through a trauma-informed lens. Critically, this position is responsible for ensuring that staff maintain accurate and up-to-date records in the Center's SRP data management system and submit accurate and timely compliance reports for each participant supervised by the program.
Additionally, the Case Management Coordinator is responsible for the implementation of new policies and protocols, with the goal of supporting staff to deliver the best possible services to participants enrolled in the Supervised Release Program.

Responsible for quality assurance, monitoring the team's compliance with program requirements and court reporting obligations;

Co-facilitate and participate in required de-escalation and crisis intervention training;
Work closely with the SRP Court Reform Training Specialist, Training Institute, and site Training Liaison to ensure SRP staff are consistently trained as required;
Support the recruitment and hiring of and onboarding and training for new case management staff;
Ensure regular and equitable access to program-related external professional development and training opportunities for all staff;

Qualifications:

Responsibilities
Works with the Management staff to provide perspective residents with tours and lunch visits and then provides input to the Admission / Management team with regard to the individual's acceptance in to the program.
Ensure that each resident receives comprehensive service planning in a person-centered approach during the resident's tenure withthe program. This includes the delivery of recovery-oriented services and on-going discharge planning practices. Ensure that each plan is complete and up-to-date in accordance with standards of practice and the NYS Office of Mental Health (OMH).
Conduct / complete quarterly service plan reviews with the resident and his / her support team in an effort to ascertain the resident's progress and to make revisions / updates to the plan when necessary.
Provides teaching, monitoring and recovery-oriented supports to residents in accordance with OMH 595 including the followingrehabilitative services; Assertiveness / Self Advocacy Training, Community Integration / Resource Development, Daily Living Skills Training, Health Services Education, Medication Management and Training, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services and Symptom Management.
Observe, monitor, document and report resident behaviors as well as their progress in accordance with agency standards.
Maintain accurate and timely resident files, including the documentation of progress notes, the updating of service plans, the maintenance of psychiatric and medical records, as well as other pertinent information relating to the resident.
Ensures that annual appointments for physicals, eye exams, medical specialist appointments (such as OB / GYN appointments) and dental appointments are scheduled and ensures that transportation is arranged for these appointments when necessary.
Works collaboratively with the Resident's Health Home Care Manager and other community service providers to ensure continuity of care for each individual.
Serves as a key liaison between the resident and his / her clinical provider with regard to the monitoring of the individual's mental health and well-being.
Provides direct assistance to the residents with regard to daily living skill training including the upkeep of resident rooms, training in the areas of preparing simple meals and assistance with laundry when indicated.
Helps residents plan for the purchasing of clothes and other necessities.
Works with residents to encourage a healthy connection with family and communities and to develop new supports and interests.
Coordinates and participates in resident's recreational activities when indicated.
Assists the Medication Coordinators with the supervision of resident medications when necessary and assumes dining room responsibilities during breakfast when assigned.
Provide safe transporting of residents when necessary.

Education and Experience
